Abstract
Description
The present invention relates to medicine, namely to surgery, ultrasound diagnostics and can be used in the study of pleural cavities in trauma, respiratory failure, acute depression of the patient's consciousness.
89% (Lichtenstein D. Α., Relevance of Lung Ultrasound in the Diagnosis of Acute Respiratory Failure. The BLUE protocol. CHEST. 2008; 134: 117-125) до 99,2% (Соколова Ε.П. Москва, 2016), а специфичность от 94 до 100% (Yousefifard Μ., Baikpour Μ., Ghelichkhani P., Asady Η., Nia K.S., Jafari A.M., et al. Screening Performance Characteristic of Ultrasonography and Radiographyin Detection of Pleural Effusion; a Meta-Analysis. Emergency. 2016; 4(1): 1-10).The determination of free fluid in the pleural cavities is carried out by qualitative and quantitative research. The main goal of a qualitative study is to determine only the presence of free fluid. According to many authors, echosonography of the pleural cavities for a qualitative study has a high diagnostic accuracy. So the sensitivity of the method is from 89% (Lichtenstein D. A., Relevance of Lung Ultrasound in the Diagnosis of Acute Respiratory Failure. The BLUE protocol. CHEST. 2008; 134: 117-125) up to 99.2% (Sokolova E.P. Echography in the diagnosis of intrapulmonary injuries and complications in patients with closed chest trauma: abstract of the thesis ... Candidate of Medical Sciences. Moscow, 2016), and the specificity is from 94 to 100% (Yousefifard M., Baikpour M., Ghelichkhani P., Asady H., Nia KS, Jafari AM, et al. Screening Performance Characteristic of Ultrasonography and Radiography in Detection of Pleural Effusion; a Meta-Analysis. Emergency. 2016; 4 (1): 1-10).
100%.Quantitative ultrasound involves an accurate determination of the volume of free fluid in the pleural cavities. Currently, a large number of formulas and calculations have been proposed to determine the volume of pleural effusion. However, all calculations have a large diagnostic error from 50 to 100%.
Die Real-Time Sonographic in der Diagnostik von Pleuraerguessen. Gebhard et al. Ultraschalldiagnostik’ 89.- Berlin: Springer, 1990.-P. 385-387):Within the framework of the eFAST protocol, which is used in trauma, the formula proposed by Goecke W. et al, 1990 (Goecke W., Schwerk W.B. Die Real-Time Sonographic in der Diagnostik von Pleuraerguessen. Gebhard et al. Ultraschalldiagnostik' 89.- Berlin) is used: Springer, 1990.-P. 385-387):
V (ml) \u003d 70 x (basal peripheral distance between the lower edge of the lung and the diaphragm in cm + maximum height of the effusion in cm).
100%.However, this formula can only be applied if the patient is in an upright position, which is not always feasible in case of trauma and in patients in the intensive care unit who are on mechanical ventilation. Also, its use is not possible in infants. The error in determining the volume is from 80 to 100%.
Также широкую популярность в рамках BLUE - протокола получила формула, предложенная Balik Μ. et al, 2006 год (Balik Μ., Plasil P., Waldauf P., Pazout J., Fric M., Otahal M., et al. Ultrasound estimation of volume of pleural fluid in mechanically ventilated patients. Intensive Care Med. 2006; 32: 318-3216):Also, the formula proposed by Balik Μ has gained wide popularity within the BLUE protocol. et al, 2006 (Balik M., Plasil P., Waldauf P., Pazout J., Fric M., Otahal M., et al. Ultrasound estimation of volume of pleural fluid in mechanically ventilated patients. Intensive Care Med. 2006 ;32:318-3216):
V (ml)=20 x Sep (mm), where Sep is the viscero-parietal separation. The patient is in a horizontal position, the pleural cavities are scanned from the PLAX point (along the posterior axillary line in the region of the 8th-9th intercostal space). The mean volume prediction error using Sep was 149.3±164.4 ml and 158.4±160.6 ml.
Известно определение больших объемов свободной жидкости в плевральных полостях, когда более применима формула, предложенная Telchgraber U.K. et al., 2018 год (Teichgraber U.K, Hackbarth J. Sonographic Bedside Quantification of Pleural Effusion Compared to Computed Tomography Volumetry in ICU Patients. Ultrasound Int Open. 2018; 4: E131-E135):It is known to determine large volumes of free fluid in the pleural cavities, when the formula proposed by Telchgraber U.K. is more applicable. et al., 2018 (Teichgraber U.K, Hackbarth J. Sonographic Bedside Quantification of Pleural Effusion Compared to Computed Tomography Volumetry in ICU Patients. Ultrasound Int Open. 2018; 4: E131-E135):
V (ml)=13,33 x Sep (mm)V (ml)=13.33 x Sep (mm)
The patient is in a horizontal position, the pleural cavities are scanned along the posterior axillary line in the region of the 5th-6th intercostal space. The error in calculating the volume of free liquid with this formula ranges from 50 to 80%. But the effectiveness of its use begins with large volumes, more than 500 ml.
The technical result of the proposed method is to increase the accuracy of the determination, the safety and non-invasiveness of the method.
New in achieving the technical result is that when examining the right pleural cavity, the sensor is installed in the right hypochondrium with the direction of the sensor to the right shoulder, while the scanning beam passes in the oblique frontal plane (with this position of the sensor, the right pleural cavity is visualized in a larger volume, which gives a more informative measurement); scanning of the left pleural cavity is carried out along the left posterior axillary line in the region of the 7-10th intercostal space in the sagittal plane.
What is also new is that the first measurement (Sep) is carried out from the diaphragm to the lung tissue, and the second measurement (L) is carried out at an angle of 90 degrees to Sep regarding the diaphragm, while the caliper is set from the medial to the lateral points of the chest wall tissue, then the area is determined surface of the body (BSA) and calculate the volume of free fluid in the pleural cavity (V) using the formula:
V=Sep x L x BSA x K, гдеV=Sep x L x BSA x K where
V - объем жидкости в мл;V is the volume of liquid in ml;
Sep - висцеро-париетальная сепарация, измеряемая от диафрагмы до ткани легкого;Sep - viscero-parietal separation, measured from the diaphragm to lung tissue;
L - длина от медиальной до латеральной точек ткани грудной клетки;L is the length from the medial to the lateral points of the chest tissue;
BSA - площадь поверхности тела;BSA - body surface area;
8,5.K is a correction factor equal to 8.5.
In all the proposed formulas and calculations for determining the exact volume of free fluid in the pleural cavities, there is no personalized approach. The gender-weight characteristics of patients are not taken into account. So, for example, splitting sheets of the parietal and visceral pleura with a thickness of 1 cm will have different volumes of free fluid in an infant weighing 5 kg and in an adult patient weighing 100 kg.
табл.).The method proposed by the authors for determining the volume of free fluid has an individual approach to the patient. It can be freely applied to both newborns and adults. The authors experimentally established a correction factor, which also makes it possible to eliminate a large diagnostic error. The study was conducted on 30 corpses. A 12 FR thoracic drain was inserted into the pleural cavity in the second intercostal space along the midclavicular line. Physiological saline was injected into the pleural cavity through the catheter. Knowing the exact volume introduced into the pleural cavity, the coefficient K was calculated as follows: K=V(ml): Sep: L: BSA (see table).
The table shows the calculation of the coefficient K for different volumes during the experiment on the right pleural cavity. After all calculations of the K coefficient, its average value was determined, which was 8.43±0.59. For practical application, the coefficient is rounded up to 8.5. The method is carried out as follows.
For ultrasound, convex, vector and linear sensors with a frequency of 2-7.5 MHz are used, depending on the specific situation and the size of the patient's body.
The patient is in a horizontal position, lying on his back. The doctor is on the right.
Initially, the right pleural cavity is examined, while the sensor is placed in the right hypochondrium, the direction of the sensor is towards the right shoulder, the scanning beam passes in the oblique frontal plane. This allows you to more fully explore the right pleural cavity.
If it is impossible to scan in the first position (as a rule, due to increased gas formation in the colon), the sensor is placed along the right posterior axillary line in the region of the 7th-10th intercostal space, the sensor is directed towards the left shoulder, the scanning beam is directed in the sagittal plane.
After scanning the right pleural cavity, the left pleural cavity is examined, for this, the sensor is placed along the left posterior axillary line in the region of the 7th-10th intercostal space, the direction of the sensor is towards the right shoulder, the scanning beam passes in the sagittal plane.
In the proposed positions, the following anatomical structures are visualized:
1. Liver (when examining the right pleural cavity), spleen (when examining the left pleural cavity).
2. Diaphragm + area of the parietal pleura (visualized as a hyperechoic structure, as a "thick white line" in the form of a crescent).
3. Lung tissue (determined as an artifact due to the reflection of ultrasound from air lung tissue, with the obligatory identification of the B-profile: thin white lines directed from the surface of the lung in the form of a “comet tail”).
4. Free fluid is determined primarily in the pleural sinuses (located between the lung tissue and the diaphragm).
The specialist needs to derive the maximum distance for splitting the sheets of the visceral and parietal pleura.
Первое измерение проводят от диафрагмы до ткани легкого - эта позиция обозначается как «Sep» (висцеро-париетальная сепарация).The first measurement is taken from the diaphragm to the lung tissue - this position is referred to as "Sep" (viscero-parietal separation).
Length, длина).The second measurement is carried out strictly at an angle of 90 ° to the first position, the line goes tangent to the diaphragm (not lower, not higher). The label (caliper) is set from the medial to the lateral points of the chest wall tissue. The second position is designated as "L" (from English Length, length).
В расчетах используют формулу Haycock (Haycock G.B., Schwartz G.J., Wisotsky D.H. Geometric method for measuring body surface area: a height-weight formula validated in infants, children and adults. J Pediatr 1978; 93:62-66.) The calculations use the Haycock formula (Haycock G.B., Schwartz G.J., Wisotsky D.H. Geometric method for measuring body surface area: a height-weight formula validated in infants, children and adults. J Pediatr 1978; 93:62-66.) to determine the surface area bodies:
Experimentally, the authors determined the coefficient K equal to 8.5. All measurements are entered into the formula for calculating the volume of free fluid in the pleural cavities:
V (ml)=Sep x L x BSA x KV (ml)=Sep x L x BSA x K
The proposed method is illustrated by the following clinical examples.
1.Clinical example No. 1.
Patient B., 10 years old, was treated at the Irkutsk GIMDKB with a diagnosis of Acute hematogenous osteomyelitis of the left femur, septicopyemic form. Sepsis. Bilateral septic pneumonia.
From the anamnesis: a week before hospitalization, the child had an injury in the left thigh. After the injury, there was pain in the left knee joint, which was of an intense growing character. Upon admission, the child's condition is severe, there is an increase in temperature up to 39°C. After the preoperative preparation, the child underwent surgical treatment: opening of the paraossal phlegmon, osteoperforation of the lower third of the left femur. Antibacterial and infusion therapy was prescribed. A day after the surgical treatment, an ultrasound examination of the pleural cavities was performed, where bilateral hydrothorax was detected:
V справа=1,25 (BSA) x 1,2 (Sep) x 6,5 (L) x 8,5 (K)=82 мл;V right = 1.25 (BSA) x 1.2 (Sep) x 6.5 (L) x 8.5 (K) = 82 ml;
V слева=1,25 x 1,1 x 6,3 x 8,5=74 мл.V left = 1.25 x 1.1 x 6.3 x 8.5 = 74 ml.
Three days later, the volume of free fluid in the pleural cavities increased:
V справа=1,25 x 1,7 x 7,0 x 8,5=130 мл;V right = 1.25 x 1.7 x 7.0 x 8.5 = 130 ml;
V слева=1,25 x 1,3 x 6,0 x 8,5=83 мл.V left = 1.25 x 1.3 x 6.0 x 8.5 = 83 ml.
11,9%.Due to an increase in the volume of free fluid and an increase in respiratory failure, the child underwent MSCT of the chest, which revealed signs of bilateral abscess pneumonia with foci of destruction, more on the right. Bilateral hydrothorax: on the right, the volume is 138.5 ml, on the left, 74 ml. Thus, the error of the method for determining the volume of free fluid for the right pleural cavity was 5.8%, and for the left 11.9%.
5,5%.During the control echosonography of the pleural cavities, an increase in the volume of hydrothorax was noted: on the right up to 170 ml, on the left up to 90 ml. Which was an indication for puncture and drainage of the right pleural cavity, resulting in 180 ml of serous-hemorrhagic contents. Thus, the error of the method was 5.5%.
After drainage, the child continued treatment for the underlying disease.
2.Clinical example No. 2.
Patient M., aged 15, was treated at the Irkutsk State Institute of Clinical Hospital with a diagnosis of community-acquired left-sided lower lobe pneumonia. Medium severity. Left-sided exudative pleurisy.
From the anamnesis: 10 days before hospitalization, she felt chills, body aches, sore throat, cough. Within 3 days, the temperature rises to 39°C, general weakness. In this connection, the local pediatrician was called, antibacterial, symptomatic, local therapy was prescribed. On the background of treatment, the state of health did not improve, an elevated temperature persisted up to 38.7°C, shortness of breath occurred up to 30 respiratory movements per minute, cough intensified. She was referred for hospitalization.
Upon admission, a chest X-ray was performed, signs of lower lobe pneumonia were revealed on the left in S 9-10, hydrothorax on the left? In connection with this, an ultrasound examination of the pleural cavities was performed, on the left, a free fluid was revealed with splitting of the sheets of the visceral and parietal pleura up to 31 mm. The formula for determining the volume of free liquid was used: V=1.6 (BSA) x 3.1 (Sep) x 4.3 (L) x 8.5 (K)=181 ml.
Given the increase in respiratory failure, the presence of free fluid, the child underwent a puncture of the left pleural cavity, and 200 ml of serous exudate was obtained. Thus, the error of the method was 9.5%. Repeated punctures were not required; against the background of ongoing treatment, the child was discharged with recovery.
