Description
The invention relates to medicine, namely to prostheses of internal organs.
The purpose of the invention is the formation of the epithelial layer.
Figure 1 schematically shows the design of the prosthesis; figure 2 is the same in operation.
The prosthesis of the tubular organ contains a porous tube 1, which can be made of synthetic mesh or from a knitted tube, corrugated and reinforced by a spring. An elastic thin-walled tube 2 is installed inside the tube 1, and the ends of the tubes are hermetically connected to form a space into which the tubes 3 and 4 open. The ends of the prosthesis can be closed by an elastic membrane 5, the space is filled with a culture of epithelial tissue,
The prosthesis works as follows.
If necessary, perform plastic surgery, for example, an extensive section of the trachea 6 (figure 2). parallel to the latter establish the proposed prosthesis, the diameter of which should approximately correspond to the diameter of the replaced section of the trachea. Pipes 3 and 4 are removed over the skin. In the postoperative period, the connective tissue surrounding the prosthesis begins to grow through the pores of the tube. In parallel, a piece of tracheal mucosa with an unaffected epithelial lining is taken by biopsy. By known methods, a culture of epithelial cells of the tracheal mucosa is prepared and cultured. When tube 1 has fully grown and is lined with connective tissue from the inside, through the tubes 3 and 4, the space between the porous tube and the elastic tube is washed with a culture of epithelial cells and a little culture is left there. Viable epithelial cells are fixed to the connective tissue lining of the tube and form a continuous epithelial layer. Then perform access to the affected area of the trachea and prosthesis. The affected area of the trachea is excised. The prosthesis, without isolating from the surrounding tissues, is crossed in two places at the levels of the intersection of the trachea. Excess sections of the prosthesis with ends, an elastic tube and tubes are removed, and the cut edges of the prosthesis anastomose with the cut edges of the trachea.
In the postoperative period, anastomoses regenerate like a normal tracheal suture, while the entire prosthesis has an epithelial lining similar to the trachea itself, which means that it will not be exposed to stenosis or infection more than the trachea itself.
If the operating conditions require it, the prosthesis can be initially installed and epithelized in a round skin stem, and then on the feeding legs of the stem it is moved to the right place.
