(54) СПОСОБ ОПРЕДЕЛЕНИЯ ОБРАТИМОСТИ НАРУШЕНИЯ КРОВОСНАБЖЕНИЯ КОНЕЧНОСТИ ПРИ АНАЭРОБНОЙ ГАЗОВОЙ ИНФЕКЦИИ (54) METHOD FOR DETERMINING THE REVERSIBILITY OF BLOOD LIMB BLOOD SUPPORT AT ANAEROBIC GAS INFECTION
Изобретение относитс к медицине и может быть использовано в хирургической практике. Известен способ опре/ елени обратимости нарушени кровоснабжени ко{1ечиости при анаэробной газовой инфекции с помощью рентгенографии 1. Однако такой способ не дает объективной информации о характере изменени кровообращени при анаэробной газовой инфекции и определени целесообразности и уровн ампутации. Целью изобретени вл етс определение целесообразности и уровн ампутацци. Это достигаетс тем, что осуществл ют исходную ангиографию, после чего провод т перфузию конечности спазмолитическими и антибактериальными лекарственными веществами и через 45-60 мин провод т пов торную ангиографию и по изменению диаметра сосудов конечности устанавливают границу нарущени кровоснабжени и определ ют целесообразность или уровень ампутации . Способ осуществл ют следующим образом . Больному общеприн тым пункционным методом в бедренную артерию пораженной конечности ввод т 20,0 мл контрасного вещества и производ т рентгенограмму (ангиограмму ). После проведени ангнограммы на пораженную конечность на уровне тазобедренного сустава накладывают жгут и сосудистое русло конечностиподключ ают к аппарату искусственного кровообращени дл проведени перфузии лекарственной сме СЬЮ, обладающей спазмолитическим, антибактериальным , антикоагул ционным, тромболитическим и дезинтексикационным действием (в течение 45-60 мин). По окончании перфузии при аналогичных технических услови х производ т повторную ангиограмму пораженной конечности. На полученных до и после перфузии ангиограммах на разных уровн х сверху вниз измер ют диаметры крупных сосудов (бедренной , подколенной, малоберцовой, переднеберцовой , заднеберцовой артерий). Затем на выбранных уровн х определ ют насколько диаметр сосудов второй ангиограммы (после перфузии) больще таких же сосу- . дов первой. The method is carried out as follows. A 20.0 ml contrast material was injected into the femoral artery of the affected limb using a conventional puncture method and an x-ray (angiogram) was taken. After an angnogram is performed on the affected limb at the level of the hip joint, a tourniquet is applied and the vascular bed of the extremity is connected to a cardiopulmonary bypass for conducting perfusion of the drug mixture, which has an antispasmodic, antibacterial, anticoagulant, thrombolitic and disintegrating, anti-bacterial, anti-coagulant, thrombolytic and disintegrating, anti-bacterial, anti-bacterial, anti-coagulation, thrombolytic and disintegrating, anti-bacterial, anti-bacterial, anti-coagulant, thrombolytic, and disintegrating procedure. Затем на область тазобедренного сустава накладывалс жгут, и конечность на 60 мин подключалась к аппарату искусственного кровообращени дл проведени перфузии лекарственной смесью, обладающей спазмолитическим, антибактериальным, антикоагул цирнным , тромболитическим, дезинтоксикационным действием. На полученных до и после перфузии айгиограммах бедра и голени на выбранных уровн х измер лись диаметры крупных сосудов, а именно, бедренна артери измер лась в области отхождени подкожной артерии, подколенна - в .проекции межсуставной щели, малоберцова , заднеберцова , передиеберцова артерии - на 1 см ниже их отхождени . При сопоставлении результатов замеров на ангиограмме после перфузии отмечено значительное изменение ширины просветов крупных сосудов пораженной конечности больного Ш.; после 1 час перфузии по данным ангиограмм () - увеличение диаметра бедренной артерии, умеренное увеличение подколенной. Диаметры малоберцовой , переднеберцовой, заднеберцовой артерий после перфузии не изменилось.
