Description
The invention relates to: it’s to medicine, namely, to traumatology, orthopedics and maxillofacial surgery, in particular, to the treatment of fractures of the tubules of the weak bones, as well as the fractures of the mandible. . A device for compression osteosynthesis is known, comprising a bracket made of titanium ikelide having a corrugated region Cl J in the middle part. A disadvantage of the known device. It is that it does not provide the necessary strength of osteosynthesis in case of non-transverse fractures of the tubular bones, since by creating this compression brace it acts on fragments on one side only. The goal of the invention is to increase the prostate osteosynthesis. This goal is achieved by the fact that in a device for compressive osteosynthesis, containing a bracket made of titanium nickelide, having a corrugated section in the middle part, the bracket is provided with a straight section parallel to the corrugated section, the length of the straight section exceeding the length of the corrugated section. plot 1.5-2 times. In addition, the bracket is provided with an additional corrugated section connected with it at the point of transition of the main corrugated section to the straight line section for bone osteosynthesis with a bone crest located directly under the skin, for example, at a fracture of the olecranon. FIG. 1 shows the device, top viewJ in FIG. 2 - the same, side view; in fig. 3 - the same, after deformation in liquid nitrogen. In FIG. 4, the same, in the working position during the synthesis of a diaphragmous fracture of a tubular bone; in fig. 5 - the device with an additional corrugated section, top view; in fig. b - the same, side view .; in fig. 7 - the same, in the working position with osteosynthesis of the olecranon, side view; in fig. 8 - the device, rear view. The device for compression osteosynthesis is made in the form of a staple with a corrugated section 1, ending with the end 2, a straight line section 3 and Additional corrugated section 4 ending with the end 5. Nickel-titanium is used as the material. The device works as follows. Nickel-titanium billet A new wire is heated to 573 ° K and a cramped tongs from one half of it make a corrugated section 1 ending with an end 2 and from. the second one forms a rectilinear section 3 so that it is located parallel to the corrugated section 1. After comparing the fragments 6 of the tubular bone, one hole 7 is drilled through the drill bit penetrating into the bone marrow channel so that the distance between the holes is 11, 5 cm was longer than the length of the corrugated section 1. Then the finished device was immersed in liquid nitrogen, where the bends of the corrugated section 1 were stretched, and the end 2 and straight linear section 3 were shaped to fit into the bone (Fig. 3). Next, the rectilinear section 3 is inserted through the opening 7 of one fragment 6 into the medullary canal and advanced through the fracture line, after which the end 2 of the corrugated section 1 is inserted into the opening 7 of the other fragment 6. After 15-40 seconds, the device tends to take an initial shape. thereby connecting the bone fragments 6 with uniform compression. The operative wound is sutured. For osteosynthesis of bone fragments with a bone ridge located directly above the skin, for example, at the fracture of the olecranon (Figure 2), the nickel-titanium wire blank in the heated state (at a temperature of 573 K) is bent in the middle until both halves are in contact. The latter are twisted among themselves about 2/3 of their length, i.e. form a rectilinear section 3, and then in the manner described above create bends on the corrugated sections 1 and 4 and bend at the ends 2i. 5. Further actions are performed similarly to the device used in the osteosynthesis of tubular bone fractures, the tips 2 and 5 of the corrugated sections 1 and. 4 is fixed in the holes 7 on the sides of the bone ridge. The proposed device allows to increase the strength of osteosynthesis, which contributes to shortening the time of treatment of patients and improving its outcome.
