Изобретение относится к медицине, а именно к челюстно-лицевой хирургии и хирургической стоматологии. Способ заключается в точном позиционировании титановых накостных минипластин (ТНМП) без риска повреждении корней зубов, сосудисто-нервного пучка нижней челюсти с помощью индивидуального хирургического шаблона (ИХШ) при остеосинтезе переломов угла и тела нижней челюсти.The invention relates to medicine, namely to maxillofacial surgery and surgical dentistry. The method consists in precise positioning of titanium extraosseous miniplates (TNMP) without the risk of damage to the roots of the teeth, the neurovascular bundle of the lower jaw using an individual surgical template (ISM) in osteosynthesis of fractures of the angle and body of the lower jaw.

На предоперационном этапе выполнено МСКТ нижней челюсти по предложенному протоколу, проведено назубное шинирование по методу «Тигерштедта». После проведенной иммобилизации смещение отломков не устранено. Пациенту назначена антибактериальная, анальгезируюшая, десенсебилизирующая терапия. Назначена операция в экстренном порядке. Полученные результаты МСКТ были обработаны в графическом редакторе, выполнено виртуальное моделирование оперативного вмешательства в виде репозиции костных фрагментов, позиционирование фиксирующих устройств с учетом характера перелома и индивидуальных анатомических особенностей. Далее смоделирован индивидуальный хирургический шаблон, который в последующем изготовлен с помощью 3D-принтера.Clinical example. Оперативное лечение проводилось согласно стандартному протоколу накостного остеосинтеза с применением предложенной нами оригинальной методикой использования ИХШ: под эндотрахеальным наркозом выполнен разрез левой подчелюстной области 5 см, отступя 2 см ниже края нижней челюсти и параллельно ему, тупо и остро пройдено к нижнему краю тела нижней челюсти. Скелетировано наружная поверхность нижней челюсти в области перелома, проведена репозиция и фиксация костных фрагментов с помощью костного зажима, в ИХШ установлены ТНМП и адаптированы согласно ранее заданному рельефу поверхности. ИХШ через операционную рану подведен и установлен согласно выбранной позиции с последующей фиксацией временным винтом. Через шахты ИХШ с помощью сверла с водяным охлаждением сформированы ложа для фиксирующих элементов. ТНМП фиксируются винтами, удаляется временный фиксирующий винт и ИХШ. Выполнена оценка стабильности остеосинтеза и состояние прикуса, ткани послойно ушиты, дренирование, асептическая повязка.Surgical treatment was carried out according to the standard protocol of bone osteosynthesis using the original technique proposed by us for the use of ISI: under endotracheal anesthesia, a 5 cm incision was made in the left submandibular region, 2 cm below the edge of the lower jaw and parallel to it, bluntly and sharply passed to the lower edge of the body of the lower jaw. The outer surface of the lower jaw in the area of the fracture was skeletonized, the bone fragments were repositioned and fixed with a bone clamp, TNMPs were installed in the ISW and adapted according to the previously specified surface topography. IHSH was brought through the surgical wound and installed according to the selected position, followed by fixation with a temporary screw. Beds for fixing elements are formed through the ISH shafts using a water-cooled drill. TNMP are fixed with screws, the temporary fixing screw and ISW are removed. The evaluation of the stability of osteosynthesis and the state of bite was performed, the tissues were sutured in layers, drainage, aseptic dressing.
Послеоперационный период проходил без осложнений. Швы сняты на седьмые сутки. На десятые сутки сняты назубные шины. На контрольном рентгенологическом обследовании стояние отломков правильное, повреждения зубов и перфорации канала нижней челюсти титановыми винтами не определялось. В отдаленном послеоперационном периоде через три месяца открывание рта в полном объеме, прикус ортогнатический.The postoperative period passed without complications. The sutures were removed on the seventh day. On the tenth day, the dental splints were removed. On the control X-ray examination, the position of the fragments was correct, no damage to the teeth and perforation of the mandibular canal with titanium screws was detected. По данной методике прооперировано 8 пациентов с хорошими функциональными и эстетическими результатами.According to this technique, 8 patients were operated on with good functional and aesthetic results.
Учитывая спланированное положение ТНМП и фиксирующих винтов - исключается риск повреждения сосудисто-нервного пучка и зубов нижней челюсти, сокращается время оперативного вмешательства. Все признаки формулы изобретения являются существенными и позволяют снизить травматичность хирургического вмешательства за счет повышение точности позиционирования ТНМН для фиксации костных фрагментов при переломе угла и тела нижней челюсти, а также снижение возможных ятрогенных осложнений таких как повреждение нижнего альвеолярного нерва и сосудистого пучка нижнечелюстного канала, повреждение корней зубов, тканей периодонта и корневой пульпы.
