Abstract
Полезная модель относится к медицинской технике, а именно к инструментам, применяемым в нейрохирургии, и может быть использована для удаления внутримозговых гематом при геморрагическом инсульте, черепно-мозговой травме. Канюля для удаления внутримозговых гематом выполнена в виде воронки, имеющей высоту 40-60 мм, диаметр большего основания - 23-27 мм. диаметр меньшего основания - 13-17 мм, причем на большем основании установлен ограничительный фланец. Использование полезной модели позволяет снизить травматичность и упростить операционную технику удаления внутримозговых гематом. Больная К., 54 лет, поступила с диагнозом "Острое нарушение мозгового кровообращения в бассейне правой средней мозговой артерии по геморрагическому типу. Внутримозговая гематома в правой теменно-височной области". Тяжесть состояния и расстройство сознания по шкале комы Глазго при поступлении составило 8 баллов, что соответствовало сопору. У больной имелась левосторонняя гемиплегия с гемианестезией. На компьютерной томографии выявлено латеральное кровоизлияние в правой теменно-височной области объемом около 90 см3, вызвавшее значительное смещение срединных структур головного мозга. Произведена операция, во время которой в правой теменно-височной области осуществлен линейный разрез кожи и мягких тканей длиной 3,5-4,0 см. Надкостница скелетирована. Корончатой фрезой диметром 2,5 см на кость наложено трефинационное отверстие и костный диск временно удален. Твердая мозговая оболочка вскрыта крестообразным разрезом. В безсосудистой зоне кора мозга коагулирована. Трубкой-расширителем, наружный диаметр которой 6 мм, осуществлена пункция полости гематомы сквозь паренхиму и на глубине 3,0-3,5 см добыта густая, черного цвета кровь. Шприцом удалось отсосать около 4,0 мл крови. Далее по первой трубке-расширителю проведена вторая трубка большего размера, внутренний просвет которой плотно совпадает с внешним диаметром первой, а наружный диаметр составляет 15 мм. Затем по второй трубке-расширителю провели воронкообразную канюлю высотой 40 мм с диаметром большего основания 25 мм и диаметром меньшего основания 15 мм. Основание канюли, имеющее ограничительный фланец, зафиксировали в костном окне. После извлечения обеих трубок-расширителей стали выделяться сгустки гематомы. Электроотсосом удалена большая часть гематомы. На большее основание воронкообразной канюли надет резиновый резервуар для сбора оставшихся сгустков крови. Для костного диска сделано временное ложе в подапоневротическом пространстве. В течение суток выделилось около 20 мл сгустков крови. Воронкообразная канюля и дренаж удалены, твердая мозговая оболочка зашита. Костный диск извлечен из подапоневротического пространства и уложен на свое место. Надкостница, апоневроз, кожа зашиты. Больной сделана контрольная компьютерная томография, где определялось около 10-15 мл оставшейся крови, при этом смещения срединных структур головного мозга не обнаружено. Состояние больной улучшилось. Больная выписана на 15 сутки. Состояние сознания оценивалось в 15 баллов, что соответствовало ясному сознанию. In the avascular zone, the cerebral cortex is coagulated. A tube-expander, the outer diameter of which is 6 mm, was used to puncture the hematoma cavity through the parenchyma and thick, black blood was obtained at a depth of 3.0-3.5 cm. About 4.0 ml of blood was sucked off with a syringe. Next, a second larger tube is drawn along the first expander tube, the inner lumen of which fits tightly with the outer diameter of the first, and the outer diameter is 15 mm. Then, a funnel-shaped cannula 40 mm high with a diameter of a larger base of 25 mm and a diameter of a smaller base of 15 mm was passed through a second tube-expander. The base of the cannula, having a restrictive flange, was fixed in the bone window. After removing both dilator tubes, hematoma clots began to stand out. An electric suction device removed most of the hematoma. A rubber reservoir is placed on the larger base of the funnel-shaped cannula to collect the remaining blood clots.
