Description
This invention application claims the priority of Chinese Patent Application No. 202011477966.6, filed December 15, 2020, which for all purposes is incorporated herein by reference.
FIELD OF THE INVENTION
The present application for invention relates generally to the field of medicine, and in particular to the use of mitoxantrone hydrochloride liposome in the treatment of ovarian cancer, gastric cancer or squamous cell carcinoma of the head and neck.
BACKGROUND ART
Ovarian cancer is one of the most common malignant neoplasms in women. Due to the lack of typical symptoms and signs, ovarian cancer is mostly in the middle or late stage once diagnosed, and patients have very short survival periods. Ovarian cancer has many pathological types, and the most common type is epithelial ovarian cancer, accounting for 70% of cases, followed by malignant terminal tumors and ovarian sex cord stromal tumors.
The overall prognosis of epithelial ovarian cancer is poor due to the difficulty of early diagnosis and the lack of effective treatment for drug-resistant recurrent epithelial ovarian cancer. Surgery along with chemotherapy is the main therapeutic regimen for ovarian malignancies. Chemotherapy plays an important role in adjuvant treatment and treatment of relapse. Platinum-based combination chemotherapy is the main recommended postoperative chemotherapy regimen for advanced ovarian cancer. Between 70% and 80% of patients relapse, even if complete remission is achieved with the above treatment. Currently, the initial standard chemotherapy regimen for advanced epithelial ovarian cancer is platinum-based combination chemotherapy. The choice of specific regimens should take into account the personalized characteristics of patients. Other recommended treatment regimens include carboplatin along with the anthracycline doxorubicin liposome. Additionally, doxorubicin liposome is recommended for use alone or in combination with bevacizumab for the treatment of recurrent drug-resistant ovarian cancer. However, among all the current monotherapies for recurrent platinum-resistant ovarian cancer, none is better than the others in terms of safety and efficacy.
Stomach cancer is the fifth most common cancer in the world. For patients who cannot obtain curative treatment through surgery or suffer from metastatic gastric cancer, it is now recognized that comprehensive treatment options based on systemic drug therapy should be adopted. Commonly used systemic chemotherapy drugs include 5-fluorouracil (5-FU), capecitabine, tigio, cisplatin, oxaliplatin, paclitaxel, docetaxel, albumin paclitaxel, irinotecan, and epirubicin. Targeted therapies include trastuzumab and apatinib. Treatment of advanced recurrent/metastatic gastric cancer is difficult, especially with poor efficacy and limited second-line and third-line treatment options.
The incidence rates of head and neck tumors rank sixth worldwide, including tumors originating from the nasal cavity, sinus, nasopharynx, oropharynx, hypopharynx, cervical esophagus, thyroid, salivary gland, oral cavity, larynx, and ear. 90% of head and neck tumors are squamous cell carcinomas of the head and neck (SCCHN), with more than 550,000 new cases and more than 300,000 deaths per year worldwide. Most patients cannot be cured due to local recurrence and metastasis. The five-year survival rate is less than 50%. Surgery and chemotherapy do not achieve satisfactory treatment results.
Among patients with recurrent head and neck tumors, only a small proportion can be treated with definitive local treatments such as surgery or radiation therapy, and the majority, such as those with metastasis, receive any palliative systemic therapy (PS 0-1) or optimal supportive care (PS≥2). Cisplatin in combination with 5-FU (PF regimen) or in combination with paclitaxel is a common option in first-line chemotherapy regimens. Cisplatin can be replaced with carboplatan if the patient is not suitable for cisplatin. All of the above regimens may additionally include cetuximab.
PD-L1.For recurrent and metastatic head and neck squamous cell carcinoma that has failed first-line platinum-based therapy, there is currently no standard treatment regimen. Commonly used drugs are methotrexate, paclitaxel, docetaxel, cetuximab or immune checkpoint inhibitors. For patients who have failed first-line platinum-based therapy and require second-line/conservative treatments, immunotherapy drugs or other regimens may be selected based on PD-L1 test results.
Thus, for treatment-resistant cancers such as ovarian cancer, gastric cancer or squamous cell carcinoma of the head and neck, there is an urgent need for better therapeutic products and treatment regimens.

DETAILED DESCRIPTION OF THE INVENTION
Mitoxantrone hydrochloride is an anthraquinone anticancer drug. Indications for use approved by the FDA include multiple sclerosis, prostate cancer and acute myeloid leukemia. Clinical studies have demonstrated some effectiveness against malignant lymphoma, breast cancer, acute myeloid leukemia, lung cancer, melanoma, soft tissue sarcoma, multiple myeloma, liver cancer, colorectal cancer, kidney cancer, prostate cancer, endometrial cancer, testicular tumors, ovarian cancer and head and neck cancer. Clinical dosing of mitoxantrone hydrochloride is limited due to relatively serious side effects such as bone marrow suppression, leukopenia and thrombocytopenia (dose-limiting toxicity), palpitations, premature beats, or abnormal electrocardiograms. Typically, mitoxantrone hydrochloride must be used in combination with other drugs. However, previous studies have demonstrated that the effectiveness of these combination treatment regimens was not ideal. Additional improvement in antitumor efficacy without serious toxic side effects is a challenge in the clinical use of mitoxantrone hydrochloride.
Liposome is a new form of drug carrier. In pharmaceutical definitions, liposomal drugs generally refer to microvesicles formed by encapsulating the drug in a lipid bilayer. Studies have shown that liposomal drugs can alter the distribution of encapsulated drug in vivo, allowing the drug to accumulate primarily in tissues and organs such as the liver, spleen, lung and bone marrow, thereby increasing the therapeutic index, decreasing the therapeutic dose and reducing toxicity. These characteristics make the use of liposome-loaded drug of much greater importance in anticancer drug research. The authors of the present application have conducted studies on liposomal compositions of mitoxantrone. For example, Chinese Patent Application No. 200610102339.8, filed December 29, 2006, and PCT Publication WO 2008/080367A1, filed December 29, 2007, disclose a mitoxantrone liposome. Descriptions of the two above references are included here for all tasks in full.
Mitoxantrone liposomal preparation is a special preparation and is different from regular mitoxantrone injections. Its absorption, distribution and metabolism after entering the body differ significantly. Thus, it is difficult to provide a suitable treatment regimen according to the indication for a compound among different indications or among different tumor types. Therefore, the efficacy and indications for liposomal formulations of mitoxantrone should be studied independently. To this end, the inventors of the present invention have carefully studied the effectiveness of mitoxantrone liposomal compositions in difficult-to-treat ovarian cancer, gastric cancer or head and neck squamous cell carcinoma, and have created various inventions in the present application.

When "about" is used here for a numeric value or range of numerical values, it represents the degree of variation of the numerical value acceptable in the art, and usually means ±10% based on the numerical value or range of numerical values, such as ±9% or ±8% or ±7% or ±6%, or ±5%, or ±4%, or ±3%, or ±2%, or ±1%.
As used herein, the term "pharmaceutical composition" means a combination of at least one drug and, optionally, a pharmaceutically acceptable carrier or adjuvant to function together to achieve a specific pharmaceutical objective. In the technical context of liposome compositions according to the present application, pharmaceutically acceptable carriers are typically aqueous carriers, for example, water, aqueous buffers, isotonic saline solutions (eg, saline or phosphate buffered saline (PBS)), or sugar solutions. While injection is a suitable form of composition for the purposes of this application (and thus there are appropriate types of pharmaceutically acceptable carriers), those skilled in the art will recognize that other types of pharmaceutically acceptable carriers may be selected when other forms of composition are possible (e.g. , compositions for oral administration).
1. Mitoxantrone hydrochloride liposome and composition Mitoxantrone hydrochloride (mitoxantrone HCl) is an anthraquinone antitumor drug having the following structure:
In a broad sense, a liposome is an artificial membrane. In water, the hydrophilic head of the phospholipid molecule is immersed in water, the hydrophobic tail of the liposome extends into the air, and after shaking, a spherical liposome of lipid bilayer molecules is formed. In the biomedical field, the liposome is commonly used in transgenic processes or drug manufacturing. The liposome can be fused to cell membranes to deliver drugs inside cells. In pharmaceutical definitions, liposomal drugs generally refer to microvesicles formed by encapsulating the drug in a lipid bilayer. The chemical composition of liposomes typically includes phospholipids (including natural and synthetic phospholipids) and cholesterol.
Methods for preparing liposomal pharmaceutical compositions are known in the art. Technical guidance for the preparation of liposomal compositions of mitoxantrone hydrochloride can be found by one skilled in the art.
In some embodiments, the mitoxantrone hydrochloride liposome is the only active ingredient in the drug.
In the technical field of the present application, the present inventors have found that mitoxantrone hydrochloride liposome preparations prepared by the method disclosed in Chinese Patent Application No. 200610102339.8 or PCT Publication WO 2008/080367A1 have good efficacy.
Without wishing to be bound by theory, the present inventors have discovered that one or more than one or all of the following properties are advantageous for the mitoxantrone hydrochloride liposome composition.
NanoZS.First, in liposomal compositions/drugs, the particle size of the mitoxantrone hydrochloride liposomes is from about 30 to 80 nm, such as about 30, 25, 40, 45, 50, 55, 60, 65, 70, 75 or 80 nm. More suitable ranges include from about 35 to 75 nm, from about 40 to 70 nm, and from about 40 to 60 nm. In one example (see examples below), the particle size of mitoxantrone hydrochloride liposomes is approximately 60 nm. Particle size can be measured in a variety of ways, including but not limited to NanoZS.
Second, mitoxantrone hydrochloride reacts with multivalent counterions (eg, sulfate, citrate, or phosphate) in the liposome to form a poorly soluble precipitate.
PEG2000 (DSPE-PEG2000).Third, the phospholipid bilayer in the mitoxantrone hydrochloride liposome contains phospholipids having a phase transition temperature (Tm) higher than body temperature (eg, 36 to 38°C), such that the phase transition temperature of the liposome is higher than body temperature. Phospholipid molecules, which are important components of liposomes formed by different kinds of phospholipid molecules, have solid-gel-liquid phase transition properties at different temperatures, and the composition of phospholipids largely determines the phase transition temperature of the liposome. Phospholipid particles useful herein to achieve the desired phase change temperatures include, but are not limited to, hydrogenated soy phosphatidylcholine, phosphatidylcholine, hydrogenated egg yolk lecithin, bispalmitate lecithin, bisstearate, or any combination thereof. In addition, one skilled in the art can select other suitable phospholipid particles and combination proportions based on tests of phase change temperatures. In some embodiments, in addition to cholesterol, the mitoxantrone hydrochloride phospholipid bilayer liposomes of the present application contain phospholipid molecules including hydrogenated soy lecithin and distearoylphosphatidylethanolamine modified PEG2000 (DSPE-PEG2000).
In some embodiments, the phospholipid bilayer in the mitoxantrone hydrochloride liposome contains hydrogenated soy phosphatidylcholine, cholesterol and distearoylphosphatidylethanolamine modified with PEG2000 in a mass ratio of approximately 3:1:1, mitoxantrone hydrochloride reacts with a multivalent acid ion in the liposome to form a poorly soluble precipitate, and the liposome particle size mitoxantrone hydrochloride in the drug is approximately 60 nm.
In liposomal pharmaceutical compositions, "lipid to drug ratio" refers to the mass ratio of the components (including phospholipids and cholesterol) of the phospholipid bilayer in the liposome to the drug (mitoxantrone in this application). For example, in an example of a liposomal pharmaceutical composition according to the present application, the lipid to drug ratio refers to the mass ratio of the phospholipid bilayer components (including HSPC, DSPE-PEG2000 and Chol) in the liposome to mitoxantrone.
As a non-limiting example, liposomal compositions of mitoxantrone hydrochloride in accordance with the present application can be prepared in accordance with the following protocols.
Hydrogenated soybean phosphatidylcholine (HSPC), cholesterol (Chol) and distearoylphosphatidylethanolamine modified PEG2000 (DSPE-PEG2000) were weighed in a mass ratio of 3:1:1 and dissolved in 95% ethanol to obtain a clear solution (i.e. ethanol phospholipid solution) . An ethanol solution of phospholipids was mixed with a 300 mM ammonium sulfate solution. After shaking for 1 hour at 60-65°C for hydration, a heterogeneous multicompartmental liposome was obtained. The liposome particle size was then reduced using a microfluidic device. The resulting sample was diluted 200 times with 0.9% NaCl solution and detected using NanoZS. The average particle size was approximately 60 nm. The main peak is concentrated between 40 and 60 nm. The ammonium sulfate of the outer phase of the control liposome was then removed using an ultrafiltration device, and the outer phase was replaced with 290 mM sucrose and 10 mM glycine to form a transmembrane ammonium sulfate gradient. A solution of mitoxantrone hydrochloride (10 mg/ml relative to mitoxantrone) was added to the initial liposome in a lipid/drug ratio of 16:1 at a drug loading temperature of 60-65°C. After incubation for approximately 1 hour, encapsulation efficiency can be demonstrated to be approximately 100% using particle size exclusion chromatography. The product thus obtained was named PLM60 and was used in the examples described below. The mass ratio of HSPC:Chol:DSPE-PEG2000:mitoxantrone in PLM60 was 9.58:3.19:3.19:1. The osmolality of the glycine sucrose solution is close to the physiological value.
It is understood that various technical details and parameters in the above example production methods can be adjusted and determined by those skilled in the art within reasonable limits. For example, amino acids that can be used to replace glycine in the outer phase used to form a transmembrane ammonium sulfate gradient include, but are not limited to, histidine, asparagine, glutamic acid, leucine, proline, and alanine. In another example, the weight ratio of HSPC, Chol and DSPE-PEG2000 can be adjusted accordingly. As another example, for lipid to drug ratios in the manufacture of specific liposomal pharmaceutical compositions, one skilled in the art can develop, test and ultimately determine suitable lipid to drug ratios in order to maximize drug loading while reducing leakage medicine. For liposomal compositions of mitoxantrone hydrochloride according to the present application, the lipid to drug ratios that can be used are in wide ranges, for example, from a minimum of 2:1 to a maximum of 30:1, 40:1 or 50:1, and more suitable ratios The lipid to drug ratio may be from about 15 to 20:1, for example about 15:1, 16:1, 17:1, 18:1, 19:1 or 20:1. Thus, some of the beneficial properties of the mitoxantrone hydrochloride liposomal formulations described above are more important, and various methodologies exist to achieve these properties.

3. Dose schedules
As described above, liposomal drugs refer to a specific type of drug. The absorption, distribution, and metabolism of a liposomal drug once it enters the body may differ compared to the free drug. Thus, dosing regimens based on free drug development and testing are not necessarily suitable for liposomal formulations containing the same drug. Likewise, the dosing schedule determined for a liposomal pharmaceutical composition during development and testing for a particular type of cancer is not necessarily suitable for administration of a liposomal pharmaceutical composition for other types of cancer.
Previous studies have demonstrated that the safe and effective dose of a single drug can vary significantly for different indications, as follows.
Doxil (liposomal formulation of doxorubicin hydrochloride) has three FDA-approved indications, i.e. (1) ovarian cancer with a recommended dose of 50 mg/m 2 by intravenous administration once every four weeks; (2) Kaposi's sarcoma with a recommended dose of 20 mg/m 2 by intravenous administration once every three weeks; and (3) multiple myeloma with a recommended dose of 30 mg/m 2 by intravenous administration on the fourth day after administration of bortezomib.
Abraxane (paclitaxel injection [albumin-bound]) has three FDA-approved indications with different dosing schedules, i.e. (1) metastatic breast cancer with a recommended dose of 260 mg/m 2 by intravenous drip over 30 minutes, once for three weeks; (2) non-small cell lung cancer with a recommended dose of 100 mg/ m2 by intravenous drip over 30 minutes and a therapeutic course of 21 days with administration on days 1, 8 and 15, respectively (on day 1 immediately after administration of paclitaxel injection (albumin -related), carboplatin is administered and then administered once every 21 days); and (3) pancreatic cancer with a recommended dose of 125 mg/ m2 by intravenous drip over 30-40 minutes and a therapeutic course of 28 days with administration on days 1, 8 and 15, respectively (for each time of therapy, immediately after administration paclitaxel for injection (albumin-bound), gemcitabine is administered).
Ambisome (a liposomal formulation of amphotericin B for injection) has different indications with different starting doses. For empirical treatment, the recommended dose is 3 mg/kg/day. For the treatment of systemic fungal infection (Aspergillus, Candida or Cryptococcus), the recommended dose is 3 to 5 mg/kg/day. For the treatment of cryptococcal meningitis in HIV (human immunodeficiency virus) infected patients, the recommended doses are 6 mg/kg/day (days 1-5), and 3 mg/kg/day (days 4 and 21). For the treatment of immunocompromised patients suffering from visceral leishmaniasis, the recommended doses are 4 mg/kg/day (day 15), and 4 mg/kg/day (days 10, 17, 24, 31 and 38). The inventors of the present invention expected that the mitoxantrone liposome, after being administered to the human body by intravenous infusion, exhibited the effects of slow release, targeting, facilitation and increased efficacy. The administered dose was higher than the normal injection dose. As a single agent treatment, administration of mitoxantrone liposome could improve efficacy and reduce the incidence of side effects.

1. Research protocols
All participating subjects were treated exclusively with mitoxantrone hydrochloride liposome injections at a dose of 20 mg/m 2 once for three weeks (q3w). The study design included at least 30 subjects (men or women) suffering from recurrent/metastatic squamous cell carcinoma of the head and neck. Screening tests were completed and baseline assessments were performed 28 days before administration. After determining inclusion/exclusion criteria, participating subjects received therapies. All participating subjects received 8 courses of treatment. Treatment may be discontinued when disease progression (PD), intolerable toxicity or death is detected, or long-term benefit cannot be achieved in the opinion of the investigators. Administration may be delayed after the second course, but the delay should not exceed three weeks. Dose adjustment can be made after the second course to a minimum dose of 12 mg/ m2 . Safety and efficacy assessments were performed following administration according to protocols. Observation was carried out 28 days after the last dose. Subjects who withdraw early from the study should be followed up after the last dose if possible.
After the last dose, the observation period began. Tumor assessments continued every six weeks for subjects who withdrew from the study for reasons other than radiographically assessed disease progression or receipt of new anticancer therapy until disease progression was detected. Survival surveillance was performed every six weeks when the subject developed radiologically detectable disease progression or began receiving new antineoplastic therapy.
Each subject's study includes a selection period, a treatment period, and an observation period.
After subjects signed the informed consent form and completed all baseline tests during the eligibility period, subjects who qualified for participation were treated with a mitoxantrone hydrochloride injection in order of participation. In accordance with the protocols, all subjects received the tests specified in the protocols throughout the study treatment period to assess safety.
2. Duration of the study
The study included a screening period of four weeks (28 days) and a treatment period of 8 courses (24 weeks). Safety monitoring occurred four weeks (28 days) after the last dose. PFS observations were then made every six weeks, and survival observations were made every six weeks after disease progression. The expected study duration for each patient was approximately 12 months.
The study design included at least 30 subjects, and the entire study duration is expected to be between 24 and 36 months.

The inventors anticipate the use of mitoxantrone liposomes. The drug has the effects of slow release, targeted action, relief and increased effectiveness after intravenous infusion into the human body. The dose is not only higher than that of a conventional injection, but also represents a monotherapy, which may improve efficacy and reduce the incidence of side effects.
A total of 34 subjects were enrolled as of November 5, 2021 (the study is still ongoing), including 23 cases of nasopharyngeal carcinoma and 11 cases of non-nasopharyngeal carcinoma (five cases of tongue carcinoma, two cases of hypopharyngeal carcinoma, two cases of tonsil carcinoma, one case of laryngeal carcinoma and one case of gingival carcinoma). All subjects were administered mitoxantrone hydrochloride liposome by injection at a dose of 20 mg/m 2 every three weeks (q3w). Efficacy was assessed after every two courses. 19 subjects received at least one efficacy assessment (including 18 subjects suffering from nasopharyngeal carcinoma and 1 subject suffering from non-nasopharyngeal carcinoma (hypopharyngeal carcinoma)). The results of the efficacy analysis showed that the overall ORR was 42.1% (8/19) and the DCR was 78.9% (15/19). The ORR for nasopharyngeal carcinoma was 38.8% (7/18) and the DCR was 77.8% (14/18). One case of hypopharyngeal carcinoma was assessed as PR. None of the patients experienced intolerable side effects.
