Description

腰椎微創固定裝置之骨釘
本創作是有關於一種骨釘,尤指一種微創侵入性脊柱固定組件之骨釘。
當脊椎一旦發生移位或產生壓迫或甚至周圍肌肉、韌帶組織緊張,都會直接影響脊椎中的神經,間接引起神經供應的末端器官,肌肉或分泌線體,造成行動不便或疼痛、身體不適,通常治療這種症狀可利用手術方法在脊椎骨間安裝固定裝置,以達減壓的效果;在某些案例中,則會施行椎間盤切除術將夾於脊椎骨間的故障椎間盤移除,接著置入一椎間融合器到該移除部位以重建相鄰脊椎骨間的高度,並在脊椎骨間安裝固定裝置,以利在後續的癒合程序間,可提供脊椎柱的穩固且校直。
在以往這種安裝固定裝置的手術略係將先骨釘分別植入固定在上、下兩節脊椎骨上或是複數個欲固定的脊椎骨的適合位置,骨釘的頂部具有連結桿固定座,因此可於各骨釘間鎖結固設一連結桿,使骨釘間連結成一剛性固定桿結構,據此保持各節脊椎骨間的定位,並提供一適度的支撐力,達減壓與校直的效能。
目前脊柱固定裝置的微創手術已被應用在椎骨病變的治療,因其開設的傷口小、可大幅降低對治療部位及附近組織的破壞性以提 升手術安全性,且可減少創傷與恢復時間。
典型的習知技術如美國專利公開號US 2012/0323278 A1「MINIMALLY INVASIVE SPINAL STABILIZATION SYSTEM」(脊柱微創穩定系統)乙案,揭露一種可提供一系列脊椎骨相對固定的微創侵入性脊柱固定組件,其包括一固定棒與多數個骨釘,每一骨釘包括一頂部及一螺紋本體,頂部配置為接受該固定棒之一部分,螺紋本體自該頂部之一第一端延伸且配置為用以嚙合此些脊椎骨其中之一。此組件更包括一扣件,扣件包括多數個外螺紋,外螺紋配置為用以嚙合形成於頂部之一內表面之多數個對應螺紋。固定棒透過扣件被固定於頂部,且頂部包括一分離區域。分離區域包括頂部之一部分,頂部之此部分相較於頂部之剩餘部分在結構上較弱,用以定義一位置,在供應足夠的力至頂部之至少一第一部分時,頂部之第一部分可輕易地於此位置與頂部之剩餘部分分離。
該骨釘的具體結構在圖4中詳示,骨釘的頂部30為一具有U型鏤空35的管狀,在該頂部之一環周切槽36定義出一固定座31及一分離區域32。施行前述脊柱固定裝置的微創手術時,將多個骨釘的螺紋本體40分別植設在多個脊椎骨上,使頂部30的上端開口停留在皮膚之上,使用一板尺狀的校準治具以對準個別骨釘之頂部30,使彼等骨釘的植入高度對齊,以及骨釘頂部30的U型鏤空35之縱向軸彼此平行,以便將一固定棒設置到各骨釘頂部的固定座31上,然後於骨釘頂部30的上端開口套置一帽蓋60,用以支撐與穩固頂部30上端二側之結構較為脆弱的分離區域32,以便確保後續的固定棒鎖固安裝,以及將骨釘頂部30之二側的分離區域32移除操作之順利進行。
前述骨釘頂部30為開放型開口,因此在頂部30上端二側之結構較為脆弱的分離區域32,容易在骨釘植入過程中或固定棒的安裝鎖固的操作中,承受外力而造成崩裂或變形,妨礙手術順利進行,嚴重影響手術結果;又即便是以在釘頂部的開口安裝一帽蓋60,用來克服前述不利特徵,但在開口組裝一帽蓋之作法,卻已益增組件構造以及組裝操作的複雜度,進一步延長手術時間及增加風險;另外,依照前述骨釘頂部的構造設計,骨釘植設過程中僅其頂部30的上端開口一小部分仍然停留在皮膚之上,但因人體的各節脊椎骨的生成組合角度均不相同,所以即便用板尺狀的校準治具使骨釘的植入高度對齊,亦難達符合人體脊椎之準確曲率;又,該板尺狀的校準治具係將多個骨釘頂部30的開口套置在一導槽中進行位置及角度的校準,而骨釘頂部30的U型鏤空35的截面恰與校準治具的導槽之軸線互呈垂直,故難以藉由該導槽之校準,使骨釘頂部30的U型鏤空35之縱向軸準確形成彼此平行之狀態,然而U型鏤空35之縱向軸若不能以彼此平行設置,會造固定棒串設安裝時的干涉,導致固定棒無法順利組裝到各骨釘頂部的固定座31上。
本創作主要目的乃提供一種便於體外準確定位的微創侵入性脊柱固定組件之骨釘結構,其具有一個增長的定位套,使微創手術過程中各個骨釘植裝後,其定位套的校準部可敞露於體外,以便於利用連桿安裝校準治具進行安裝位置及設置角度之校準,據此提升微創手術安裝脊柱固定組件之效率及精準性。
本創作另一目的乃提供一種簡化結構的微創侵入性脊柱固 定組件之骨釘結構,其定位套的上端開口呈係封閉型,可在手術過程中確保整體定位套結構不致變形或崩壞,因此無須另額加裝帽蓋之補強結構,可簡化操作步驟、節省手術時間,但在該定位套還設有軸向切槽及環型切槽,可藉對側牆提供一外力而使其裂解之後去除之。
為達上述創作目的,本創作提供一種便於安裝及準確定位的骨釘,其至少包括:一定位套及一螺釘體;該定位套實質上為杯狀,其包括一連桿固定座及一校準部,該連桿固定座與校準部係呈一體的,而該連桿固定座與校準部之間由一環型切槽所界定;該連桿固定座的底牆具一球型坑,且於球型坑底部中央部位設一貫穿孔,另在該球型坑上方的內孔表面上設有螺紋,並於該連桿固定座的側牆上設有一對完全地對齊的U形開口,使二U形開口之間形成一橫向穿越該連桿固定座的通道;該校準部的側牆上設有數對完全地對齊的開口,使各對開口之間形成一橫向穿越該校準部的通道,而該等數對開口包含至少二對連桿校準開口以及一對∩形開口,其中,該對∩形開口分別與在該連桿固定座的U形開口相連通,另於前述各開口之鄰接處的側牆上設有軸向切槽;以及該螺釘體,包含一球形頭及一螺桿,該球形頭外徑大於該螺桿外徑,且該球形頭連結於該螺桿的頂部,在該球形頭上設有與驅動工具相應的嵌接槽,並在球形頭外表面設有可增進摩擦力的止滑紋,於該螺桿上設有外螺紋,且螺桿的前端部具一呈銳角的螺尖端;其中,該螺釘體可樞接於該定位套的底端部位,使該螺釘體的球形頭套置在該連桿固定座的球型坑中;該定位套的校準部可藉由對側牆提供一向外的撐開力,使前述軸向切槽及環型切槽呈斷裂,該校準部得以裂解成二片並與該連桿固定座成為分離,以便自該定位套去除該校準 部。
特別是,所述定位套實質上為一加長的中空杯狀體,其長度介於85mm至110mm之間,因此骨釘植入脊椎骨後該校準部的大部分區域可停留在皮膚之上,以便於校準作業之進行。
特別是,還包含所述連桿校準開口之孔形係呈上大下小之燈泡形(bulb-shaped)或鎖孔形(keyhole-shaped),該孔形可使校準治具之組裝與調整操作變成簡易而快速,可大幅縮減手術時間。

請參照圖1~2所示,本創作實施例之腰椎微創固定裝置之骨釘係由一定位套200及一螺釘體100共同組成的;其中,該定位套200實質上為加長型的中空杯狀,其包括一連桿固定座210及一校準部220,而該連桿固定座與校準部之間由一環型切槽230所界定;其中,該連桿固定座210的底牆具一球型坑211,且於球型坑底部中央部位設一貫穿孔,另在該球型坑上方的內孔表面上設有螺紋213,並於該連桿固定座的側牆上設有一對完全地對齊的U形開口214,使二U形開口之間形成一橫向穿越該連桿固定座的通道;該校準部220的側牆上設有二對連桿校準開口221、222以及一對∩形開口223,使各對開口之間形成一橫向穿越該校準部的通道,其中,該對∩形開口223分別與在該連桿固定座的U形開口214相連通,另於前述各開口之鄰接處的側牆上設有軸向切槽224;而前述環型切槽230與軸向切槽224係具有較薄的壁厚,因此由該等切槽230、224所界定分隔的校準部220在結構上相對較弱於該定位套200的其他區域,亦即沿著該等切槽230、224對著校準部220的側壁施加足夠的力,可使校準部220的側壁與連桿固定座210輕易分離。
該螺釘體100包含一球形頭110及一螺桿120,該球形頭外徑大於該螺桿外徑,且該球形頭連結於該螺桿的頂部,在該球形頭110上設有與驅動工具相應的嵌接槽112,該嵌接槽可以例如是六角形或其他多角形的凹坑、十字型或是一字型的凹槽等,另,在球形頭外表面設有可增進摩擦力的止滑紋113,以便增加鎖固後的穩定性;於該螺桿120上設有外螺紋 121,且螺桿的前端部具一呈銳角的螺尖端122,以利將該螺桿120旋鑽植入骨骼內。
如圖2所示,該螺釘體100可轉動自如地樞接於該定位套200的底端部位,使該螺釘體的球形頭110停留在該連桿固定座210的球型坑211中,而其螺桿120穿過該連桿固定座底牆的貫穿孔延伸向外。
在圖3A~3E中顯示以微創手術在脊柱上安裝固定組件,依此可進一步說明本創作骨釘結構的應用方式;在微創手術中,將骨釘從皮膚501的創口***,利用一驅動工具伸入該定位套200的中空管內,嵌接該螺桿的球形頭110上之嵌接槽112以旋動該螺桿120,使該螺桿120植入脊椎骨502,由於脊椎骨502在皮膚底下且位於一般人肌肉深度約35mm至50mm深處,而該定位套200的長度設定約100mm,因此當骨釘植設在脊椎骨502之後,該定位套的校準部220有大部分的區域可停留在皮膚之上,特別是,該校準部側牆上的二對連桿校準開口221、222的部分;重複前述植入骨釘之操作步驟,用以將各個骨釘穩固植設在每一脊椎骨502。在繪示之實施例中顯示四支骨釘被分別植設在四個脊椎骨上。
接續,使用一校準治具600對準個別定位套的校準部220上之連桿校準開口221、222,並進行各個骨釘安裝位置及角度的調整,直到各個骨釘的連桿校準開口221、222設置均可符合該校準治具600的規範;而該等連桿校準開口221、222之孔形係呈上大下小之燈泡形,因此可使校準治具600之組裝與調整操作變成簡易而快速,大幅縮減手術時間。此外在這格實施例中,該校準部220具有二對連桿校準開口221、222,可共同形成一面狀的校準參數,因此對於骨釘的安裝位置及角度之校準操作較易執行且可 獲得較佳之精準度。該校準治具600具有與該脊柱固定組件安裝部位的人體脊椎相同的彎曲率,亦即經校準程序後各個骨釘之安裝排列均與人體脊椎排列的曲率相同;而且校準治具600又與欲安裝的脊柱固定組件之連桿701具有相應的結構,因此該連桿701可被精準地設置在各個骨釘定位套200的連桿固定座210上。
完成校準治具600對骨釘之校準程序後,一連桿701被設置到各個連桿固定座210的U形開口214中,並在其內孔之螺紋213搭配設置一止付螺絲(set screw/grub screw)702將連桿701鎖固在各個骨釘定位套200的連桿固定座210上。然後以一分離工具(未顯示於圖面)伸設到該定位套200的中空管內,並對該定位套的校準部220之側牆提供一向外的撐開力,使前述環型切槽230與軸向切槽224產生斷裂,該校準部220被裂解成二片並與該連桿固定座210成為分離,並自身體抽出分離的校準部220裂解片;據此完成在脊柱上安裝固定組件。

  1. 一種腰椎微創固定裝置之骨釘,至少包括:一定位套,包括一連桿固定座及一校準部,該連桿固定座與校準部係呈一體的,而該連桿固定座與校準部之間由一環型切槽所界定;該連桿固定座的底牆具一球型坑,且於球型坑底部中央部位設一貫穿孔,另在該球型坑上方的內孔表面上設有螺紋,並於該連桿固定座的側牆上設有一對完全地對齊的U形開口,使二U形開口之間形成一橫向穿越該連桿固定座的通道;該校準部的側牆上設有多對完全地對齊的開口,使各對開口之間形成一橫向穿越該校準部的通道,而該等多對開口包含至少二對連桿校準開口以及一對∩形開口,其中,該對∩形開口分別與在該連桿固定座的U形開口相連通,另於前述各開口之鄰接處的側牆上設有軸向切槽;以及一螺釘體,包含一球形頭及一螺桿,該球形頭外徑大於該螺桿外徑,且該球形頭連結於該螺桿的頂部,在該球形頭上設有與驅動器相應的嵌接槽,並在球形頭外表面設有增進摩擦力的止滑紋,於該螺桿上設有外螺紋,螺桿的前端部具一呈銳角的螺尖端;其中,該螺釘體樞接於該定位套的底端部位,使該螺釘體的球形頭套置在該連桿固定座的球型坑中;該定位套的校準部可藉由對側牆提供一向外的撐開力,使前述軸向切槽及環型切槽呈斷裂,該校準部裂解成二片並與該連桿固定座成為分離,以便自該定位套去除該校準部。
  2. 如申請專利範圍第1項所述之腰椎微創固定裝置之骨釘,其中所述定位套實質上為一加長的中空杯狀體,其長度介於85mm至110mm之間。
  3. 如申請專利範圍第1項所述之腰椎微創固定裝置之骨釘,其中當所述骨釘植入脊椎骨後,該校準部之連桿校準開口係停留在皮膚的上方。
  4. 如申請專利範圍第1項所述之腰椎微創固定裝置之骨釘,其中所述連桿校準開口之孔形係呈上大下小之燈泡形或鎖孔形。
  5. 一種骨釘的定位套,其包括一連桿固定座及一校準部,該連桿固定座與校準部係呈一體的,而該連桿固定座與校準部之間由一環型切槽所界定;該連桿固定座的底牆具一球型坑,且於球型坑底部中央部位設一貫穿孔,另在該球型坑上方的內孔表面上設有螺紋,並於該連桿固定座的側牆上設有一對完全地對齊的U形開口,使二U形開口之間形成一橫向穿越該連桿固定座的通道;該校準部的側牆上設有多對完全地對齊的開口,使各對開口之間形成一橫向穿越該校準部的通道,另於前述各開口之鄰接處的側牆上設有軸向切槽;其中,該螺釘體樞接於該定位套的底端部位,使該螺釘體的球形頭套置在該連桿固定座的球型坑中,該定位套的校準部可藉由對側牆提供一向外的撐開力,使前述軸向切槽及環型切槽呈斷裂,該校準部裂解成二片並與該連桿固定座成為分離,以便自該定位套去除該校準部。
  6. 如申請專利範圍第5項所述之骨釘的定位套,其中所述多對開口包含至少二對連桿校準開口。
  7. 如申請專利範圍第6項所述之骨釘的定位套,其中所述連桿校準開口之孔形係呈上大下小之燈泡形或鎖孔形。
  8. 如申請專利範圍第5項所述之骨釘的定位套,其中所述多對開口更包含一對∩形開口,且該對∩形開口分別與在該連桿固定座的U形開口相連通。
  9. 如申請專利範圍第5項所述之骨釘的定位套,其中所述定位套實質上為一加長的中空杯狀體,其長度介於85mm至110mm之間。
  10. 如申請專利範圍第5項所述之骨釘的定位套,其中更包含一螺釘體,其包括一球形頭及一螺桿,該球形頭外徑大於該螺桿外徑,且該球形頭連結於該螺桿的頂部,在該球形頭上設有與驅動器相應的嵌接槽,並在球形頭外表面設有增進摩擦力的止滑紋,於該螺桿上設有外螺紋,螺桿的前端部具一呈銳角的螺尖端;該螺釘體樞接於前述定位套的底端部位,使該螺釘體的球形頭套置在該連桿固定座的球型坑中。
