Abstract
This invention relates to the field of medicine, namely to neurology. The method allows to determine the level of damage to the trigeminal nerve, which is achieved by recording somatosensory evoked potentials for stimulation of exit points 1, II. Ill branches of the trigeminal nerve from the patient and intangible sides, analyzing the components of the evoked response range of 3-9 ms and diagnosing extracranial damage with a decrease in the amplitude of components by 15% or more in the response of one of the branches of the trigeminal nerve, and intracranial damage - with a similar decrease in amplitude in the responses of all three branches of the trigeminal nerve compared with the intact side. s Ј
Description
The invention relates to medicine, namely to neurology.
The aim of the invention is to determine the level of the lesion.
The method is carried out as follows.
During the examination, the patient should be in a calm and awake state in a darkened room.
In order to obtain trigeminal somatosensory evoked potentials (SSEPs), electrocutaneous irritations are applied in the symmetrical exit areas of the I, II, and III branches of the trigeminal nerve on both sides of the face. Square current pulses are applied to a bipolar cutaneous electrode (interelectrode distance of 2-2.5 mm) with a frequency of 1-2 pulses per second, pulse duration 0.1 ms. and intensity
10-20.corresponds to 1.5-3 thresholds of initial sensations on the healthy side of the face. SSVP register needle needle electrodes installed over the area of the back of the central gyrus in accordance with the somatotopic projection of the face at points With 5 -Se (active electrodes) and at the point FZ (reference electrode) on the international system 10-20.
For the analysis of components arising contra-laterally with respect to the stimulated points of the face, average SSEP values are used for 128-256 realizations, the analysis epoch for early components is 100-200 ms with the amplifier bandwidth of 5.3 - 1000 Hz. The studies were carried out using a set of instrumentation including a neural apparatus, a plotter and

При снижении амплитуды компонентов с латентным периодом 3-9 мс на 15% и более или при их отсутствии в вызванных ответах при сти- мул ции каждой из трех ветвей тройничного нерва определ ют интракраниальный уровень поражени тригеминального узла и корешка тройничного нерва.electronic stimulator When the amplitude of components with a latent period of 3–9 ms is reduced by 15% or more or is absent, the extracranial level of damage to the trigeminal nerve is determined in the evoked response to stimulation of one of the branches of the trigeminal nerve. With a decrease in the amplitude of components with a latent period of 3–9 ms by 15% or more or their absence in evoked responses, when stimulating each of the three branches of the trigeminal nerve, the intracranial lesion of the trigeminal node and root of the trigeminal nerve is determined.
П р и м е р 1. PRI me R 1. Patient G. 35 years. Electrodermal irritations were applied in the symmetric areas of the face in the exit area of all three branches of the trigeminal nerve using the described procedure.
Analysis of the obtained SSEPs showed that when stimulating the exit region of the II branch of the trigeminal nerve on the left, the absence of a component in the 5 ms latency region was determined, which indicated an extracranial level of damage to the II branch of the trigeminal nerve on the left.
П р и м е р 2. PRI mme R 2. Sick C, 55 years. An SSEP study was conducted under similar conditions using the same technique. An analysis of the obtained SSEPs showed that when stimulating all three branches of the right trigeminal nerve, a decrease in the components in the 5 ms latency region was observed, indicating a lesion at the level of the trigeminal ganglion, the sensory root of the trigeminal nerve
