Abstract
Description
The invention relates to medicine, namely to neurosurgery in the surgical treatment of patients with tumors of the base of the skull, its anterior section with invasive growth and extracranial spread.
A known method of surgical treatment of tumors of the anterior section of the skull base, including a combined transfacial transcranial approach and tumor removal ("Craniofacial block resections in malignant tumors of the skull base." // Problems of neurosurgery 2004; 2; Cherekaev V.A., Reshetov I.V., Belov A.I.), in which, after resection of the tumor of the anterior part of the skull base in a single block, the plasty of the postoperative defect is performed with a periosteal flap on the vessels.
However, despite the possibility of total removal of the tumor, its disadvantage is the formation of a bone defect in the anterior cranial fossa and subsequent prolapse of the brain substance with membranes into the nasal cavity.
A known method of reconstruction of extensive defects in the bottom of the anterior cranial fossa (RF patent No. 2570619, publ. 12/10/2015, Bul. 34), in which the replacement of the defect is carried out with a pre-made individual endoprosthesis of porous titanium nickelide, placed on the edges of the defect from the side of the cranial cavity, when In this case, the defect of the bottom of the anterior cranial fossa is preliminarily covered with a pericranial periosteal flap on the supra-block and supraorbital arteries, fixing it to the edges of the skull base defect with an adhesive composition, the specified endoprosthesis is installed over the pericranial flap, after which the defect of the dura mater (TMO) is sutured using the wide fascia of the femur.
The disadvantage of this method is the use of an artificial endoprosthesis made of porous titanium nickelide. In a large number of cases, the use of artificial endoprostheses leads to their rejection and wound suppuration, in connection with which meningitis or an abscess of the brain may develop.
(Atsushi Hatano, Masato Nakajima. Craniofacial resection for malignant nasal and paranasal sinus tumors assisted with the endoscope. There is also known a method of surgical treatment of tumors of the anterior skull base with plastic surgery of a bone defect in the anterior cranial fossa using split autobone (Atsushi Hatano, Masato Nakajima. Craniofacial resection for malignant nasal and paranasal sinus tumors assisted with the endoscope. Auris Nasus Larynx 36 (2009 ) 42-45.), Taken as a prototype. In the known method, after removal of the en-block tumor, a split bone flap is taken from the convexital surface of the skull, the split bone flap is placed on the defect of the base of the anterior cranial fossa and fixed using miniplates and screws. Next, plastic surgery of the dura mater is performed with a previously prepared periosteal flap on the vessels.
The disadvantages of the prototype are:
- во-первых, фиксация расщепленного костного лоскута к основанию черепа с помощью минивинтов технически сложна и небезопасна из-за малой толщины кости над орбитами, в результате чего минивинты не выполняют функции фиксации к основанию черепа и выступают в полость орбит, что создает опасность инфицировния и травмирования структур орбиты (мышца поднимающая верхнее веко и т/д);- firstly, fixation of a split bone flap to the base of the skull using miniscrews is technically difficult and unsafe due to the small thickness of the bone above the orbits, as a result of which the miniscrews do not perform the function of fixation to the base of the skull and protrude into the cavity of the orbits, which creates a risk of infection and trauma to the structures of the orbit (muscle lifting the upper eyelid, etc.);
- во-вторых, расщепленный костный лоскут уложенный на дефект прикрывается надкостницей с внутренней стороны, таким образом со стороны полости носа (послерезекционной) находится неприкрытая кровоснабжаемыми тканями кость, что ведет к инфицированию костного лоскута, с последующим инфицированием полости черепа.- secondly, the split bone flap placed on the defect is covered by the periosteum from the inside, thus from the side of the nasal cavity (postresection) there is bone exposed by blood supply to the tissues, which leads to infection of the bone flap, with subsequent infection of the cranial cavity.
The invention is aimed at creating a method for reconstructing the base of the anterior cranial fossa when performing block resection of a common tumor of the anterior skull base, which eliminates the indicated disadvantages and increases its efficiency.
The technical result of the proposed method is to prevent the appearance of liquorrhea and prolapse of the brain substance with membranes into a post-resection defect, which in turn reduces the number of complications associated with liquorrhea and intracranial infectious complications.
The specified technical result in the implementation of the invention is achieved by the fact that in the method of reconstruction of the base of the anterior cranial fossa when performing block resection of a common tumor of the anterior skull base, a split bone flap is formed from the convexital surface of the skull, corresponding in size to the defect of the anterior cranial fossa (PCF), according to grooves are made on the sides of the bone flap, an artificial dura mater is placed subdurally into the defect of the dura mater (TMO) of the base of the brain, which is larger than the defect in the dura mater, the periosteal flap on the vascular pedicle, formed earlier, is placed on the base of the PS, a split bone flap is placed over the periosteal flap, they are fixed with grooves into the PSF defect and enveloped with a periosteal flap, they are sealed with a tachocomb and fibrin-thrombin glue, after which the dura mater defect is sutured using a flap previously formed from the fascia lata of the thigh.
Thus, a multilayer sealed plastics of the skull base is achieved, preventing postoperative liquorrhea and prolapse of the brain substance with membranes into the postresection cavity.
The method is carried out as follows.
After bicoronary skin incision, the periosteum is preserved for subsequent plasty of the skull base defect. A bifrontal craniotomy is performed. Next, block is performed - resection of the tumor of the anterior part of the skull base. In advance, from a linear incision in the anterolateral region of the thigh, a flap is taken from the fascia lata of the thigh. Then, with the help of a high-speed bur, a reciprocating saw and a chisel, a split bone flap is formed from the convexital surface of the skull, slightly larger than the defect in the PSF area. On the sides of the bone flap, with the help of a bur, grooves are made for its attachment in the defect of the CPF. In the defect of the dura mater (DM) of the base of the brain, artificial dura mater is placed subdurally, which is larger in size than the defect in the dura mater. The previously formed periosteal flap on the vascular pedicle is placed on the base of the PCF. A split bone flap is placed on top of the periosteal flap, fixed with grooves in the PCF defect and enveloped with a periosteal flap. Sealing is performed using a tachocomb and fibrin-thrombin glue, after which the dura mater defect is sutured using a flap from

The inventive method was developed and passed clinical trials on the basis of the 5th neurosurgical department of the FBGU NMITs im.
