Abstract

Description
本專利申請案主張2014年5月22日申請之美國臨時申請案第62/002,076號及2014年8月12日申請之美國臨時申請案第62/036,476號之權利,其以全文引用之方式併入本文中。
細胞週期包含S期(DNA複寫)、M期(有絲***)及S與M期之間的兩個間隔期(G1及G2期)。細胞週期中之檢查點確保精確進展,諸如監測DNA完整性、DNA複寫、細胞大小及周圍環境之狀態(Maller,J.L.Curr.Opin.Cell Biol.,3:26(1991))。保持基因組完整性對於多細胞有機體尤其重要,且存在多個監測基因組狀態之檢查點。其中G1及G2檢查點分別存在於DNA複寫及有絲***之前。在進入S期之前校正DNA損傷亦為重要的,因為一旦受損DNA經複寫,其常產生突變(Hartwell,L.Cell,71:543(1992))。不修復廣泛DNA損傷的經由G1及G2檢查點之進展誘導細胞凋亡及/或災變。
大部分癌細胞在G1檢查點相關蛋白質(諸如p53、Rb、MDM-2、p16INK4及p19ARF)中攜有異常(Levine,A.J.Cell,88:323(1997))。或者,突變可導致癌基因產物(例如Ras、MDM-2及細胞週期素D)之過度表現及/或過度活化,其降低G1檢查點之嚴格度。除此等突變以外,過度生長因子信號傳導可由生長因子之過度表現而產生且可降低
G1檢查點之嚴格度。連同功能突變之損失及增加,生長因子受體或下游信號轉導分子之連續活化可藉由更動G1檢查點而導致細胞轉化。作廢之G1檢查點促成癌細胞中觀測到之較高突變率及多種突變。因此,大部分癌細胞依賴於G2檢查點以相對於過度DNA損傷而存活(O'Connor及Fan,Prog.Cell Cycle Res.,2:165(1996))。
咸信促使細胞週期G2在DNA損傷之後停滯之機制保留於酵母至人類之物種中。在受損DNA存在下,使Cdc2/細胞週期素B激酶因蘇胺酸-14及酪胺酸-15殘基對Cdc2激酶之抑制性磷酸化而保持不活化或降低細胞週期素B之蛋白質量。在有絲***之起始,雙磷酸酶Cdc25移除此等抑制性磷酸鹽且藉此激活Cdc2/細胞週期素B激酶。Cdc2/細胞週期素B之活化相當於M期之起始。
在***酵母中,回應受損DNA之細胞週期停滯需要蛋白激酶Chk1。Chk1激酶充當若干拉德(rad)基因產物之下游且藉由DNA損傷後之磷酸化而經修飾。已知出芽酵母之激酶Rad53及***酵母之激酶Cds1傳導來自未複寫DNA之信號。Chk1與Cds1之間似乎存在一些複聯,因為Chk1與Cds1之消除均在破壞由受損DNA誘導之G2停滯中達到頂點。有趣的是,Chk1及Cds1均使Cdc25磷酸化且促使Rad24與Cdc25結合,其使Cdc25螯合至細胞溶質且阻止Cdc2/細胞週期素B活化。因此,Cdc25似乎為此等激酶之共同標靶,意謂此分子為G2檢查點中不可缺少之因子。
在人類中,回應於DNA損傷,hChk1(***酵母Chk1之人類同系物)與Chk2/HuCds1(出芽酵母Rad53及***酵母Cds1之人類同系物)均使Cdc25C在絲胺酸-216處(重要調節位點)磷酸化。此磷酸化產生用於較小酸性蛋白質14-3-3s(***酵母Rad24與Rad25之人類同系物)之結合位點。此磷酸化之調節作用藉由以下事實明示,即在Cdc25C上將絲胺酸-216取代為丙胺酸可破壞人類細胞中之細胞週期G2停滯。然

CBP501(SEQ ID NO:80)為抗癌候選藥物,其在兩個隨機化II期臨床試驗中研究用於患有非鱗狀非小細胞肺癌(NSCLC)及惡性胸膜間皮瘤(MPM)之患者。已顯示CBP501具有兩種作用機制,即鈣調蛋白調節及G2檢查點廢除。本文揭示預測對CBP501之敏感性之生物標記物。經由短期(1小時)共暴露於CDDP及CBP501或單獨任一者之治療藉由定量分析CBP501之順-二胺二氯-鉑(II)(CDDP)增強活性將二十八個NSCLC細胞株分類為CBP501敏感及不敏感之兩個子組。對此等細胞株進行微陣列分析以鑑別與CBP501敏感性相關之基因表現模式。已發現多種核因子紅血球-2相關因子2(Nrf2)靶基因在CBP501不敏感細胞株中呈現高表現。NSCLC細胞株中Nrf2之西方墨點法及免疫細胞化學分析亦表明CBP501不敏感細胞株中之較高蛋白質量。此外,CBP501敏感性由抑制或蘿蔔硫素(SEN)誘導之Nrf2過度表現調節。此等結果表明Nrf2轉錄因子為抗CBP501之生物標記物之潛在候選。此實例鑑別將很好地對NSCLC之CBP501及CDDP組合療法作出反應的彼等患者亞群。
確立預測對特定藥物之敏感性的可定量標記物為個性化藥物之目標。已基於已知致癌突變發展若干抗癌藥物且已發現突變存在本身為此等藥物之有用生物標記物。實例包括一些靶向NSCLC中之表皮生長因子受體(EGFR)的酪胺酸激酶抑制劑,包括吉非替尼(gefitinib)
及埃羅替尼(erlotinib),其功效視特定EGFR突變之存在而定(1,2);克卓替尼(Crizotinib),其在NSCLC細胞具有EML4-ALK融合基因時極有效(3);及BRAF抑制劑維羅非尼(Vemurafenib),其在黑素瘤細胞攜有BRAF之V600E突變時極有效(4)。
已表明CDDP加CBP501之組合在患有抗鉑卵巢癌及MPM之患者中展示臨床活性證據(5)。療效之主要終點已對MPM達成(6)。G2檢查點廢除已基於以下觀測提出為作用機制(MOA):CBP501(i)抑制多種使CDC25C在Ser216處磷酸化之激酶,(ii)直接結合於14-3-3蛋白質,其與磷酸基-CDC25C形成抑制錯合物,(iii)減弱CDC25C在Ser216處之磷酸化及(iv)在與CDDP或博萊黴素(BLM)超長組合暴露後,減少癌細胞在G2/M期積聚(7,8)。
說明CBP501之其他MOA。此需要CBP501與鈣調蛋白之直接相互作用,與廢除G2檢查點之所需條件相比,其在暴露於CBP501更短時間下及更低CBP501劑量下即可提高癌細胞吸收CDDP(9)。已知CDDP之吸收視多種轉運子及通道而定(10)。鈣調蛋白-CBP501相互作用可能影響多種轉運子,以提高癌細胞吸收CDDP。
Nrf2為轉錄因子,其調節諸多抗氧化基因(包括彼等與麩胱甘肽合成相關之抗氧化基因)之表現(11)。在腫瘤形成中,Kras、BRAF及Myc之特定致癌對偶基因之表現藉由誘導Nrf2表現來提高胞內抗氧化劑之量(12)。Nrf2亦調節與藥物去毒及轉運相關之基因之表現(13)。
此實例藉由僅短期暴露於CBP501及CDDP後偵測細胞,來關注CBP501對促進CDDP吸收而非廢除G2檢查點之影響。藉由分析二十八個NSCLC細胞株在經過CDDP及CBP501短期(1h)共同暴露後相較於經過任一單獨藥物暴露後之反應,依定量方式界定其為對CBP501敏感或不敏感。對此等細胞株進行全面基因表現分析(微陣列)以鑑別此CBP501 MOA之敏感性標記物。由Nrf2轉錄因子調節之多種基因經鑑
別為在CBP501不敏感細胞株中具有高度表現。另外,可藉由使用Nrf2 sh-RNA減弱Nrf2時,表示Nrf2表現係抵抗CBP501所必需。
細胞培養物及試劑。於37℃、5% CO2/空氣下,在多種培養基中培養人類NSCLC細胞株,各補充有10%胎牛血清(Invitrogen,San Diego,CA)。將培養基RPMI1640(Sigma-Aldrich,St.Louis,MO)用於NCI-H1755、NCI-H2030、NCI-H1437、NCI-H2122、NCI-H2172、NCI-H2228、NCI-H1563、NCI-H1944、NCI-H1993、NCI-H1734、NCI-H1568、NCI-H2444、NCI-H2291、NCI-H2347、NCI-H1838、NCI-H1299、HCC827、NCI-H1975、NCI-H1155、NCI-H522及NCI-H1703;補充有2mM L-麩醯胺酸(Invitrogen)之RPMI1640用於NCI-H727;補充有4.5g/L D-葡萄糖(Sigma-Aldrich)、10mM HEPES(Sigma-Aldrich)及1mM丙酮酸鈉(Sigma-Aldrich)之RPMI1640用於NCI-H358、NCI-H520、NCI-H23、NCI-H647及NCI-H838;Ham氏F12K(Sigma-Aldrich)用於A549。CDDP購自Sigma-Aldrich。蘿蔔硫素(Sulforaphane)(SFN)購自Sigma-Aldrich。所有NSCLC細胞株購自ATCC且已由ATCC進行STR分析,以鑑認各細胞株之標識。所有細胞株在解凍後三個月內使用。
細胞週期分析。將細胞塗覆於24孔培養板中且培育24小時。使用或不使用添加加或不添加CBP501之CDDP,在指定濃度下處理細胞指定次數。收集細胞且用Krishan溶液(0.1%檸檬酸鈉、50μg/ml碘化丙錠、20μg/ml RNase A、0.5% NP-40)染色。使用CELLQuest軟體(Becton Dickinson),藉由FACSCalibur(Becton Dickinson,Franklin Lakes,NJ)分析染色細胞。
抗體。抗體購自指定來源:抗NR0B1、抗Grx、抗Prx、抗γGCSc、抗γGCSm及抗Gpx1(SantaCruz,Dallas,TX);抗MLC、抗
G6PD、抗ABCC2、抗KEAP1、抗BACH1、抗Trx1、抗SOD1、抗HO1、抗NQO1、抗IQGAP1及抗ATM(Cell Signaling Technology,Beverly,MA);抗GSR、抗AKR1C1、抗AKR1C3、抗AKR1B10及抗Nrf2(Epitomics,San Francisco,CA)。
微陣列及基因表現分析。在RNA分離之前,在單層中生長NSCLC細胞株。使用RNeasy微套組(Qiagen,Tokyo,Japan)分離總RNA。在TAKARABIO Inc.(Shiga,Japan)進行分離之總RNA樣品之按捷倫表現陣列(Agilent Expression Array)分析。一式三份地進行細胞株總RNA之各分析。由TAKARABIO Inc.進行基因表現資料之基本統計處理(單因子變異數分析及本亞明-霍赫貝格方法(Benjamini-Hochberg method):錯誤發現率0.05)。
慢病毒感染。Nrf2 shRNA慢病毒粒子及對照shRNA慢病毒粒子購自SantaCruz。根據製造商之說明書進行慢病毒感染。在病毒感染之前用5μg/ml凝聚胺(SantaCruz)治療細胞(50%匯合)。添加病毒之後一天,將細胞轉移至新製培養基且藉由用嘌呤黴素(SantaCruz)治療選擇細胞。
免疫螢光。蓋玻片上生長之細胞與4%多聚甲醛結合(15min),用磷酸鹽緩衝鹽水(PBS)洗滌(五分鐘,三次),接著在1×PBS中用1%阻斷Ace(DS pharma,Osaka,Japan)進行阻斷。接著其使用標準方案用抗體培育。初級抗體包括抗Nrf2(1:100)、抗AKR1C1(1:100)、抗AKR1C3(1:100)及抗AKR1B10(1:100)。用Alexa-488(Invitrogen)標記二級抗體且使用Hoechst 33342(DOJINDO,Kumamoto,Japan)將DNA染色。使用FV10i共焦顯微鏡(OLYMPUS,Tokyo,Japan)進行共焦顯微。
西方墨點分析。用或不用SFN在指定濃度下治療細胞(50%匯合)指定次數。收集細胞且在裂解緩衝液[50mM Tris-HCl(pH 8.0)、5
mM EDTA(pH 8.0)、100mM NaCl、0.5% NP-40、2mM DTT、50mM NaF、1mM Na3VO4、1μM微囊藻素與蛋白酶抑制劑混合物(Roche,Mannheim,Germany)]中裂解(於冰上30min)。藉由離心(20600g,4℃)澄清裂解液,且根據製造商之說明書使用清潔劑可相容之蛋白質分析套組(Bio-Rad,Hercules,CA)對清液層分析蛋白質含量。在10-12% SDS-PAGE上操作全細胞裂解液(60μg)。將來自各凝膠之蛋白質轉移至聚偏二氟乙烯(PVDF)膜(Bio-Rad)上。於室溫下在含有1%阻斷Ace(DS pharma)之TBST(10mM Tris-HCl[pH 8.0]、150mM NaCl及0.05% Tween-20)中將膜阻斷1h且在4℃下用初級抗體培育隔夜。在洗滌後於室溫下進一步用抗過氧化酶結合二級抗體(Cell Signaling)將膜培育一小時,且使用增強化學發光偵測系統(ECL高級西方墨點偵測套組,GE Healthcare)偵測信號。使用Lumino-mater LAS-4000儀器(Fujifilm,Tokyo,Japan)量化偵測帶。
NSCLC細胞株之突變搜尋。在NSCLC細胞株中搜索Keap1突變使用在線資料庫,即癌症體細胞突變目錄(Catalogue of somatic mutations in cancer,COSMIC)(http://cancer.sanger.ac.uk/cancergenome/projects/cosmic/)進行。
如多種方法所示,CBP501使腫瘤細胞對CDDP敏感,該等方法包括群落形成分析、活體外存活力分析及流動細胞測量分析(其量測細胞週期分佈中之全身變化)。不同方法之間的結果很好地相關(8,9)且不同細胞株對CBP501之差異性敏感性不斷變化(9)。
本文中,如流動細胞測量所指示,使用細胞週期分佈中之變化來估算NSCLC細胞株對CDDP經CBP501增強細胞毒性之敏感性。在CBP501存在或不存在之情況下,在用CDDP治療之細胞中監測此類變
化。將此分析用於評估抗癌治療之有效性係基於G1/S檢查點調節功能在諸多類型癌細胞之細胞週期中不存在(14)。功能性G1/S檢查點之缺乏導致癌細胞在其暴露於DNA損傷抗癌劑後在G2/M期中積聚(14,15)。
為評定CBP501之CDDP增強活性,一者可偵測兩個劑量-反應曲線中之任一者,其中X軸表示CDDP劑量且Y軸表示亞G1或G2/M中之細胞數量。對於CBP501敏感細胞,在CBP501存在下,任一劑量-反應曲線中之轉變及峰會轉移至左側,表明CDDP改變細胞週期分佈之能力增強。藉由此類NCI-H1703細胞分析,CBP501之存在使得CDDP改變細胞週期分佈之有效性提高約八倍(圖1A-D)。另一方面,NCI-H1437細胞株未在CDDP劑量-反應曲線中展現由CBP501所產生之任何可偵測轉移(圖1E-H)。基於CBP501轉移CDDP誘導之亞G1或G2/M劑量-反應曲線之能力使用此規則,將十二個NSCLC細胞株分類為CBP501敏感或CBP501不敏感。將轉移曲線兩倍或兩倍以上之細胞株分類為CBP501敏感細胞且將轉移曲線小於兩倍之細胞株分類為CBP501不敏感(表1)。NSCLC細胞株中CBP501敏感性之分類。
全面基因表現分析表明一些Nrf2靶基因在CBP501不敏感細胞株中可能具有上調表現。
對表1中所列十二個NSCLC細胞株進行全面基因表現(微陣列)分析。將表現量之臨限信號值設定為5000後,四十個基因展示70%以上與CBP501敏感性之相關性(圖2A)。將此等經鑑別基因再分成兩個類別:在敏感細胞株中高度表現之基因或在不敏感細胞株中高度表現之基因。對分別分類為敏感或不敏感之基因分組之平均表現量值的偵測表明,當比較CBP501敏感及不敏感細胞株時,一些不敏感基因分組不斷展現可辯別之表現量差異(圖7)。亦對表1中所列十二個NSCLC細胞株進行超過四十個蛋白質之西方墨點分析,該等蛋白質在包括壓力反應、抗藥性、代謝、分化及抗氧化反應之多種生物過程中起作用(資料未顯示)。發現Nrf2蛋白質之表現量在同一組NSCLC細胞株中很好地與CBP501敏感性相關(圖2B、C)。基於此等分析,將焦點置於來自圖2A之麩胱甘肽還原酶(GSR)、葡萄糖-6-磷酸鹽去氫酶(G6PD)、ATP結合盒式區亞家族C成員2(ABCC2)、醛基-酮基還原酶家族1C1(AKR1C1)及AKR1C3之mRNA表現水準上,因為已知此等基因之表現由共同轉錄因子Nrf2調節(16-20)。藉由西方墨點分析已確認蛋白質表現量與mRNA表現相關(圖2D)。
使用免疫細胞化學偵測若干NSCLC細胞株中Nrf2之胞內定位。此等測試揭示相較於CBP501敏感細胞株,CBP501不敏感細胞株中Nrf2之高度核定位以及Nrf2之顯著較高之全細胞表現量(圖3)。此等結果表明CBP501不敏感細胞株中Nrf2蛋白質之高表現量產生高靶基因表現量。
進行西方墨點分析來偵測Nrf2之其他已知基因標靶之表現量(16-20)。NAD(P)H去氫酶醌1(NQO1)、AKR1B10、γ-麩胺醯半胱胺酸合
成酶調節劑次單元(γGCSm)及麩胱甘肽過氧化酶1(GPX1)各自在CBP501不敏感細胞株中展示較高表現量(圖4A)。此等結果支持在正常培養條件下Nrf2靶基因之表現在CBP501不敏感細胞株中增強。
為說明CBP501敏感性與Nrf2表現之間的直接因果關係,產生對CBP501敏感之H1703之穩定Nrf2阻斷細胞株(圖4B)。將此細胞株一式三份地暴露於CBP501、CDDP及SFN,研究已知Nrf2活化子(17)。如圖4B中所示,SFN導致Nrf2蛋白質量在用對照shRNA轉染之細胞中以劑量相關方式提高。SFN未導致Nrf2蛋白質量在用Nrf2 shRNA轉染之細胞中有類似提高。添加SFN減弱CBP501在用對照shRNA轉染之細胞中之作用(圖4C、D)。然而,SFN之作用在Nrf2阻斷株系中消除(圖4B-D)。
研究CBP501不敏感細胞株H1437中之Nrf2阻斷作用。發現Nrf2阻斷導致AKR1C3、G6PD及GSR之表現量降低(圖5A)。對於此阻斷細胞株,發現CBP501提高CDDP對亞G1及G2-M之細胞週期分佈變化之作用,逆轉初始CBP501不敏感性。另一方面,用對照shRNA轉染之細胞株展示在有或無CBP501之情況下CDDP活性無差異(圖5B、C)。此等結果表明高Nrf2表現量可能誘導CBP501抗性。
Nrf2蛋白質之高表現為預測CBP501抗性之候選標記物;Nrf2標靶(在蛋白質或mRNA量下)為其他候選標記物。
儘管Nrf2或其若干下游轉錄標靶之表現可能可預測CBP501抗性,但CBP501敏感性之此等預測標記物之總體可靠性仍待確定。分析CBP501與CDDP在十六個其他NSCLC細胞株中之組合作用,該等細胞株各自藉由先前標準分類為CBP501敏感或不敏感(表2)。其他NSCLC細胞株中CBP501敏感性之分類。

接著對此等細胞株進行與初始十二個細胞株相同之微陣列分析。再次分析二十八個細胞株之擴增組之基因表現熱圖來鑑別具有70%以上精確性之CBP501敏感性預測標記物候選。鑑別七個此類基因,包括三個Nrf2靶基因GSR(85.7%預測率)、AKR1C3(75%)及G6PD(85.7%)(圖6A)。亦藉由西方墨點法確認此等細胞株中相應蛋白質之表現量(圖6B)。值得注意地,AKR1C3及GSR之高表現量同等地保持與CBP501抗性及Nrf2表現相關(圖6B、C)。為研究此觀測之實際效用,偵測若干NSCLC細胞株以確定藉由免疫細胞化學方法偵測AKR1C3之蛋白質表現是否可充當可能之CBP501敏感性預測標記物。此類方法有希望大體上適用於標記物偵測,因為其在通常可購自NSCLC腫瘤活檢體之小組織樣品中足夠敏感以偵測經表現蛋白質量(21)。根據此等測試,推斷與Nrf2之結果類似,相較於CBP501敏感細胞株(圖6D、E),實際上可在CBP501不敏感細胞株中偵測AKRIC3之較高量(圖3及6B)。此等結果表明Nrf2及一系列Nrf2基因靶分子之免疫組織化學偵測可為預測至少活體外CBP501抗性之高度可靠標記
物。
已表明CBP501作為抗癌候選藥物具有至少兩種作用機制(8,9)。此等中之一者,即G2檢查點廢除於較長治療持續時間及較高劑量下發生(8)。CBP501之第二抗癌活性,即經由與鈣調蛋白結合促進CDDP吸收於較短治療持續時間及較低劑量下發生(9)。在此實例中,將焦點置於後一作用上且藉由用較低劑量CBP501治療細胞持續較短時間來表徵諸多不同NSCLC細胞株之CBP501敏感性。此CBP501二級抗癌活性之詳細分子機制仍待闡明,因為CBP501-鈣調蛋白相互作用似乎影響CDDP吸收中涉及之眾多通道及轉運子中之若干者(10)。此等關於CDDP吸收之不明確性使最獨特地受CBP501影響之單一CDDP轉運路徑之鑑別複雜化。如藉由微陣列分析所測定,藉由鑑別CBP501敏感與不敏感細胞株之間的基因表現概況之主要差異來預測CBP501敏感性。此基因表現之全面分析首先產生若干CBP501敏感性之預測標記物候選。將來自由Nrf2調節之共同轉錄路徑的基因之提高表現鑑別為CBP501不敏感性之可能指示物。已知Nrf2為與細胞保護功能相關之基因關鍵轉錄因子(22-26)。在體內恆定機制條件下,藉由蛋白酶體降解系統經由其與結合Kelch類表氯醇之蛋白質1(Keap1)及Cul3 E3接合酶之結合來使Nrf2保持於極低胞內濃度下(27-30)。已發現Nrf2蛋白質表現量在CBP501不敏感細胞株中比在CBP501敏感細胞株中高。然而,Keap1表現量並未亦與CBP501敏感性相關(圖2B)。此結果可表明Nrf2蛋白質在體內恆定機制條件下降解之能力在不同細胞株之間變化。若干報導表明NSCLC細胞株及腫瘤中Keap1突變與不良預後或化學抗性之間的關係(31-33)。Keap1突變存在於此實例中所使用之一系列NSCLC細胞株中(下表3)。Keap1突變具有存在於CBP501不敏感細胞株中之傾向。此表明Keap1突變情況為CBP501抗性之有用指示物。各突變類型之作用將進一步檢驗。
將若干Nrf2靶基因於RNA或蛋白質量方面之差異性表現鑑別為鑑別預測CBP501不敏感性之標記物候選之方式。在此等初始候選中,G6PD、GSR及AKR1C3在CBP501敏感與不敏感細胞株之間的mRNA及蛋白質表現量中均顯示顯著差異。公認調節此等經鑑別基因之表現在稱為抗氧化反應元件(antioxidant response element,ARE)之Nrf2結合元件之共同控制下進行(23)。若干已知轉錄因子可藉由爭奪ARE處之相互作用來拮抗Nrf2。此等轉錄因子包括小MAF蛋白質、BACH1、c-FOS及FRA1(23,34,35)。此等轉錄調控子可參與控制不同Nrf2靶基因之差異性表現。實際上,BACH1蛋白質為其在NSCLC細胞株中之表現似乎在CBP501敏感細胞株中提高之蛋白質(圖2B)。
Nrf2藉由誘導眾多基因之表現而參與廣泛範圍之不同生物現象,包括代謝、異生物質反應及抗氧化反應(22-26)。舉例而言,誘導
G6PD及GSR之表現提高麩胱甘肽量且此引起抗氧化反應(36,37)。AKR1C3可介導引起睾固酮合成及***素形成之路徑(38,39)。儘管Nrf2阻斷實驗表明Nrf2可調節CBP501敏感性,但關於何種Nrf2靶基因或現象直接影響CBP501敏感性仍無決定性解答。一種或若干Nrf2靶基因可參與測定細胞對CBP501對CDDP吸收之作用之敏感性。此類直接作用可能難以藉由習知微陣列分析偵測,該分析有時可能受技術限制阻礙,諸如個別探針偵測中變化之敏感性。儘管已表明在至少七個細胞株中CBP501/CDDP對短期暴露之組合作用很好地與CDDP吸收相關(9),但並未說明NSCLC細胞株中之鉑積累。因此CBP501可能不僅影響CDDP吸收之增強且亦影響相對於CDDP之Nrf2相關細胞保護作用之抑制。舉例而言,可由Nrf2上調的降低之麩胱甘肽(GSH)結合於CDDP且可降低毒性(40)。然而,所量測之NSCLC細胞株中胞內降低之GSH量不與CBP501敏感性相關(資料未顯示)。顯而易見,需要進一步調查以確定具體機制,Nrf2藉由該機制調節CBP501不敏感性。
然而,對於實際選擇可由CBP501敏感性受益之患者,免疫組織化學(IHC)可為所選首要方法,因為可能自經由生檢可利用之小腫瘤樣品獲得敏感性之明確指示。可藉由IHC而非西方墨點法分析較小樣品(21)。對Nrf2、AKR1C1、AKR1B10及AKR1C3之免疫細胞化學量測之研究表明其可靠地再現西方墨點實驗中獲得之結果(圖3、6C、D、圖8及9)。驗證使用IHC偵測腫瘤生檢樣品中之此等蛋白質及進一步驗證與CBP501敏感性之相關性可提供有用工具來預測患者對組合CDDP+CBP501治療之提高之反應性。

- 如請求項68或69之組合物,其中該選擇性地偵測NRF2蛋白質或由NRF2靶基因編碼之蛋白質或KEAP 1蛋白質之試劑包含抗體。
- 如請求項65、66、68及69中任一項之組合物,其中該NRF2靶基因為麩胱甘肽還原酶(GSR)、葡萄糖-6-磷酸去氫酶(G6PD)、ATP結合盒式區亞家族C成員2(ABCC2)、醛基-酮基還原酶家族1C1(AKR1C1)、醛基-酮基還原酶家族1C3(AKR1C3)、NAD(P)H去氫酶醌1(NQO1)、AKR1B10、γ-麩胺醯半胱胺酸合成酶調節劑次單元(γGCSm)或麩胱甘肽過氧化酶1(GPX1)中之任一者。
