Изобретение относится к медицине, а именно к медицинской реабилитации, лечебной физкультуре, физиотерапии и пульмонологии, и может быть использовано для медицинской реабилитации, восстановительного и санаторно-курортного лечения больных перенесших двухстороннюю пневмонию, ассоциированную с новой коронавирусной инфекцией (COVID-19).The invention relates to medicine, namely to medical rehabilitation, physical therapy, physiotherapy and pulmonology, and can be used for medical rehabilitation, rehabilitation and sanatorium-resort treatment of patients who have suffered bilateral pneumonia associated with a new coronavirus infection (COVID-19).
Новая коронавирусная инфекция (COVID-19) стремительно охватила практически каждую страну на планете, впоследствии приобретя характер пандемии. COVID-19 представляет собой острое вирусное заболевание с преимущественным поражением органов дыхания. Наряду с этим ведущим клиническим проявлением COVID-19 является двусторонняя пневмония. Необходимо подчеркнуть, что пациенты, перенесшие среднетяжелую или тяжелую форму пневмонии, ассоциированную с COVID-19, часто страдают от таких жизнеугрожающих осложнений, как дыхательная недостаточность, тромботические осложнения, нарушения со стороны нервной системы и др. Соответственно, пациенты, после перенесенных среднетяжелых и тяжелых форм COVID-19 нуждаются в обязательных реабилитационных мероприятиях. Однако медицинское сообщество на сегодняшний день столкнулось с ситуацией, при которой научных данных полученных на принципах доказательной медицины об эффективности оказания медицинской помощи по реабилитации пациентов с COVID-19 недостаточно, так как это заболевание является новым. Также, необходимо учитывать сложность патогенеза заболевания, которая обуславливает недостаточную эффективность применения известных методов реабилитации после COVID-19, что в свою очередь делает актуальным поиск новых подходов, разработку и совершенствование методов реабилитации пациентов с пневмонией, ассоциированной с COVID-19.The new coronavirus infection (COVID-19) has rapidly spread to almost every country on the planet, subsequently becoming a pandemic. It must be emphasized that patients who have suffered a moderate or severe form of pneumonia associated with COVID-19 often suffer from such life-threatening complications as respiratory failure, thrombotic complications, disorders of the nervous system, etc. Accordingly, patients who have suffered moderate and severe forms of COVID-19 require mandatory rehabilitation measures. However, the medical community today is faced with a situation in which scientific data obtained on the principles of evidence-based medicine on the effectiveness of providing medical care for the rehabilitation of patients with COVID-19 is insufficient, since this disease is new. Also, it is necessary to take into account the complexity of the pathogenesis of the disease, which determines the insufficient effectiveness of the use of known methods of rehabilitation after COVID-19, which in turn makes it urgent to search for new approaches, develop and improve methods of rehabilitation of patients with pneumonia associated with COVID-19.

При поступлении в санаторно-курортное учреждение у больных оцениваются показания и противопоказания к процедурам, включенным в данный способ реабилитации. При прохождении тропы здоровья за счет чередования подъемов и плато, а также правильного использования скандинавских палок, физическая нагрузка у пациентов формируется по интервальному типу. Использование в реабилитации больных, перенесших пневмонию, ассоциированную с новой коронавирусной инфекцией аэробной интервальной физической нагрузки на занятиях скандинавской ходьбой в комплексе с базовыми процедурами приводит к повышению уровня качества жизни и в отдаленном периоде после реабилитации, что подтверждается значительным снижением выраженности симптомов заболевания, несмотря на высокую суммарную нагрузку, а также показателями соматического здоровья по сравнению с исходным в 2 раза, более выраженным терапевтическим эффектом на физическую работоспособность с повышением статических и динамических дыхательных объемов, показателей толерантности к гипоксии, снижению реактивности сердечно-сосудистой системы на физическую нагрузку с повышением ее переносимости. Преимущества интервальных физических нагрузок заключаются в отсутствии осложнений, побочных явлений, лекарственной зависимости и аллергии у больных, которые могут наблюдаться у больных перенесших двухстороннюю пневмонию, ассоциированную с новой коронавирусной инфекцией, такте нагрузки хорошо переносятся больными. Предлагаемым способом реабилитационные мероприятия были проведены 24 пациентам с диагнозом перенесенной пневмонии, ассоциированной с новой коронавирусной инфекцией (COVID-19), при этом данные пациенты были включены в клиническое исследование по оценке эффективности предложенной реабилитационной программы с оформлением добровольного информированного согласия на каждого пациента и сравнением с группой пациентов получавших только базовый комплекс реабилитационных мероприятий. The use of aerobic interval physical activity in Nordic walking classes in combination with basic procedures in the rehabilitation of patients who have suffered pneumonia associated with a new coronavirus infection leads to an increase in the level of quality of life in the long-term period after rehabilitation, which is confirmed by a significant decrease in the severity of symptoms of the disease, despite the high total load, as well as indicators of somatic health in comparison with the initial one by 2 times, a more pronounced therapeutic effect on physical performance with an increase in static and dynamic tidal volumes, indicators of tolerance to hypoxia, a decrease in the reactivity of the cardiovascular system to physical activity with an increase in its tolerance. The advantages of interval physical activity are the absence of complications, side effects, drug dependence and allergies in patients, which can be observed in patients who have had bilateral pneumonia associated with a new coronavirus infection; the exercise cycle is well tolerated by patients. Using the proposed method, rehabilitation measures were carried out on 24 patients diagnosed with pneumonia associated with a new coronavirus infection (COVID-19), and these patients were included in a clinical study to evaluate the effectiveness of the proposed rehabilitation program with voluntary informed consent for each patient and comparison with a group of patients who received only a basic set of rehabilitation measures. The patients tolerated all the procedures included in this method of rehabilitation well; no side effects from the procedures were observed.

В основной группе пациентов, при оценке функциональных систем отмечалось улучшение результатов по шкале MRC на 49% (р<0,05), по пробе Штанге на 17,9% (р<0,05), по пробе Генчи на 17,8%) (р<0,05), по тесту шестиминутной ходьбы на 10,7% (р<0,05), по пробе Мартине до нагрузки на 7,8% (р<0,05) и после нагрузки на 8,9% (р<0,05). При этом у пациентов группы сравнения, дополнительно посещающих занятия по скандинавской ходьбе, отмечалось улучшение результатов по шкале MRC на 63,8% (р<0,05), по пробе Штанге на 32,3% (р<0,05), по пробе Генчи на 29,9% (р<0,05), по тесту шестиминутной ходьбы на 16,9%) (р<0,05), по пробе Мартине до нагрузки на 11,8% (р<0,05) и после нагрузки на 13,8% (р<0,05). The results of the clinical study showed that after a course of rehabilitation measures in both groups of patients, the severity of shortness of breath statistically significantly decreased, the body’s resistance to physical activity increased, which generally reflects the positive impact of the rehabilitation programs used on the functioning of the cardiorespiratory system, adaptive reserves and fitness of the body.
При сравнительном анализе динамики функционального состояния пациентов выяснилось, что исходные данные полученные в группах статистически значимо не различались, однако, после комплекса реабилитационных мероприятий в группе сравнения прирост результатов был выше по всем шкалам, пробам и тестам (р<0,05), чем в основной группе. Наряду с тем, в основной группе наблюдалось уменьшение уровня тревоги пациентов на 45,7% (р<0,05) и депрессии на 41,2% (р<0,05), а также улучшение качества жизни на 18,3% (р<0,05). В группе сравнения отмечалось уменьшение уровня тревоги пациентов на 57,4% (р<0,05) и депрессии на 59,6% (р<0,05), а также улучшение качества жизни на 27,7% (р<0,05).
