Прогрессирующие увеличение числа лиц пожилого и старческого возраста, страдающих остеопорозом, неуклонно приводит к росту переломов проксимального отдела бедра в структуре общего травматизма. Среди данного вида травмы особое медицинское и социальное значение имеет перелом шейки бедренной кости, приводящий к стойкой потери не только трудоспособности, но и навыков самообслуживания на долгий период. Кроме того, высокий процент летальности пострадавших в первый год после травмы (от 7,6 до 9% и от 27,3% до 45 % по мнению разных авторов), преимущественно пожилой и старческий возраст пациентов с уже имеющимися фоновыми патологиями, ставят актуальной задачей дальнейший поиск эффективных способов восстановления целостности проксисмального отдела бедра [Белинов Н.В. и соавт. На современном этапе развития медицины приоритетным методом лечения переломов шейки бедра является оперативный. В настоящее время существуют различные методики и конструкции для лечения переломов шейки бедренной кости: остеосинтез гвоздями (Knowles, Deyerle, Смит- Петерсена, Thornton, Hansson, DHS, канюлированные винты АО и др.), конструкции на основе аппарата Илизарова, винты Tronzo (1988). Многообразие металлофиксаторов и техник операции ставит перед травматологом-ортопедом сложную задачу выбора, которая, чаще всего, ограничивается материально-технической базой стационара больницы и навыками самого специалиста. Кроме того, применяемый метод лечения должен обладать малой травматичностью по отношению не только к мягким тканям, но и к костному веществу. Большинство же металлоконструкций являются достаточно массивными, и, проходя через проксимальный и дистальный фрагмент перелома, в значительной степени нарушают целостность костных трабекул и кровеносных сосудов, увеличивая риск развития послеоперационных осложнений и неэффективной консолидации [Слободской А.Б. и соавт. Currently, there are various techniques and designs for the treatment of fractures of the femoral neck: osteosynthesis with nails (Knowles, Deyerle, Smith-Petersen, Thornton, Hansson, DHS, cannulated AO screws, etc.), designs based on the Ilizarov apparatus, Tronzo screws (1988 ). The variety of metal fixators and surgical techniques poses a difficult choice task for the traumatologist-orthopedist, which, most often, is limited by the material and technical base of the hospital hospital and the skills of the specialist himself. In addition, the applied method of treatment should have low trauma in relation to not only soft tissues, but also to bone substance. Most of the metal structures are quite massive, and, passing through the proximal and distal fragment of the fracture, significantly violate the integrity of the bone trabeculae and blood vessels, increasing the risk of postoperative complications and ineffective consolidation [Slobodskoy A.B. et al. Существуют менее инвазивные способы оперативного лечения перелома шейки бедра с помощью применения спиц. Остеосинтез пучком тонких спиц широко освещен в отечественной литературе: В.А. Гончаренко и М.Г. Лейкин (1981), Э.Я. Жейдурс и В.В. Зирдзинын (1983), Е.В. Зверев и В.Г. Евстратов (1989), А.Ф. Лазарев (1999). Предложено различное количество спиц с разным диаметром а также методики их расположения.There are less invasive ways to treat a hip fracture using pins. Устранить данный недостаток можно при чрескожном введении спиц. А.Ф. Лазарев (1999) предложил выполнять остеосинтез пучком V-образно изогнутых спиц. Наружные концы спиц он рекомендовал оставлять под кожей, над кожей или под фасцией. Однако данный способ также имеет недостатки: концы спиц служат «входными воротами» для инфекции и в период активизации приводит к усилению болевого синдрома в связи с травматизацией концом спиц мягких тканей.This disadvantage can be eliminated by percutaneous introduction of the needles. Введение спиц по предложенному нами способу осуществляется следующим образом (Фигуры 1 и 2). В условиях операционной осуществляют укладку больного на ортопедическом операционном столе. Затем с соблюдением правил асептики-антисептики проводят обработку операционного поля и отграничивают его стерильным бельём. Спицы (1) вводят в шейку бедренной кости (2) между большим (3) и малым вертелом (4) под углом 110-160 градусов под рентгенологическим контролем через линию перелома (5), при этом апикальный конец спицы не должен доходить до края головки бедра на 2-3 см. Затем спицы надкусывают над кожей, вводят еще на 2-3 см. (6) и обламывают у кости путем качательных движений дистального отдела спиц. Отломанный дистальный конец спицы удаляют. Введение спиц может осуществляться без разрезов под местной анестезией через кожу подвертельной области бедра, наложение швов не требуется. На послеоперационные раны накладывают асептические повязки.The introduction of the needles according to our proposed method is carried out as follows (Figures 1 and 2). Then the needles are bitten over the skin, inserted another 2-3 cm. (6) and break off at the bone by rocking movements of the distal part of the needles. The broken off distal end of the wire is removed. The insertion of the wires can be carried out without incisions under local anesthesia through the skin of the subtrochanteric region of the thigh; suturing is not required. Aseptic dressings are applied to postoperative wounds.

Новизна предлагаемого метода заключается в перкутанном способе введения спиц в шейку бедра, при котором спицы проводят чрезкожно и обламывают на границе кортикального слоя, что приводит к минимальной травматичности мягких тканей и костного вещества, с возможностью проведения остеосинтеза только на фоне местной анестезии.The novelty of the proposed method lies in the percutaneous method of introducing the pins into the femoral neck, in which the pins are passed percutaneously and break off at the border of the cortical layer, which leads to minimal trauma to soft tissues and bone matter, with the possibility of performing osteosynthesis only against the background of local anesthesia.
