Description
The invention relates to medicine, in particular to ophthalmology, and can be used for surgical treatment of eye cataracts.
Despite the well-known achievements in the field of ophthalmology, there are still a number of unsolved problems in the surgical treatment of patients with cataracts. Therefore, it is not surprising that for a long period, and especially in recent years, all new and new developments related to the improvement of cataract extraction methods appear. However, without looking at numerous proposals and nuances, it is still possible to single out the main stages of cataract extraction, which, as a rule, are preserved in one form or another in all methods of treatment of this eye pathology that are offered. These include the following: dissection of the fibrous capsule of the eye, sectioning of the anterior capsule of the lens, removal of the lens nucleus and masses, installation of an artificial intracapsular lens, centering the lens relative to the optical axis of the eye, and sealing the wound.
One of the most relevant and difficult stages of cataract extraction is the removal of crystalline masses from the capsular bag. It is at this stage that the surgeon has to face various situations, which force him to use different methods, techniques and tools each time, which ensure, in aggregate, minimal trauma to the eye. This is due to the fact that the consistency of the crystalline substance changes with the age of a person from a liquid state to a solid state. Negligence or inaccuracy in the manipulation of the instrument during the removal of crystalline masses often leads to damage to the capsule, cornea, iris during surgery, postoperative complications, repeated cataracts, etc. That is why ophthalmologists from different countries pay special attention to the development of technologies for removing crystalline masses.
In recent years, along with the traditional surgical extraction of cataracts, more progressive methods of treating this pathology using ultrasound or lasers have been used. However, it is not always possible to achieve success when using these methods of cataract extraction. This is due to the fact that, for example, the time of exposure to ultrasound should be limited 3-due to its traumatic nature, and ultrasonic vibrations are attenuated as they move away from the source of their generation, which forces the latter to constantly move inside the capsule, and also the vibrations have a negative effect on intraocular structures. Laser exposure is less traumatic, but it also has its drawbacks.
In the literature, there are descriptions of a number of complications that arise when applying the laser method of cataract extraction in practice: reactive hypertension, epithelial-endothelial dystrophy, iridocyclitis, cystic edema of the retina in the macula, retinal tears, retinal detachment, etc. The occurrence of these complications is due to the use of laser pulses with high single energy or a large number of pulses with high total energy.
It is also necessary to note the fact that all methods of cataract extraction using laser technology are difficult to implement in practice (special caution is required when using them, since an overdose of energy leads to additional traumatism), long in time, which also increases the likelihood of postoperative complications, as well as laser installations have a significant cost and require the use of an additional source of energy, which generally has a negative impact on the cost of cataract treatment.
Therefore, despite some progress in the field of creating laser technology, the methods of cataract extraction are most often used in practice, in which tools are used for simple mechanical removal of the crystal nucleus and masses.
So, for example, American specialists recently developed the AIopzo lens core cutter, which is a narrow trisector that segments the lens and removes fragments of 1.5 mm in size. After the removal of the first fragment, free space remains for manipulation and removal of other fragments of the lens.
This trisector is designed specifically for use with the AiYiyuopzo cutting platform. This cutting platform is used as a support for the AMopzo trisector. On the upper surface of the platform, a groove is made along the entire length, which allows you to hold and stabilize the core during its fragmentation.
- 2000. -The industry produces two models of the AiYyupvo platform, and their choice depends on the length of the operational section. For hydroexpresion of lens fragments, APop5o-Mesipiugue spoons have been developed, which have one irrigation hole for hydroexpresion and removal of residual lens fragments after manual phacofragmentation. The industry produces two models of spoons with a width of 3.0 and 4.5 mm, and their choice is determined by the length of the operative incision (see the journal: Orpina Itoioudu Titev. - 2000. -
- Р.20).Mo). - Me13. - R.20).
The general disadvantage of these tools is that, for their use, it is necessary to perform a large surgical incision, which increases the likelihood of postoperative complications.
There is a known method of extracapsular nuclear cataract extraction through a small tunnel incision, in which, after conducting a circular opening of the anterior capsule of the lens, hydrodissection of the nucleus is performed, followed by its division into parts and the removal of fragments from the capsular bag to the outside. Separation of the nucleus is carried out after the destruction of the upper cortical layer inside the capsular bag. To implement this method, a spatula is used to fix the nucleus at the point closest to the incision, and a phacochopper is used to make a cut 2/3 of the thickness of the nucleus in the direction of the incision. A spatula is inserted into the formed crack, the core is fractured and Fragments are separated with their successive removal (see Russian patent Me2157676 class A 61 E 9/00 published on October 30, 1998).
The disadvantage of the known method is the high probability of receiving an additional injury during the fracture of the nucleus and the dilution of the resulting fragments. This shortcoming is due to the imperfection of the design of the tool used.
12).A method of cataract extraction is also known, in which the front capsule of the lens is opened, then a device connected to an aspiration-irrigation system is inserted into the surgical incision. By moving the device in the surgical wound, aspiration and irrigation with physiological solution are carried out simultaneously. The tool for removing crystal masses consists of thin-walled tubes with nozzles for connection to the aspiration-irrigation system. The working parts of the tubes are interconnected, and the first tube is made longer than the second. The first, longer tube has a hole for aspiration, and the second tube has a hole for irrigation of crystal masses (see Russian patent Mo2058763 in class A 61 E 9/00 published on 04/27/96 in Bull. Me12).
The disadvantage of the known method is the limited scope of its use: it can be used only in those cases when the crystal mass has a liquid or semi-liquid (gel) consistency.
The disadvantage of the known tool is that it does not have the ability to influence (segment) the solid crystal mass because its design does not provide for the presence of any cutting edges.
The method of cataract extraction with a dense core and the device for its implementation are the closest in essence and the effect achieved, and are taken as a prototype, in which, after opening the anterior chamber, an instrument is introduced into the operative incision, with which the lens is segmented, simultaneously with with segmentation, washing fluid is injected into the operative wound and aspiration of crystalline masses is carried out, after which the instrument is removed from the eye and, depending on the further course of the operation, the incision is sealed, or vice versa, it is expanded for further manipulations for the implantation of an artificial lens, etc. The device for implementing this method of cataract extraction consists of a handle, to which the working body is attached and made in the form of an aspiration and irrigation cannula, and the irrigation cannula is located under the aspiration cannula and its working end protrudes beyond the end of the aspiration cannula and is rounded. The side surface of the irrigation cannula is made with a bevel formed by a plane passing through the longitudinal axis of the cannula and smoothly connected to the creating surface of the irrigation cannula at the point of its connection with the aspiration cannula. The end of the aspiration cannula is made in the form of a wave-shaped edge sharpened in recesses.
1).The presence of a cutting edge in the recesses of the aspiration cannula ensures effective and quick removal of viscous crystal masses due to the element of cutting them, and the blunt protruding edges of the cannula prevent damage to the surrounding tissues (see Patent of Ukraine Me12657 in class A 61 E 9/00 published on 28.02.97 in Bul. Mo1).
The main disadvantage of the known method of cataract extraction is that the quality of the segmentation of crystalline masses depends entirely on the qualifications and experience of performing such operations. This disadvantage is explained by the fact that when fragmenting crystal masses, the surgeon is forced to apply certain forces to the tool to cut the cutting edge into the crystal masses due to the fact that the cutting edge of the tool is structurally fixed.
Since the surgical wound has small dimensions, it is necessary to carefully calculate the force of pressing on the tool. In case of excessive pressure on the tool, additional injuries to the patient's eye occur. In addition, due to the fact that the cutting edge of the tool is stationary, the surgeon is forced to constantly turn the tool in the surgical wound as the volume of crystal masses decreases, and such manipulation requires an increase in the size of the surgical incision, which can lead to postoperative complications. Thus, the known method of cataract extraction has an increased degree of danger of additional injury to the organs of vision and leads to a longer period of wound healing with an increased likelihood of postoperative complications.
The main disadvantages of the known device for implementing the method of cataract extraction described above are the complexity of its design and the low efficiency of segmentation of crystalline masses. The first drawback is due to the presence of connected cannulas with a complex surface and the presence of a cutting wavy edge, which, due to the small size of the cannula, is difficult to perform technically and further maintain in a sharpened state. The second drawback is due to the fact that the device has a fixed cutting edge relative to the cannula, which forces the surgeon to perform additional manipulations in the surgical wound to cut the crystalline masses, which, in turn, leads to additional injury to the patient's eye.
The basis of the invention is the task of increasing the efficiency of the method of cataract extraction with a dense core by reducing the trauma and more complete removal of crystal masses due to the rarefaction of the latter by mechanical rotation with a device of a simpler and more reliable design, which has a rotating cutting edge on the end of the working organ.
The solution to the given problem is achieved by the fact that in the known method of cataract extraction with a dense nucleus, in which, after opening the anterior chamber and anterior capsulectomy and filling the anterior chamber with viscoelastic, an instrument is introduced into the surgical incision, with the help of which the lens is segmented, simultaneously with the segmentation, it is fed into the surgical wound washing liquid and aspiration of crystal masses is carried out, lens segmentation is carried out by mechanical destruction (disruption) until the crystal masses are completely rarefied. At the same time, the segmentation of crystal masses can be carried out both simultaneously by aspiration and irrigation, and separately with a subsequent aspiration-irrigation procedure. In the device for implementing the proposed method of cataract extraction, which contains a handle to which the working body is attached, the latter is made in the form of a rotating drill with cutting edges on the rear hemisphere of the head.
The essence of the invention is explained by the illustrative material, which shows the following: Fig. 1 - general view of the device for segmentation of crystalline masses according to the proposed method of cataract extraction; Fig. 2 - the head of the working body of the proposed device with a semi-smooth sphere; fig.3 - the same, with cutting edges throughout the sphere.
The device for carrying out the proposed method of cataract extraction contains a handle 1, inside which a rotator is located. As such a handle, the handle of a cutter used in dentistry for the treatment of teeth can be used. Therefore, its design, due to common knowledge, is not shown in Fig. 1. This well-known and widespread tool in dentistry is reliable, there is practically no vibration during its operation.
1.At the end of the handle 1, a working body 2 is installed in a collet clamp, which is made in the form of a drill with cutting edges 3 on the rear hemisphere of the head 4. When the device is turned on, its working body 2 begins to rotate relative to the longitudinal axis of the handle 1.
After opening the anterior chamber, the working body 2 of the proposed device is introduced into the operating incision, after which it is turned on. At the same time, the working body 2 begins to rotate and with its cutting edges З mechanically affects the crystal masses. Under the action of the cutting edges of the working body 2 of the rotating device, the crystal masses rotate, as a result of which their mechanical rarefaction is observed. To prevent damage to the surrounding tissues, the front hemisphere 4 of the drill working body 2 is made smooth (see Fig. 2). In another embodiment, cutting edges C can be applied to the curved surface of the head, but at the same time they do not protrude beyond the sphere, preserving the possibility of rotation of the crystal masses. In this way, the proposed device allows you to influence the dense core in a different way and mechanically thin it out. At the same time, in rotation, crystal masses can be removed by the irrigation-aspiration method, or the procedure for removing crystal masses can be carried out separately after the end of rarefaction of the dense core. After the capsule is completely cleaned of crystal masses, the device is turned off and removed from the eye. Depending on the further course of the operation, the incision is sealed or, on the contrary, expanded for further manipulations to implant an artificial lens.
The essential difference between the subject of the invention and the previously known ones is that during the segmentation of crystal masses, they are thinned by rotation of the working body of the rotating device. This difference allows you to minimize the traumatism of the surgical wound due to the fact that no additional efforts are required on the part of the surgeon and due to the fact that the manipulation of the tool during the segmentation of crystal masses is minimized. Such influence on crystal masses becomes possible due to the fact that a device with a rotating working body is used for segmentation, in which parts of well-known instruments are used, in particular, a rotator, widely known in dentistry, which allows to reduce the cost of the instrument. None of the known methods of cataract extraction can possess the mentioned advantages, because during their implementation it is necessary to use either expensive laser equipment, or the surgeon is forced to apply a certain strictly dosed effort to the tool, which calls into question the favorable outcome of the operation. No known instrument for segmentation of crystal masses can ensure their mechanical rarefaction, since they do not include a rotating working body in their design.
The technical advantages of the proposed technical solution, in comparison with the prototype, include the following: - expansion of the field of use due to the fact that the working body of the rotating device can effectively affect crystal masses of any consistency; - obtaining the same positive result of the segmentation of crystalline masses is independent of the surgeon's qualifications, due to the fact that during rotation, complex manipulations of the working body of the device in the surgical wound are not required; - the possibility of mechanical rarefaction of crystal masses due to the fact that the working body of the device is designed to rotate; - reduction of the cost of the device due to the fact that its design includes well-known, well-developed nodes in the industry, in particular, the rotator of the working body; - increasing the reliability of the device for the same reason.
The therapeutic effect of the implementation of the invention, in comparison with the use of the prototype, is obtained by reducing the probability of additional trauma to the eye during cataract extraction and by reducing the probability of postoperative complications.
7 | 7 , | | іThe economic effect of the implementation of the invention, in comparison with the use of the prototype, is obtained due to a significant reduction in the cost of the cataract extraction device. 7 | 7, | | and
