Лабораторное исследование эякулята является неотъемлемой частью обследования мужчины для определения его фертильности. Заключение о реальной репродуктивной способности мужчины можно дать лишь по факту наступления беременности. Анализ эякулята не дает однозначного ответа о мужской фертильности, но является важным индикатором его вероятного фертильного потенциала [1]Laboratory examination of the ejaculate is an integral part of the examination of a man to determine his fertility. Основным методом лабораторной диагностики мужского бесплодия является проведение исследования эякулята согласно руководства ВОЗ (5 издание, 2010 г.) Это достигается тем, что интерпретацию результатов проводят следующим образом: активность креатинфосфокиназы более 362,3 Е/л соответствует нормозооспермии; от 167,0 до 362,3 Е/л включительно соответствует олигозооспермии; от 109,4 до 167,0 Е/л включительно соответствует криптозооспермии; активность КФК ниже 109,4 Е/л включительно соответствует азооспермии по данным спермограммы.This is achieved by the fact that the interpretation of the results is carried out as follows: the activity of creatine phosphokinase more than 362.3 U / l corresponds to normozoospermia; from 167.0 to 362.3 U / l inclusively corresponds to oligozoospermia; from 109.4 to 167.0 U / L inclusively corresponds to cryptozoospermia; CPK activity below 109.4 U / l inclusive corresponds to azoospermia according to spermogram data.

Критериальные значения активности КФК в спермальной плазме определены нами при обследовании 123 пациентов; на первом этапе проводили исследование эякулята согласно руководству ВОЗ (5 издание, 2010 г.), в дальнейшем, согласно данным спермограммы исследуемые были разделены на 4 группы: пациенты с нормозооспермией (n=61), с азооспермией (n=48), с криптозооспермией (n=9), с азооспермией (n=5). Всем включенным в исследование было проведено биохимическое исследование спермальной плазмы согласно предлагаемому способу. Оказалось, что имеются достоверные различия в активности КФК в спермальной плазме у пациентов при различном состоянии эякулята. В таблице 1 представлена активность КФК в спермальной плазме в зависимости от состояния эякулята.The criterion values of CPK activity in sperm plasma were determined by us during examination of 123 patients; at the first stage, the ejaculate was examined according to the WHO guidelines (5th edition, 2010); later, according to the spermogram data, the subjects were divided into 4 groups: patients with normozoospermia (n = 61), with azoospermia (n = 48), with cryptozoospermia (n = 9), with azoospermia (n = 5). На фигуре 1 представленны данные ROC-анализа, где А - ROC-кривая, характеризующая зависимость активности КФК в спермальной плазме при нормозооспермии и олигозооспермии (AUC-0,849; р<0,001; пороговое значение: >362,3 Ед/л); В - ROC-кривая, характеризующая зависимость активности КФК в спермальной плазме при олигозооспермии и крипто-/азооспермии (AUC-0,746; р<0,001; пороговое значение: >167,0 Ед/л); С - ROC-кривая, характеризующая зависимость активности КФК в спермальной плазме при криптозооспермии и азооспермии (AUC-0,857; р<0,05; пороговое значение: ≤109,4 Ед/л). The performed ROC analysis demonstrated a high predictive ability to determine the activity of CPK in sperm plasma to determine the quality of the ejaculate; allowed to establish the optimal threshold value of this indicator and acceptable levels of sensitivity and specificity for the selected threshold values, indicated in the method for determining the quality of ejaculate in men by the activity of creatine phosphokinase in sperm plasma.
Сущность предложенного метода заключается в следующем: образцы эякулята, полученные путем мастурбации и семяизвержения, собирают в чистый контейнер из пластика; время доставки образцов в лабораторию не превышает 30 минут, во время транспортировки контейнеры с образцами держат при температуре от 20°С до 37°С. Образцы помещают в инкубатор (37°С) для разжижения. После полного разжижения образцы спермы центрифугируют в течение 10 мин при 1000 g. Затем спермальную плазму переносят в чистую пробирку и проводят определение активности КФК на автоматическом биохимическом анализаторе кинетическим методом с последующей оценкой качества эякулята: активность КФК более 362,3 Е/л соответствует нормозооспермии по данным спермограммы; от 167,0 до 362,3 Е/л включительно соответствует олигозооспермии; от 109,4 до 167,0 Е/л включительно соответствует криптозооспермии; активность КФК ниже 109,4 Е/л включительно соответствует азооспермии.The essence of the proposed method is as follows: samples of ejaculate obtained by masturbation and ejaculation are collected in a clean plastic container; the time of delivery of samples to the laboratory does not exceed 30 minutes; during transportation, containers with samples are kept at a temperature of 20 ° C to 37 ° C. 1. Пациент А., 25 лет. Имеет 1 ребенка. Обратился в рамках прегравидарной подготовки. Предлагаемым способом определена активность КФК в спермальной плазме 1349,3 Е/л, что соответствует нормозооспермии (более 362,3 Е/л). По результатам спермограммы установлена нормозооспермия (количество сперматозоидов - 249,9 млн; концентрация сперматозоидов - 119 млн/мл), что подтверждает полученные с помощью предлагаемого способа данные.1. По результатам спермограммы установлена олигозооспермия (количество сперматозоидов - 7 млн; концентрация сперматозоидов - 1,4 млн/мл), что подтверждает полученные с помощью предлагаемого способа данные.2. По результатам спермограммы установлена криптозооспермия (количество сперматозоидов - 0,96 млн; концентрация сперматозоидов - 0,6 млн/мл), что подтверждает полученные с помощью предлагаемого способа данные.3. По результатам спермограммы установлена азооспермия (количество сперматозоидов - 0; концентрация сперматозоидов - 0), что подтверждает полученные с помощью предлагаемого способа данные.4.
