Abstract
Description
FIELD OF TECHNOLOGY
The invention relates to medicine and pharmaceuticals, namely to hemostatic agents, and can be used to stop bleeding of varying severity, including arteriovenous.
BACKGROUND OF THE INVENTION
0.13÷0.22.Known local hemostatic agent to stop intense arterio-venous bleeding [RU 2414225], including a mixture of synthetic zeolites type CaA formula mCaO⋅nNa 2 O⋅2SiO 2 ⋅Al 2 O 3 ⋅0.5H 2 O in the amount of 70-80 wt. % and CaX type of the formula mCaO⋅nNa 2 O⋅2.5SiO 2 ⋅Al 2 O 3 ⋅0.5H 2 O in an amount of 20-30 wt. %, the crystal lattices of which contain calcium cations in the amount of 8÷10 wt. %, where the ratio n:m is 0.13÷0.22.
The disadvantage of a hemostatic agent based on zeolites is a high temperature, up to 85°C, when it comes into contact with blood, resulting in a thermal burn of tissues due to the release of heat when the zeolite comes into contact with the aqueous medium of blood plasma due to high values of the heat of hydration.
MgO; m=0,5÷3,0; n=0,01÷0,05; k=0,2÷1,2.Known hemostatic agent according to patent RU 2522206 (prototype), providing a short time of onset of hemostasis and low exothermic effect when interacting with blood. The invention relates to a local hemostatic agent based on synthetic calcium hydrosilicate. The agent additionally contains oxides of aluminum and/or magnesium and corresponds to the formula unit: CaO⋅(SiO 2 ) m ⋅(M) n⋅(H 2 O)k, where M represents Al 2 O 3 and/or MgO; m=0.5÷3.0; n=0.01÷0.05; k=0.2÷1.2.
This hemostatic agent involves an expensive autoclave synthesis of a hydrosilicate, while the porosity is achieved due to the evolution of hydrogen during the interaction of magnesium or aluminum powders with water. In addition, since calcium in this product is present exclusively in the form of an oxide, the hemostatic effect is achieved only by increasing the concentration of natural blood clotting factors in the wound (platelets, etc.) as a result of water sorption from the blood plasma.
DISCLOSURE OF THE INVENTION
The objective to which the invention is directed is the creation of new and effective hemostatic granules with a high sorption capacity, obtained in a simple and inexpensive way.
The technical result achieved in the implementation of the present invention is to increase the sorption capacity while maintaining a low exothermic effect when interacting with it.
The proposed task and the set technical result are achieved by new granules for stopping bleeding of varying severity based on a porous material obtained by hydrothermal synthesis, which are particles from 0.2 to 3 mm, containing calcium chloride up to 15% by weight.
An essential feature of the invention is the improved morphology of the porous structure of the granules, which at the same time contain calcium chloride up to 15% by weight. Together, this provides an increase in the sorption and hemostatic properties of the product both due to the active absorption of the liquid phase of the blood (molecular sieve mechanism), and simultaneously due to the rapid release and interaction of calcium ions present in the composition in the form of salts with blood. The combination of these factors triggers an efficient blood clotting process. In view of the use of the new material as the basis of the granules, there is no increase in the exothermic effect when they interact with blood.
IMPLEMENTATION OF THE INVENTION
According to the present invention, the material is obtained by hydrothermal synthesis. At the first stage, xonotlite is formed by precipitation from a solution of sodium silicate with a solution of calcium chloride according to the following scheme:
nCaCl2+mNa2SiO3+kH2O → nCaO*mSiO2*kH2O+2mNaClnCaCl 2 +mNa 2 SiO 3 +kH 2 O → nCaO*mSiO 2 *kH 2 O+2mNaCl
The reaction starts already at room temperature. The dispersion and specific surface area of the resulting product, the parameters that will further determine the sorption capacity of the product, are controlled by the process temperature. The optimum temperature at which the product with the highest sorption capacity is obtained is 30-35°C.
The precipitate is filtered off, washed with water and dried at 60°C to constant weight. The resulting xonotlite is characterized by the presence of calcium carbonate (up to 30 wt. %), which is formed by interaction with atmospheric carbon dioxide and low mechanical strength.
Next xonotlite is fired at 950°C for 2 hours. In this case, xonotlite transforms into wollastonite, which is characterized by high mechanical strength. Finely dispersed particles are sintered, forming granules with a porous structure. The present calcium carbonate decomposes with the release of carbon dioxide, thereby further increasing the porosity of the resulting product and improving the morphology of its porous structure.
At the synthesis stage, a certain excess of calcium chloride is taken so that free calcium chloride is present in the finished product.
As a result, the final product corresponds to the following formula:
CaO*(SiO2)n*(MCl2)m*(MCO3)k*(H2O)p CaO*(SiO 2 ) n *(MCl 2 ) m *(MCO 3 ) k *(H 2 O) p
где М представляет собой Са и/или Mg (наличие последнего обусловлено примесями хлорида магния в исходном сырье и на свойство получаемого продукта не влияет); n=1,0-2,5; m=0,05-0,2; k=0,05-0,5; р=0,2-2,0.where M represents Ca and/or Mg (the presence of the latter is due to impurities of magnesium chloride in the feedstock and does not affect the property of the resulting product); n=1.0-2.5; m=0.05-0.2; k=0.05-0.5; p=0.2-2.0.
The resulting porous material is crushed, the required fraction is selected with a particle size of 0.2 to 3 mm. Particles smaller than 0.2 mm when using this material as a hemostatic agent can enter the bloodstream and block it, which can be harmful to human health. Particles larger than 3 mm absorb blood at a slower rate, because as the particle size increases, the area-to-volume ratio of the material decreases.
Before packaging, the granules are heated in a dry oven to 200°C to remove water adsorbed from the air and packed in sealed bags at a temperature of 100°C. Packaged granules in bags are subjected to radiation sterilization.
Below is an example of the implementation of the invention and obtaining a hemostatic agent with optimal technical and operational characteristics.
To a 10% sodium silicate solution in a reactor equipped with a stirrer, a 15% calcium chloride solution is gradually added with vigorous stirring. The molar ratio of calcium chloride to sodium silicate is 1:2. The specified excess of calcium chloride is a particular example of the implementation of the invention. The degree of calcium chloride excess may be different, but still allowing for a more complete precipitation of calcium hydrosilicate, thereby increasing the yield of the reaction product. The reaction scales well and can be carried out under conditions where the total volume of reactants is from 1 to 200 liters without changing the technical characteristics of the resulting product.
The process is carried out at a temperature of 15-25°C with vigorous stirring. The slower the rate of addition of the calcium chloride solution and the more intensively the mixing, the more uniform in composition the precipitated calcium hydrosilicate is obtained.
After adding the entire volume of the calcium chloride solution, stirring is continued for another 30 minutes, the resulting precipitate is filtered off and washed on the filter with water. The amount of water for washing should be no more than 40% in relation to the total volume of reagents. This amount of water allows you to save the required amount of calcium chloride in the final product.
The sludge is unloaded onto stainless steel pallets and dried at 60°C to constant weight. Under these conditions, up to 30% of calcium carbonate is formed in the product, which, upon further annealing, decomposes with the release of carbon dioxide, thereby further increasing the porosity of the resulting product. Higher temperatures shorten the drying time and reduce the amount of calcium carbonate formed.
The dried product is fired at 950°C for 2 hours, after which it is ground and sieved. The output is a porous product corresponding to the formula CaO*(SiO 2 ) 1.5 *(CaCl 2 ) 0.1 *(CaCO 3 ) 0.1 *(H 2 O) 0.5 with the following technical characteristics:
Сорбционная емкость по воде - не менее 1 г/г;Sorption capacity for water - not less than 1 g/g;
рН водной вытяжки - от 7,0 до 9,5;pH of the aqueous extract - from 7.0 to 9.5;
