Description
The utility model relates to medicine, namely to orthopedic dentistry, and can be used to restore the crown of the tooth.
To assess the novelty and technical level of the claimed solution, we consider a number of technical means known to the applicant for a similar purpose, characterized by a combination of features similar to the claimed device, known from information that has become publicly available prior to the priority date of the utility model.
The utility model allows to solve the complex problem associated with prosthetics with a destroyed crown. Untimely dental treatment, with a destroyed crown, entails the development of morphological changes in the structure of the dentition, leading to functional disorganization in the dentofacial system. The longer the pathology exists, the more difficult it is to restore the integrity of the dentition and articulatory balance.
There are many methods to eliminate tooth defects when the index of destruction of the occlusal surface of the tooth does not exceed 80%. However, with the complete destruction of the crown part of the tooth, the urgency of the problem of restoring the impaired morpho-functional unity of the dentition prompts the search for the most optimal rehabilitation measures. One of the directions in solving this problem is to preserve the roots of teeth suitable for prosthetics, thereby preventing the formation of defects and deformations of the dentition, atrophy of the alveolar processes, leading to a violation of the chewing efficiency and esthetic status of the patient.
Of particular importance is the preservation of tooth roots in cases of unjustified removal of them, when this leads to the formation of an end defect in the dentition and, as a result, to the need to make a removable denture or to perform an expensive, traumatic and often risky implantation operation.
To solve the problem of tooth root restoration, a variety of pin designs of dentures and, in particular, the so-called pin teeth are successfully used. A pin tooth is a structure that is fixed in the root canal with a pin and is used for subtotal or complete destruction of the natural crown of the tooth as an independent prosthesis, as well as for fixing fixed dentures.
Использование штифтов различной конструкции известно из патентов РФ №№2223715, 2252725.To date, a large number of pin teeth have been proposed, but the parts that are part of the root canal and artificial crown are required parts. The use of pins of various designs is known from the patents of the Russian Federation No. 2223715, 2252725.
The widespread use of pins is very relevant today, and the requirements of the time push for new developments of in-channel pins. Carbon fiber and fiberglass pins are referred to as “third generation pins”. They were preceded by metal pins of various designs. However, in practice, some shortcomings were revealed in the interaction of the metal pin with the root canal, namely: corrosion of the metal of the pin; lack of bond strength at the metal-dentin-cement boundary; poor aesthetics during restoration of anterior teeth; difficulties in removing the pin from the channel. In practice, many proposals have been formulated that explain the weakening of tooth functions due to pins cemented in the canal. The mechanical pressure exerted on the dentin of the root with metal pins can fracture the root both during the cementation process and during chewing. The reason for this is often a mismatch between the mechanical characteristics of the material of the metal pin and the hard tissues of the supporting root. For this reason, composites or glass ionomer cements are used. In addition, in the process of cementing a canal, the interaction of materials such as a pin, a composite for stump restoration, a composite for cementing and root dentin is important. If they have different chemical and mechanical indicators, then a structure made of these materials at certain points takes on an increased concentration of the load, destroying the preserved structure of the tooth. It is necessary to achieve a homogeneous system (monoblock) of the listed metals that can integrate with each other and have similar biomechanical indicators. Obviously the perfect pin for this purpose
Эти обстоятельства привели к созданию штифтов третьего поколения из карбоновых и стеклянных волокон, расположенных продольно и погруженных по особому заводскому методу в эпоксидную пластмассовую матрицу (Bis.G.Ma), которая составляет 36,6% от всего веса штифта, см газету «Стоматология сегодня, №5(27), 2003 г.must have a modulus of elasticity, as in dentin of a natural tooth (20 GPA). These circumstances led to the creation of third-generation pins made of carbon and glass fibers, arranged longitudinally and immersed in a special factory method in an epoxy plastic matrix (Bis.G.Ma), which makes up 36.6% of the total weight of the pin, see the newspaper “Dentistry Today No. 5 (27), 2003
The fibers are a reinforcing element and comprise 63.4% of the weight of the pin. Carbon and glass fibers are continuous, and their tension is constant. They are located along the main axis of the pin. This longitudinal structure of the fibers provides an even distribution of loads on the hard tissues of the tooth. Chewing loads are completely perceived and distributed, preventing their accumulation at the edges of the root, which significantly reduces the risk of a radicular fracture. Thus, fiberglass and carbon fiber pins represent the most optimal product among intra-channel pins designed for tooth restoration. The characteristics and advantages of fiberglass and carbon fiber pins are as follows:
- модуль эластичности, приближенный к дентину, уменьшает до минимума опасность продольного перелома корня;- the elastic modulus close to dentin minimizes the risk of a longitudinal root fracture;
- поверхность карбонового и стекловолоконного штифтов негладкая из-за наличия волокон, что заметно облегчает микромеханическую адгезию композита;- the surface of the carbon fiber and fiberglass pins is not smooth due to the presence of fibers, which significantly facilitates the micromechanical adhesion of the composite;
- отсутствие окисления и коррозии обеспечивает стабильность штифтов и их биосовместимость с дентином и композитным цементом;- the absence of oxidation and corrosion ensures the stability of the pins and their biocompatibility with dentin and composite cement;
- устойчивость к растяжению усиливается за счет создания моноблока из штифта, композита для восстановления культи, композита для цементировки и корневого дентина, имеющих схожие характеристики. Эта система уменьшает нагрузки на сохранившиеся ткани зуба;- resistance to stretching is enhanced by creating a monoblock of a pin, a composite for restoration of the stump, a composite for cementing and root dentin, which have similar characteristics. This system reduces the load on the preserved tooth tissue;
- стеклянные и кварцевые штифты дают лучший эстетический результат, особенно при их использовании для реставраций или изготовления безметалловых коронок на фронтальных зубах;- glass and quartz pins give the best aesthetic result, especially when used for restorations or the manufacture of metal-free crowns on the front teeth;
- стекловолоконные штифты больше рекомендованы для реставраций на фронтальных зубах, а карбоволоконные штифты - на жевательных, поскольку - fiberglass pins are more recommended for restorations on the anterior teeth, and carbon fiber pins on chewing ones, since
их механические характеристики выше, чем у стекловолоконных штифтов;their mechanical characteristics are higher than that of fiberglass pins;
- карбоволоконные и стекловолоконные штифты являются рентгеноконтрастными, что облегчает определение места их фиксации;- carbon fiber and fiberglass pins are radiopaque, which facilitates the determination of the location of their fixation;
Fiber pins are easy to use and can also be easily removed with a drill.
