Диаметр шаровых опор составл ет 0,5-0,2 диаметра головки плечевой кости 11. Это обеспечивает достаточный запас прочности эндопротеза и его плавное вращение. Внутренний контур в охватывающих чаше выполнен по форме щаровых опор и глубиной не менее 2/3 их диаметра. Детали креплени  сферических чашек 3 и 4 выполнены в виде стержней 5 и 6, имеющих на внешних концах резьбу. Внутренними торцами они жестко креп тс  к внешней, тыль ной поверхности своих сферических чашек с отклонением от продольной оси сферы на углы, определ ющие средне-физиологическое положение верхней конечности. Эти углы корригируютс  в процессе выполнени  имплантации . На резьбовые участки стержней 5 и 6 устанавливают сначала опорные шайбы 7, у которых профиль совпадает с профилем поверхности кости is месте фиксации, затем наклады ваютс  выравнивающие щайбы 8, обеспечивающие выравнивание поверхностей под гайку 9. Таким образом, получаетс  круговое равномер ное давление гайки 9 на кости 11 и 12, исключающее деформацию вертикального сло  костей и образование нежелательных щелей между шайбами и гайками. Эндопротез используетс  следующим образом . Имплантаци  эндопротеза производитс  посл предварительного просверливани  соответствую щим сверлом трансоссальных каналов в плечевой кости Ни лопаточной кости 12 с посл дующим введением в эти каналы стержней 5 и 6. После введени  стержней в каналы костей 11 и 12 производ т поочередно установку каж дой охватывающей сферической чашки 3 и 4 относительно суставных поверхностей. Когда кажда  из охватывающих сферических чашек установлейа в нужном положении, производ т их закрепление. Вначале накладывают и .подгон ют к костным поверхност м опорные шайбы 7, на них накладывают промежуточные - выравнивающие щайбы 8, а затем производ т зажим их и закрепление стержней гайками 9 к кост м. Сила зажима определ етс  оперирующим хирургом. После закреплени  обоих стержней в кост х 11 и 12 провер ют степень свободы движ ний верхней конечности и правильность перемещени  плавающего элемента, т.е. скольжени и вращение .шаровых опор 1 и 2 в чашках 3 и 4. Плавающий элемент, состо щий из двух ша ров, опор, слитых воедино с обхватывающими сферическими чашками, образует двойное шаровое соединение, допускающее перемещени соединенных костей относительно сустава в объеме, равном сумме объемов движений каж 4 дого из составл ющих шарниров, т.е. больше объема движений в нормальном плечевом суставе . В Данном случае объем движений в имплантированном эндопротезе будет ограничиватьс  только костными образовани ми и м гкими ткан ми сустава и соответствовать естественному. Таким образом, благодар  наличию в эндопротезе плавающего элемента и трансоссально закрепл емых деталей в виде стержней с шайбами и гайками на концах упрощаетс  операци  имплантации эндопротеза, сокращаетс  врем  ее проведени . Сила, действующа  на Эндопротез, благодар  двойной шаровой опоре плавающего элемента разлагаетс  на составл ющие , что снижает функциональные нагрузки на каждый из элементов устройства и повышает его износоустойчивость. Трансоссальна  фиксаци  эндопротеза позвол ет сохранить естественный объем костномозгового канала плечевой кости, предотвратить повышение давлени  в нем, что крайне болезненно, и предупредить жировую эмболию, как нередкое операционное осложнение при данных хирургических вмешательствах. Из всех элементов и частей эндопротеза внекостно располагаетс  только один плавающий элемент, отграниченный от м гких околосуставных тканей кра ми суставной поверхности лопатки, что обеспечивает безболезненность движений. Малый размер двойного щарового соединени  позвол ет сохранить проксимальньш отдел плечевой кости с местом прикреплени  лолщц и естественного ложа сухожили  двуглавой мыщцы и предотвратить укорочение верхней конечности при необходимости реимплантации эндйпротеза с последующей операцией вторичного эндопротезировани  или артродеза. The fastening parts of the spherical cups 3 and 4 are made in the form of rods 5 and 6, having a thread on the outer ends. With their inner ends, they are rigidly attached to the outer, rear surface of their spherical cups with a deviation from the longitudinal axis of the sphere by angles that determine the average physiological position of the upper limb. These angles are corrected during the implantation process. On the threaded sections of the rods 5 and 6, firstly, support washers 7 are installed, in which the profile coincides with the bone surface profile is the fixing point, then alignment pins 8 are applied to ensure the alignment of the surfaces under the nut 9. Thus, a circular uniform pressure of the nut 9 is obtained bones 11 and 12, eliminating the deformation of the vertical layers of bones and the formation of undesirable gaps between the washers and nuts. The endoprosthesis is used as follows. Implantation of the endoprosthesis is performed after preliminary drilling by the corresponding drill of transossal canals in the humeral bone of the scapula 12 and subsequent insertion of rods 5 and 6 into these channels. After inserting the rods into the bone canals 11 and 12, each female spherical cup 3 and 4 relative to the articular surfaces. When each of the surrounding spherical cups is set in the correct position, they are fastened. Initially, support washers 7 are placed on the bone surfaces, intermediate ones are placed on them - leveling pins 8, and then they are clamped and the rods are fixed to the bone with nuts 9. The clamping force is determined by the operating surgeon. After both rods are fixed in bones 11 and 12, the degree of freedom of the upper limb movements and the correctness of the movement of the floating element, i.e. skidding and rotation of ball bearings 1 and 2 in cups 3 and 4. A floating element consisting of two balls, supports merged together with spherical grasping cups, forms a double ball joint allowing displaced bones relative to the joint in a volume equal to the sum the volumes of movements each 4 of the components of the hinges, i.e. more range of motion in the normal shoulder joint. In this case, the range of motion in the implanted endoprosthesis will be limited only to the bone formation and soft tissues of the joint and correspond to the natural. Thus, due to the presence in the endoprosthesis of a floating element and transossually fastened parts in the form of rods with washers and nuts at the ends, the operation of implanting the endoprosthesis is simplified, and the time it is reduced. The force acting on the Endoprosthesis, due to the double ball support of the floating element, decomposes into components, which reduces the functional loads on each of the elements of the device and increases its durability. The transossal fixation of the endoprosthesis allows one to preserve the natural volume of the bone medullary canal of the humerus, to prevent an increase in pressure in it, which is extremely painful, and to prevent fat embolism as a frequent operational complication in these surgical interventions. Out of all elements and parts of the endoprosthesis, only one floating element is located outside the limb, delimited from the soft periarticular tissues by the edges of the articular surface of the scapula, which ensures painless movements. The small size of the double glomerular joint allows the proximal section of the humerus to be preserved with the attachment of the scape and the natural bed of the biceps tendon and to prevent the upper limb from shortening when reimplantation of the endoprosthesis is necessary, followed by secondary endoprosthesis or arthrodesis. The invention of the endoprotein of the shoulder joint, comprising interconnected distal and proximal parts and fastening elements, characterized in that, in order to restore the full range of motion, maximum preservation of the proximal humerus, the distal and proximal parts are made in the form of spherical supports installed in hollow spherical cups, and fasteners in the form of threaded rods with washers and nuts, connected to hollow spherical tufts.
