Abstract
Description
The invention relates to medicine, namely to pharmacogenetics, clinical pharmacology, psychiatry, narcology, and can be used to select the dose of the tetracyclic antidepressant mirtazapine in patients with depressive disorders.
Mirtazapine is used for the treatment of disorders accompanied by the development of depressive symptoms, including included in the standards and protocols for the treatment of patients with alcohol dependence, who are in a state of actualization of pathological attraction. The use of mirtazapine is associated with a high risk of developing adverse drug reactions (headache, akathisia, etc.), which limits its use in the clinic. Currently, there is a fairly large amount of data on the influence of gene polymorphisms encoding elements of biotransformation of mirtazapine on the indicators of its effectiveness and safety.
The objective of the invention is to develop a method that allows predicting and preventing the development of undesirable drug reactions of therapy with mirtazapine, or, on the contrary, the absence of the expected effectiveness of therapy, by adjusting the dose of the drug (or recommendations for changing it), depending on the presence of certain polymorphisms in the genes encoding isozymes of biotransformation involved in the metabolism of mirtazapine.
CYP2D6 (HicksJK, BishopJR, SangkuhlK, etal. Clinical Pharmacogenetics Implementation Consortium (CPIC) Guideline for CYP2D6 and CYP2C19 Genotypes and Dosing of Selective Serotonin Reuptake Inhibitors. ClinPharmacolTher. The essence of the invention consists in calculating the value of the parameter P, which is an estimated value of the severity of changes in the rate of biotransformation of mirtazapine, depending on the state of genetic polymorphism in each individual patient. After genotyping for the points included in the panel developed by us, based on the currently available studies of evidence level 3 higher and which are publicly available in the Medline system, the value of the parameter P is calculated.The calculation uses the following principle: if the gene encodes the main isozyme biotransformation of mirtazapine CYP2D6 (HicksJK, BishopJR, SangkuhlK, etal. Clinical Pharmacogenetics Implementation Consortium (CPIC) Guideline for CYP2D6 and CYP2C19 Genotypes and Dosing of Selective Serotonin Reuptake Inhibitors. Clin. doher4. cpt.l47 ..), then in a patient who is heterozygote for the functional polymorphism of this gene, the deviation in the rate of biotransformation will be taken at 25%, and in a mutant homozygote, 50%. If there are genotyping data for several polymorphisms of the same gene, then the obtained values of the change in the rate of biotransformation are added to each other. Slowdown of biotransformation has a “-” sign, acceleration has a “+” sign. If there are several isozymes of the main or alternative pathway, then the variation in rate for each of the pathway isozymes is divided by the number of isozymes responsible for the metabolic pathway. These percentages in biotransformation rate bias were selected based on the meta-analyzes available to date from reputable consortia for other drugs (eg antipsychotics).
If the parameter P is in the interval [50; ∞) (deviation of the biotransformation rate of mirtazapinane is 50% and higher), a recommendation is given to replace the drug with one in which metabolism does not involve isozymes whose activity is impaired. When the value of the parameter P is in the range (-50; 0) (slowing down the rate of biotransformation of mirtazapine by less than 50%), a recommendation is made to reduce the dose.
If the parameter P is in the range (0; 50) (acceleration of the biotransformation rate of mirtazapine is less than 50%), a recommendation is given to increase the dose.
The use of the invention makes it possible to obtain the following technical result.
The proposed method for optimizing the dosage regimen of mirtazapine, based on the analysis of the results of genotyping for polymorphic markers capable of influencing the change in the activity of genes encoding isozymes of biotransformation of mirtazapine, has high accuracy, information content and objectivity, since it is based on the method of reliable assessment of the genetic apparatus and algorithms, included in meta-analyzes of reputable consortia when calculating similar indicators for other drugs.
The proposed method is simple and affordable and can be used in medical institutions of various types without the use of special equipment, with the exception of equipment for genotyping.
The technical result is achieved due to the fact that the authors for the first time determined the severity of the deviation in the rate of biotransformation of mirtazapine depending on the state of the genetic apparatus, established a reliable relationship between the severity of this deviation and the degree of change in the individual response to mirtazapine, and also developed recommendations for optimizing the dosage regimen of mirtazapine in depending on the severity of this deviation.
The severity of the deviation in the rate of biotransformation of mirtazapine, depending on the state of the genetic apparatus, was determined on the basis of an analysis of literature data, as well as statistical and mathematical analysis of a large amount of clinical material.
The method is carried out as follows. After receiving the results of genotyping for any polymorphic markers of the CYP2D6 genes, the data are entered into an Excel table. Next, the software developed by us is launched and the generated Excel file is loaded into it and the program automatically calculates the required indicators using the following formulas:
где:Where:
Wg - количество полиморфизмов гена CYP2D6 (основного пути биотрансформации миртазапина), данные которых были занесены в таблицу Excel;W g - the number of polymorphisms of the CYP2D6 gene (the main pathway of biotransformation of mirtazapine), the data of which were entered in the Excel table;
CYP2D6.A i - change in the rate of biotransformation of mirtazapine, depending on the genotype for each polymorphic marker of the CYP2D6 gene entered in the Excel spreadsheet.
The value of the parameter "Resultant change in the rate of biotransformation of mirtazapine" is judged on the risks of developing adverse drug reactions and the lack of therapeutic efficacy of therapy with mirtazapine.
If the parameter P is in the interval [50; ∞) (deviation of the biotransformation rate of mirtazapine by 50% and more), a recommendation is given to replace the drug with one in which metabolism does not involve isozymes whose activity is impaired.
The proposed method was tested on 13 patients with a confirmed diagnosis of “Mental and behavioral disorders caused by alcohol consumption. Middle stage addiction syndrome, uncomplicated. Abstinence in conditions excluding the use of psychoactive substances (F 10.212) "according to the criteria of ICD-10. The patients were inpatient treatment at the State Budgetary Healthcare Institution "MNPC of Narcology" DZM in 2019. The patients' age ranged from 18 to 55 years; the average age was 34.17 ± 7.25 years. All patients were male.
1.To stop pathological cravings, patients were prescribed mirtazapine in the form of tablets on the 5-10th day of hospitalization after stopping withdrawal symptoms during the period of exacerbation of pathological cravings. The selection of a therapeutic dose of mirtazapine was carried out on the basis of an assessment of the severity of the clinical picture of exacerbation of pathological craving in accordance with the standards of drug addiction medical care. The daily dosage remained unchanged throughout the period of therapy, of which mirtazapine was a component. The daily doses of mirtazapine are presented in Table 1.
Based on the results of CYP2D6 genotyping for the polymorphic marker 1846G> A in 13 patients with alcohol dependence, the following data were obtained:
Количество пациентов, являющихся носителем не мутантного гена CYP2D6 (генотип GG) составило - 9 (69%);The number of patients carrying a non-mutant CYP2D6 gene (genotype GG) was 9 (69%);
Количество пациентов, являющихся гетерозиготными носителями полиморфизма 1846G>Агена CYP2D6 (генотип GA) составило - 4 (31%).The number of patients who are heterozygous carriers of the 1846G> Agen CYP2D6 polymorphism (GA genotype) was 4 (31%).
No patients with mutant AA genotype were found.
During the study (16 days of therapy with mirtazapine), the development of dose-dependent adverse drug reactions (headache, akathisia) was recorded in 6 patients, and the distribution in terms of the frequency of occurrence of adverse reactions was as follows:
- в группе пациентов с генотипом GG- 2 НЛР типа А (22,22%);- in the group of patients with the GG-2 genotype of type A ADR (22.22%);
- в группе пациентов с генотипом GA- 4 НЛР типа А (100%);- in the group of patients with the GA-4 genotype ADR type A (100%);
Статистический анализ, проведенный в программе STATISTICA 10.0 (StatSoft, USA), с помощью двустороннего критерия Фишера показал наличие статистически значимой разницы в частоте встречаемости дозозависимых НЛР у пациентов с разными генотипами (р=0,02098).Statistical analysis performed in the STATISTICA 10.0 program (StatSoft, USA) using a two-sided Fisher test showed a statistically significant difference in the incidence of dose-dependent ADR in patients with different genotypes (p = 0.02098).
Calculation of the P index for patients with the GA genotype showed that the rate of biotransformation of mirtazapine in these patients was slowed down by 25%, and recommendations were made to reduce the dose of the drug.
In our study, the sensitivity of the technique was 66.7%, and the specificity was 100%. The low sensitivity index is due to the inclusion of only one polymorphism in the study. Probably, in patients with the GG genotype for the polymorphic marker 1846g> a, who developed type A ADR, there were other polymorphisms of the CYP2D6 gene, which led to the accumulation of the drug in the plasma, which led to the development of ADR. The inclusion of more polymorphisms in the genotyping panel will increase the sensitivity of the technique.
