Abstract
Description
Изобретение относится к области газоанализа и может быть использовано в медицине для диагностики острого инфаркта миокарда (ОИМ).The invention relates to the field of gas analysis and can be used in medicine for the diagnosis of acute myocardial infarction (AMI).
There is a method for diagnosing myocardial infarction (China Patent No. 108913769, IPC C12Q1/6883, publ. 11/30/2018), which contains the procedure for taking a biological sample of a patient (blood) and analyzing it by real-time PCR to determine the content of a single marker of myocardial infarction (expression HELLS gene). A similar approach is proposed in the method for diagnosing myocardial infarction (China Patent No. 107312852, IPC C12Q1 / 68, publ. 03.11.2017), which contains the procedure for taking a biological sample of a patient (blood) and analyzing it by PCR to determine the content of several markers of myocardial infarction (MicroRNA : miR-1468-5p and miR-892b), which improves diagnostic accuracy.
The disadvantage of this method is the invasiveness of blood sampling and the significant time required to analyze the blood sample.
(Kistenev Y.V., Borisov A.V., Kuzmin, D.A., Penkova O.V., Kostyukova N.Y., Karapuzikov A.A. Exhaled air analysis using wideband wave number tuning range infrared laser photoacoustic spectroscopy //JBO. 2017. Vol. 22, № 1. A known method for the differential diagnosis of lung cancer (Kistenev Y.V., Borisov A.V., Kuzmin, D.A., Penkova O.V., Kostyukova N.Y., Karapuzikov A.A. Exhaled air analysis using wideband wave number tuning range infrared laser photoacoustic spectroscopy //JBO. 2017. Vol. 22, No. 1 . P. 017002), which contains a procedure for taking a patient’s biological sample (expired air and its express analysis using laser optical-acoustic spectroscopy to determine the profile of the absorption spectrum of the exhaled air sample specific for lung cancer. This approach is non-invasive and reduces the time of analysis of biological samples; the disadvantage is the use of an indirect diagnostic feature (the profile of the absorption spectrum of an exhaled air sample), which is complexly associated with a set of possible volatile molecular markers of the target disease, which leads to the binding of the diagnostic feature to the used experimental implementation of the spectral analytical method.
As a prototype, a method for diagnosing myocardial infarction (US Patent No. 20090247890 dated 01.10.2009) is selected, which contains procedures for sampling exhaled air and analyzing the concentration of volatile molecular markers in it. The disadvantage of the invention is the low accuracy, since only one volatile molecular marker of myocardial infarction (pentane) is recorded.
The problem to be solved by the technical solution claimed as an invention is improving the accuracy of diagnosing acute myocardial infarction based on spectral analysis of exhaled air samples, where the concentrations of a set of volatile molecular markers indicated in Table 1 are recorded, and the assessment of the presence of AMI is carried out based on the totality of the concentrations of a set of volatile molecular markers identified by the authors.
The problem is solved by the fact that the claimed method for rapid diagnosis of acute myocardial infarction based on the registration of volatile molecular markers in the exhaled air, includes sampling the patient's exhaled air and its spectral analysis with the measurement of concentrations of a set of volatile molecular markers, including at least nitric oxide (N2O), nitrogen dioxide (NO2), ethylene (C2Hfour), pentane (C5H12), carbon monoxide (CO), carbon dioxide (CO2) and evaluating whether the population of measured concentrations is consistent with the presence or absence of acute myocardial infarction.
The assessment of the compliance of the set of measured concentrations with the presence and absence of acute myocardial infarction is carried out by comparing the measured concentrations with tabular values (in ppb) as a median value with the border of the first and third quartiles.
Correspondence assessment of the probability of the presence or absence of acute myocardial infarction is carried out using a machine learning method trained using the concentration values of volatile molecular markers previously recorded in exhaled air samples from patients with clinically confirmed acute myocardial infarction and healthy volunteers, as well as the support vector machine method trained with using the values of concentrations of volatile molecular markers previously recorded in the samples of exhaled air of patients with clinically confirmed AMI and healthy volunteers.
The probability that the set of measured concentrations corresponds to the presence ( or absence ( acute myocardial infarction (AMI) is evaluated by the formulas:

Sampling of exhaled air can be carried out either in a disposable container or directly into the technical means of spectral analysis.
[9-11].The technical means of spectral analysis can be based on a set of chemical sensors [2-5], gas chromatography methods [6-8], optical spectroscopy methods [9-11].
The assessment of the compliance of the set of measured concentrations with the presence or absence of AMI can be implemented using machine learning algorithms [1], calculating the probability of the joint occurrence of independent events, using the ergodic hypothesis, Minkowski distance.
Examples of the implementation of the claimed invention are presented below:
[11]. We have conducted a study of volatile markers in exhaled air [1]. The target group included 30 patients with primary myocardial infarction, who were recruited at the Research Institute of Cardiology, Tomsk, Russia. The interval between the case of AMI and the time of sampling and analysis of the sample was 15.75 hours. (mean). The control group included 42 healthy volunteers. Exhaled air samples were taken into a disposable container made of a chemically inert material. Measurement of the concentrations of a set of volatile molecular markers, including nitric oxide (N2O), nitrogen dioxide (NO2), ethylene (C2H4), pentane (C5H12), carbon monoxide (CO), carbon dioxide (CO2) in exhaled air samples was carried out using a laser optical -acoustic gas analyzer “LaserBreeze” (manufactured by LLC “Special Technologies”, Novosibirsk) [11].
An example of assessing the compliance of a set of measured concentrations with the presence or absence of AMI using machine learning algorithms
2. When creating a predictive model for the presence of AMI, a support vector machine was used. The collected experimental data on the concentration of the above set of volatile molecular components of the representatives of the target group (presence of AMI) and the control group (absence of AMI) were randomly divided 500 times into training and test samples in the proportion of 60%:40%. The training set was used to train the predictive model, while the test set was used to assess its specificity and sensitivity. These scores were then averaged over 500 realizations. The final values of specificity and sensitivity, depending on the set of volatile molecular markers used, are presented in Table 2.
Table 2. Evaluation of the specificity and sensitivity of diagnosing AMI based on the use of various sets of volatile molecular markers
Below is an example of assessing the compliance of a set of measured concentrations with the presence or absence of AMI based on the calculation of the probability of the joint occurrence of independent events.
Let us construct the distributions of concentrations of volatile markers normalized to the approximation area, presented in Table 1. Let us designate these distributions as and for patients with AMI and healthy individuals, respectively. Here index i is the number of the flying marker. Let's introduce the partial probability of having AMI:
, ,
тогда частная вероятность отсутствия ОИМ будет then the partial probability of the absence of AMI will be
. .
1 показаны распределения вероятностей и для i-го летучего маркера (по оси абсцисс отложена концентрация в ppb) .In FIG. 1 shows the probability distributions and for the i -th volatile marker (the concentration in ppb is plotted along the abscissa) .
Using the Minkowski measure
, ,
С учетом того, что вероятность совместного появления независимых событий равна произведению их вероятностей, найдем вероятность того, что набор измеренных концентраций для исследуемого пациента соответствует наличию () или отсутствию () ОИМ следующим образом:where r, p are parameters that define the properties of the measure. Taking into account the fact that the probability of the joint occurrence of independent events is equal to the product of their probabilities, we find the probability that the set of measured concentrations for the patient under study corresponds to the presence of ( ) or absence ( ) MI as follows:
, ,
. .
е.The selection criterion may be based on the ergodic hypothesis. According to this hypothesis, the contribution to the resulting probability from each of the significant features is equally probable, so the resulting probability that this patient has AMI is equal to the average value of the corresponding probabilities, i.e.
, ,
. .
0.4.An example of the implementation of these approaches is presented in Table 3. It can be seen that the values and for a patient with AMI, more than 0.6, and for less than 0.4.
