В категорию молекулярных маркеров могут быть формально отнесены показатели биохимического анализа (ЛДГ, мочевина, креатинин, общий белок, альбумин, билирубин, ACT, АЛТ, щелочная фосфатаза, калий, натрий), который должен выполнятся всем пациентам с ЛХ согласно федеральным клиническим рекомендациям (2020 г.). Но ни один из этих параметров не обладает диагностической специфичностью по отношению к ЛХ, их анализ позволяет лишь оценить общее состояние пациента перед или в ходе проведения терапии. При этом ДНК-аптамеры распределяются между НЧ и ВНВ, мембрана которых содержит лиганды. Принцип способа показан на Фиг. 1. Чем больше в такой системе везикул, несущих специфические лиганды, тем большее число молекул аптамеров будет связано с ВНВ и меньшее - с НЧ, при этом НЧ начинают проявлять свою фермент-миметическую активность. Отсутствие «стабилизатора» на поверхности золотых НЧ приводит к обратному их агрегированию и изменению спектра поглощения раствора. В зависимости от концентрации детектируемого везикулярного маркера в суспензии ВНВ, раствор НЧ-Au агрегирует с большей или меньшей степенью, что пропорционально оптической плотности частиц. Depending on the concentration of the detectable vesicular marker in the VNV suspension, the NP-Au solution aggregates to a greater or lesser extent, which is proportional to the optical density of the particles. The parameter for assessing the concentration of the vesicular marker in the sample was the ratio of extinction coefficients at 650 nm and 520 nm.

Плазму помещают в пробирку объемом 2 мл. Центрифугируют последовательно 200g 10 мин, 800g 10 мин, 1500g 10 мин. Далее плазма проходит через гидрофильный фильтр 0,22 мкм. Из подготовленной плазмы выделяют экзосомы (CD30(+)BHB с помощью двухфазной полимерной системы [Slyusarenko М., Nikiforova N., Sidina Е., Nazarova I., Egorov V., Garmay Y., Merdalimova A., Yevlampieva N., Gorin D., Malek A. Formation and Evaluation of a Two-Phase Polymer System in Human Plasma as a Method for Extracellular Nanovesicle Isolation // Polymers. Сер. 13. - 2021. - N3.- С. 458. - 17]. Далее в центрифужную пробирку объемом 200 мкл добавляют 10 мкл золотых наночастиц, к ним добавляют 2,5 мкл аптамера (концентрация 100 пмоль/мл), перемешивают аккуратным пипетированием 8-10 раз. Полученную смесь перемешивают в течение 30 мин при +4°С. К смеси добавляют 1 мкл экзосом (образец с концентрацией (2±1)*1011 частиц/мл), перемешивают аккуратным пипетированием 8-10 раз. Образцы венозной крови были получены от 5 здоровых доноров и 5 пациентов с вновь поставленным диагнозом ЛХ были собраны в вакутейнеры с ЭДТА. Все диагнозы были верифицированы морфологическим и ИГХ исследованием биоптатов пораженных лимфоузлов. После отделения плазмы путем центрифугирования (1500g 3 мин), 2 мл плазмы были использованы для анализа, который проведен согласно протоколу, представленному выше. Измерение поглощения света на длине волны 378 нм проведено на планшетном ридере Varioskan Flash. Проведен подсчет средних для групп образцов значений (доноры 2,6 vs. пациенты 7,1) и показателей стандартных отклонений. Результаты представлены на фиг. 5.Venous blood samples were obtained from 5 healthy donors and 5 newly diagnosed HL patients were collected in EDTA vacutainers. Образцы крови были собраны в вакутейнеры с ЭДТА. После отделения плазмы путем центрифугирования (1500g 3 мин), 2 мл плазмы были использованы для анализа, который проведен согласно протоколу, представленному выше. Измерение поглощения света на длине волны 378 нм проведено на планшетном ридере Varioskan Flash. Проведен подсчет средних для групп образцов значений (пациенты до лечения -7,1 vs. пациенты после лечения 2,3), и показателей стандартных отклонений. Заявляемый способ позволяет осуществлять диагностику и мониторинг эффективности терапии лимфомы Ходжкина с помощью новой технологии количественной оценки CD30(+)BHB, основанной на эффекте изменения фермент-миметических свойств нано-частиц золота (НЧ-Au) в результате адсорбции на их поверхности ДНК. Является безопасным и безвредным для пациентов, обладает низкой стоимостью процедуры.The claimed method allows to diagnose and monitor the effectiveness of therapy for Hodgkin's lymphoma using a new technology for quantitative evaluation of CD30(+)BHB, based on the effect of changing the enzyme-mimetic properties of gold nanoparticles (NP-Au) as a result of DNA adsorption on their surface.
