Abstract
Description
The invention relates to the field of medicine, namely to pharmacy, and can be used in dentistry for the treatment of inflammatory and destructive periodontal diseases.
The relevance of the search for new drugs for the treatment of inflammatory-destructive periodontal diseases is associated not only with the high prevalence among different age groups of the population, but also with the tendency of this pathology to progress and influence both the dental system and the body as a whole. Currently, on the Russian pharmaceutical market, the range of drugs in the form of dental gels for the treatment of periodontal pathology is small.
Medicines of synthetic origin predominate, which have a number of disadvantages, such as high sensitizing activity, a high risk of developing resistance in pathogenic and saprophytic microflora and other side effects.
There is a known method of treatment using a therapeutic and prophylactic drug for oral hygiene in the form of oral baths (RU 2406510), prepared as follows: dry pantohematogen, obtained according to TU 9158-027-40587098-06, in an amount of 1 g is mixed with 4 g of dry extract bergenia according to TU 9370-27-02068315-04, after thorough mixing, the contents are packaged in 500 mg capsules or in plastic bags. To treat the oral cavity, 1 capsule is diluted in 200 ml of boiled water, cooled to room temperature, and infused for 1.5-2 hours.
The disadvantage of this known method is the inconvenience of use and low efficiency due to the lack of adhesion of the dosage form, in the form of oral baths, to the mucous membrane of the oral cavity and gums, and, as a result, rapid erosion by saliva, reducing the diffusion of the active component in the source of inflammation.
%:A gel-based dosage form of pantohematogen for external use and a method for its preparation are known (patent RU 2200564), having the following ratio of components, wt. %:
5-35Pantohematogen in the form of a 3% aqueous-alcohol solution containing 8-14% ethyl alcohol - 5-35
0,5-15Polymer gel-forming composition - 0.5-15
Water - the rest
%:The dosage form can be characterized in that the polymer gel-forming composition contains, by weight. %:
Полимерный гелеобразователь - редкосшитый полимер акриловой кислоты или поливинилпирролидон - 1,2-5,4Polymer gelling agent - rarely cross-linked polymer of acrylic acid or polyvinylpyrrolidone - 1.2-5.4
6-8.Glycerin and/or propylene glycol - 3.0-60.0 Preservatives nipagin and nipazole - 0.06-0.36 Triethanolamine - up to pH 6-8.
The disadvantage of the known dosage form is the impossibility of use in dental practice due to the lack of a carbomer brand and prescription, with an accurate indication of the concentration of pantohematogen and excipients that provide the necessary organoleptic characteristics of the gel, as well as clinical examples proving the regenerating and anti-inflammatory effect. For the dosage form - dental gel, applied to the oral mucosa, a lower concentration of the active ingredient - pantohematogen - is required.
The closest to the claimed invention in terms of the achieved result is a method for producing a dental gel with an adaptogen for the treatment of inflammatory periodontal diseases (RU 2740450), namely the addition of a finely dispersed pantohematogen to the composition of the medical and cosmetic gel “Asepta with propolis”, as an active substance, in as an excipient - vaseline oil in the following ratio of components:
Asepta gel with propolis 10 g
Finely dispersed pantohematogen 0.05 g - 0.1 g
Vaseline oil 0.025 g - 0.05 g
A disadvantage of the known method may be a decrease in the adhesion of the gel to the oral mucosa due to the introduction of a hydrocarbon lipophilic auxiliary substance - petroleum jelly into the composition. Thus, the delivery of the active ingredients of pantohematogen to the site of inflammation is not prolonged, as a result of the rapid washing out of the gel with saliva. In addition, rubbing pantohematogen with a lipophilic liquid can reduce the release of hydrophilic substances from the gel, which are predominant in pantohematogen.
The authors propose a method for producing a dental anti-inflammatory gel with pantohematogen for the treatment of inflammatory-destructive periodontal diseases.
The technical result of the claimed invention is the expansion of the range of drugs of natural origin for the treatment of inflammatory and destructive periodontal diseases through the use of a prolonged action application dosage form - dental gel.
The technical result of the invention is achieved by using dried deer blood as the active substance of the dental gel - pantohematogen (TU 10.89.19-001-71220805-21 “Products of deer antler powder (powder) Technical conditions”), which is a fine red-brown powder colors with a specific smell.
95%.An acrylic acid derivative, the rare cross-linked acrylic polymer carbopol, was chosen as the hydrophilic base of the gel. Other excipients: solvent - purified water; co-solvent and plasticizer - glycerol and propylene glycol; pH regulator - sodium hydroxide; taste regulators - menthol, sorbitol solution 70% non-crystallizing, Etol coffee cognac flavoring; preservative - ethanol 95%.
Dental gel with pantohematogen is prepared as follows: the calculated amount of carbomer-980 is evenly applied to the surface of purified water, left until the polymer is completely dissolved, sodium hydroxide solution is added to the required gel-like consistency (pH = 6-8), after which pantohematogen is ground with glycerin and five times volume of purified water and mixed with the resulting gel base, then gradually add propylene glycol, menthol alcohol solution, sorbitol solution 70% non-crystallizing, Etol Cognac coffee flavoring.

(Испания).The rheological properties of gel compositions of optimal composition were determined on a rotational viscometer “NDL-1”, SOMESTA, S.A. (Spain).
The viscosity coefficient of the gels (η) ranged from 71±0.7 to 80±0.6 (Pa*s), which indicates that this gel with pantohematogen has a fairly high viscosity at a low shear rate - 0.1 s-1, which is typical for dental gels, as a result of this, the rheological parameters of the studied gels can be considered satisfactory.
A comparative assessment of the adhesive properties of gels with pantohematogen was carried out in comparison with factory-produced dental gels registered in the State Register of Medicines (GRLS) of the Russian Federation.
The test was carried out by the gravimetric method by measuring the specific pullout force (F') using a lever mechanism represented by a modified shoulder scale.
