Abstract
Description
The invention relates to medicine, namely to dentistry and can be used in dental implantology
Currently, in dental implantology, an urgent problem is the early rehabilitation of patients with partial and complete loss of teeth. Successful reparative regeneration is the main factor affecting the mechanical integrity of the implant-bone junction and shortening the treatment time. In 1965 first coined the term "osseointegration" to describe the successful outcome of bone integration into an implant. Clinically, the osseointegration process reflects the mechanical anchoring of the dental implant in the jawbone, which is maintained under all normal conditions of oral function.
Dental implants are generally cylindrical or tapered (root) threaded design. The design parameters primarily affect the distribution of the load in the bone tissue, which leads to the correct fixation and functioning of the implant. Implant geometry can significantly improve the stability and biomechanical fixation of the implant after the healing process. Transformation of the surface of dental implants to increase the adhesion area of the implant to the bone, that is, an increase in the degree of roughness is also of great importance in the process of osteintegration. But roughness is far from the only parameter of the surface of dental implants that affects its success.
A large number of studies have been carried out on the adhesion of protein substances to the surface of dental implants with a biocoating in the form of calcium phosphate materials. And although the osteoconductive properties of bioactive coatings certainly have advantages. However, these coatings are not devoid of such a disadvantage as the peeling of the coating from the titanium substrate, which is associated with the presence of insufficiently strong bonds between the various phases of calcium phosphates on the implant surface. Therefore, despite the existing many methods for changing the effectiveness of the implant by improving the response of bones physically, chemically or therapeutically, the problem of improving osseointegration remains relevant.
One of the promising directions is the development of a technique for treating implants with cold plasma, which, by increasing the hydrophilicity, will stimulate the growth of osteoblasts into the surface and macro-threads of the implant, which will improve the adhesion between the implant and tissues, reducing the risk of bacteria getting on the surface of the implant.
Known methods of processing implants with cold plasma.

1. [Low pressure plasma surface treatment device for dental implant, [KR 102036169 (B1) - 2019-10-24]. The method consists in treating the implant with cold plasma with the participation of argon and oxygen. The device consists of several cameras and blocks. In the device, the control unit controls pulses, which must be maintained at a frequency of 50 kHz ± 10%, the output power is at least 50 W, and the internal temperature of the chamber 20 is below 60 ° C.
The disadvantage of this method is the use of several gases: argon and oxygen, the need for separate chambers for storing and mixing gases, device elements for adjusting the gas flow rate.

Method of processing dental implant surface using ultraviolet light with plasma and surface processing device for dental implant performing the same [KR 20160065698 (A) - 2016-06-09].
The present invention relates to a method for treating an implant surface using optical plasma. The method includes: placing a titanium-containing implant in an optical plasma processing device; then injecting gaseous oxygen into the optical plasma processing device; and then irradiated with ultraviolet light. Next, the surface of the implant is treated with oxygen plasma modified with gaseous oxygen.
The disadvantage of this method is the treatment of the implant in several stages, the need to irradiate the implant with ultraviolet light, to modify the plasma with gaseous oxygen.
3. Container, apparatus and method for handling an implant. [WO 2015087326 (A1) - 2015-06-18]. 3. Container, apparatus and method for handling an implant. [WO 2015087326 (A1) - 2015-06-18]. In this method, the plasma treatment chamber is sealed and can be made of different materials. The chamber contains liquid in the form of a salt composition or in the form of a composition containing additives for treating the surface of wounds, healing additives.
The disadvantage of this analogue can be considered the need to use a liquid of a certain composition, with additives dissolved in it, in the plasma treatment of the implant.
The objective of the present invention is to create an effective and simple method for increasing the hydrophilicity of dental implants by treating the implants with cold plasma with a minimum but sufficient processing time.
The advantage of the proposed method in comparison with analogs is that the plasma chamber is a simple, single-chamber design, filled with atmospheric air saturated with water vapor. This achieves an increase in the hydrophilicity of the treated surface of the implants in an optimally short time.
The problem is solved in that the pre-milled, sandblasted and etched with hydrochloric acid implant is placed in a gas-discharge chamber connected to a high-voltage pulse generator. Water is added to the chamber in an amount of 20 μl. Using a foreline pump, air is evacuated from the chamber until a pressure of 100 Pa is reached. The stainless steel sealing plate is grounded and the brass plate is pulsed with a voltage of 40 kV: frequency 100 Hz, pulse duration 100 ns. After plasma treatment for a duration of 0 to 240 sec. The implant is removed, a drop of water with a volume of 2 μL is placed on its surface, and photographs are taken so that the optical axis coincides with the surface plane with the drop. The obtained photograph was used to measure the contact angle of wetting.
A comparison of the cold plasma-coated implant with the original sample was made according to the ISIS implant surface examination standard. The dependence of the edge incident wetting angle on the time of sample treatment in cold plasma is shown. According to the test results, the optimal treatment time for the implant with cold plasma was determined.
Implementation of the invention.
Titanium implants Dentium Super-Line root-shaped with double threads were used.
To obtain cold plasma, a high-voltage pulse generator was assembled according to the Blumlein scheme (Fig. 1) with a gas-discharge chamber connected to it (Fig. 2). A voltage of 40 kV with a frequency of 100 Hz is pulsed to the gas-discharge chamber (K), as a result of which the plasma is ignited, the capacitance of the capacitors C1 = C2 = 47 pF, the voltage V = 20 kV.
The gas discharge chamber is a cylindrical glass container with a diameter of 170 mm, a height of 40 mm and a wall thickness of 3 mm, hermetically sealed through a rubber sealing ring with a height of 3 mm by a stainless steel plate. A tin-alloyed brass sheet is glued to the outside of the bottom of the container with epoxy glue, to which 40 kV is pulsed at a frequency of 100 Hz. The second electrode of the chamber is a stainless steel plate. There is a through hole in the plate and a hose is attached for pumping air from the chamber using a foreline pump to a pressure of ~ 100 Pa. The pulse was generated by a computer and fed to the thyratron ignition unit powered by a Spellman PCM20P120 / 10000 high-voltage source. After that, a 20 kV pulse was applied to the capacitor system to increase to 40 kV.
4).Prior to the plasma treatment, the implants were milled using a Dentsply Sirona inLab MC XL apparatus, sandblasting with aluminum oxide (grain size 50 μm) and etching with 14% hydrochloric acid (
5).Then the treated sample was taken out of the chamber and compared with the untreated sample by applying water droplets. Drops on the sample treated with plasma spread over the surface and down the thread (Fig. 5).
To determine the hydrophilicity of the surface of the samples, as the main indicator of an increase in osseointegration, the flowing contact angle was used. The equilibrium contact angle was not calculated, since it was not possible to measure the outgoing contact angle, since it was immeasurably small on the rough surface under study.
6).The contact angle was measured by the lying drop method. A drop of 2 μL was applied to the flat surface of the sample to be measured. Since such a drop is small enough and its weight can be neglected in comparison with the surface tension forces, the surface of this drop can be considered spherical, that is, a drop on the surface is a spherical segment (Fig. 6).
It is determined that:
The angle α between the plane and the surface of the droplet at the cross-section is the contact angle of wetting. Therefore, it is equal to:
The volume of the spherical segment is:
где h - высота шарового сегментаwhere h is the height of the spherical segment
R - радиус сферыR - radius of the sphere
2r - диаметр капли (при 0<А<90)2r - droplet diameter (at 0 <A <90)
We rewrite the equation as:
We solve the equation for h by the method of Omar Khayyama, we get:
Substituting into equation (2) we get:
1.Therefore, to measure the contact angle, it is necessary to apply a drop of volume V to the surface and measure the drop diameter and its height. Seven series of measurements of the contact angle were carried out, depending on the time of cold plasma treatment. 42 samples of dental implants were processed using a cold plasma generator. The mean values of the experimental data of the studies are presented in FIG. 7 and Table 1.
Using the ISIS technique and the edge fit of a drop of water, a comparison was made between a sample treated with cold plasma and untreated (standard) implants from other manufacturers (Table 2).
The technical result of the proposed method is that cold plasma treatment of dental implants according to the proposed method maximizes the hydrophilicity of the implant surface, which speeds up its engraftment, reduces the rehabilitation period and reduces the percentage of complications after dental implantation.
