Abstract
Description
Изобретение относится к хирургии, травматологии, комбустиологии, дерматовенерологии, к области лечения кожных заболеваний, ожогов и поверхностных, а так же глубоких ран.The invention relates to surgery, traumatology, combustiology, dermatovenerology, to the field of treatment of skin diseases, burns and superficial, as well as deep wounds.
Различные препараты серебра находят все большее медицинское применение для лечения различных заболеваний, в том числе кожных и сопряженных с инфекционным процессом, вызванным условно-патогенной микрофлорой. Наноразмерные частицы серебра, благодаря ярко выраженным биоцидным свойствам по отношению к более чем 250 видам болезнетворных микроорганизмов, являются перспективным материалом и находят широкое применение в медицине, ветеринарии и производстве косметических средств.Various silver preparations are increasingly used for medical treatment of various diseases, including skin and associated with the infectious process caused by opportunistic microflora. Due to their pronounced biocidal properties in relation to more than 250 types of pathogenic microorganisms, nanosized silver particles are a promising material and are widely used in medicine, veterinary medicine and the manufacture of cosmetics.
Succinic acid preparations have long been widely used in the treatment of a wide range of internal, skin and systemic diseases, being a means of adjuvant therapy and significantly reducing the cost of treatment (source: N. Mazina, “Systematic approach to the rationale for the use of energy metabolism regulators in pharmacotherapy and healing regimens” Abstract of dissertation for the degree of Doctor of Medical Sciences Tomsk, 2007). For parenteral use, succinic acid preparations are widely used in the form of methylglucamine mixtures. However, the properties of the combined use of succinic acid preparations with noble metal nanoparticles have not been studied.
Известно несколько способов получения наноразмерные частицы серебра в жидких средах, среди которых наиболее традиционным является химическое восстановление растворимых соединений серебра различными восстановителями. Так, например, известен способ получения наночастиц серебра в водной среде, описанный Rodrigues-Sanchez L et al., 2000. (библиографическая ссылка: Rodrigues-Sanchez L., Blanko M.L., Lopez-Quintela M.A. Electro-chemical Synthesis of Silver Nano-particles. J. Phys. Chem. B. 2000. Vol. 104., p. 9683-9688). Several methods are known for producing silver nanosized particles in liquid media, among which the most traditional is the chemical reduction of soluble silver compounds with various reducing agents. For example, there is a known method for producing silver nanoparticles in an aqueous medium described by Rodrigues-Sanchez L et al., 2000. (bibliography link: Rodrigues-Sanchez L., Blanko ML, Lopez-Quintela MA Electro-chemical Synthesis of Silver Nano-particles J. Phys. Chem. B. 2000. Vol. 104., p. 9683-9688). It consists of the following stages: dissolution of the stabilizer (tetrabutylammonium bromide) in an organic solvent (acetonitrile); electrochemical dissolution of the anode (silver plate) in the organic medium obtained in the first stage. In this case, platinum or aluminum is used as a cathode when passing a constant electric current through the solution. In the described method, most (55-80%) of the electrochemically dissolved silver is deposited on the cathodes in the form of films (i.e., the silver nanoparticle yield to the solution is not higher than 45%). Another significant disadvantage of this method is the use of toxic acetonitrile as a solvent, which excludes the possibility of using a finishing medium containing silver nanoparticles for medicine, veterinary medicine, and the manufacture of cosmetic preparations.

Another significant disadvantage of this method is the use of toxic acetonitrile as a solvent, which excludes the possibility of using a finishing medium containing silver nanoparticles for medicine, veterinary medicine, and the manufacture of cosmetic preparations.
2010).The closest in technical essence to the proposed one is a method for producing silver nanoparticles, including dissolving stabilizers in a solvent, placing an anode made in the form of a silver plate and a cathode in the resulting solution, electrochemical dissolving the anode when a stabilized direct current is passed through the solution, distilled is used as a solvent water, a stainless steel plate is used as a cathode, and the process of dissolution of stabilizers takes place in two stages: first, In distilled water, a stabilizer selected from polyglycols, polyvinylpyrrolidone, gelatin, or sodium or potassium polyacrylate is dissolved in distilled water while heating to 45-55 ° С and then, after cooling, a stabilizer selected from ammonium, potassium, or sodium citrates is added to the resulting solution with stirring (method Kreitsberg-Golikov RU 239 03 44 C2, IPC A61K 33/38, publ. 2010).
Недостатком указанного способа получения наночастиц серебра в водной среде является его сложность осуществления, вероятность контакта рабочего персонала с профессиональными вредностями токсического плана. Организационные и ресурсно-аппаратные издержки данного способа не позволили авторам отработать и изучить комбинированные схемы применения их серебросодержащего продукта совместно с другими известными препаратами и регуляторами энергетического метаболизма для лечения кожных и других заболеваний.The disadvantage of this method of producing silver nanoparticles in an aqueous medium is its difficulty in implementing, the likelihood of contact of working personnel with occupational hazards of the toxic plan. Organizational and resource-hardware costs of this method did not allow the authors to work out and study the combined use of their silver-containing product in conjunction with other well-known drugs and energy metabolism regulators for the treatment of skin and other diseases.
Техническим результатом предлагаемого изобретения является повышение эффективности лечения кожных заболеваний, ожогов, поверхностных и глубоких ран.The technical result of the invention is to increase the effectiveness of the treatment of skin diseases, burns, superficial and deep wounds.
The technical result is achieved in that for the treatment of skin diseases, burns, superficial and deep wounds, which consists in applying to the skin or wound in the area of the lesion of an aqueous micellar solution of nanosized silver particles (or monovalent silver oxide) with or without ammonium ion stabilizers, with regularity up to three times a day for up to ten days, simultaneously with this application, an intravenous drip of a medicinal solution of succinic acid in the form of methylglucamine impurity or compounds based 5-6 g of succinic acid to 1 liter of the infusion mixture.
Nanosized silver particles are agglomerates of atomic silver, or silver compounds such as monovalent silver oxide 1-100 nm in size, the surface of which is surrounded by a layer of stabilizer molecules, which allows the water / stabilizers / nanosized silver particles to have a “life” of at least 12 months. Succinic acid preparations are well-studied medicinal substances that are used in hypoxic conditions, mitochondrial dysfunctions, and as additive therapy in many areas of medicine.
For the treatment of a wide range of diseases, such unique and difficult to reproduce in other nanotechnological analogs properties of nanosized silver particles are of interest, such as high bactericidal activity against a large number of strains of “gram +” and “gram -” bacteria, fungi, combined with low toxicity and high safety for organism. The surface properties make silver nanosized particles indispensable for controlling the release of drugs and their passage through the skin (thus, the pharmacokinetics of skin preparations can be regulated).
Предлагаемый способ местного медицинского применения наноразмерных частиц серебра в сочетании с системным назначением регулятора энергетического метаболизма включает в себя использование наноразмерных частиц серебра (оксида одновалентного серебра), в жидких средах для комбинации медицинских назначений таких частиц для местного применения с препаратами янтарной кислоты, состоит из 2-х основных операций:The proposed method for local medical use of nanosized particles of silver in combination with the systemic purpose of the energy metabolism regulator includes the use of nanosized particles of silver (monovalent silver oxide) in liquid media for the combination of medical purposes of such particles for local use with succinic acid preparations, consists of 2- x main operations:
1. Нанесение водного раствора наноразмерных частиц серебра (оксида одновалентного серебра) на очаги поражения (кожные покровы, раны) в виде курса ежедневных процедур (1-3 раза в сутки) разной продолжительности.1. Application of an aqueous solution of nanosized particles of silver (monovalent silver oxide) on the lesions (skin, wounds) in the form of a course of daily procedures (1-3 times a day) of different durations.
2. Или одновременное назначение метилглюкаминовых соединений янтарной кислоты в виде внутривенных капельных вливаний в составе сбалансированных лекарственных растворов.2. Or, the simultaneous administration of methyl glucamine succinic acid compounds in the form of intravenous drip infusions as part of balanced medicinal solutions.
Указанный способ лечения успешно использовался в клинике, что видно из следующих примеров:The indicated method of treatment was successfully used in the clinic, as can be seen from the following examples:

Оказалось, что в группе «Янтарь» длительность госпитализации уменьшалась на 10% по сравнению с контрольной группой. Так же в группе «Янтарь» уменьшалось расходование дорогостоящих медикаментов и медицинских материалов, что несколько уменьшало стоимость лечения. Кроме того, в группе «Янтарь» не наблюдалось летальных исходов (в контрольной группе произошло 2 смерти пациентов).Example 3. In the burn department of the Kirov Regional Clinical Budgetary Healthcare Institution (KOKBUZ) "Center for Traumatology, Orthopedics and Neurosurgery", 30 patients with combined traumatic injuries (burns of 2-3 severity in combination with fractures of different locations) were treated. It turned out that in the Yantar group, the length of hospitalization was reduced by 10% compared with the control group. Also, in the Yantar group, the expenditure of expensive medicines and medical materials decreased, which somewhat reduced the cost of treatment. In addition, in the Yantar group there were no fatal outcomes (2 patients died in the control group).
Improving the effectiveness of the treatment of skin diseases, burns and superficial deep wounds, as well as reducing the cost of treatment are the advantage and advantage of the proposed method compared to the prototype. At the same time, the local effects of nanosized silver particles in the foci of inflammation are enhanced by systemically prescribed succinic acid preparations, which reduce the hypoxic component of inflammation and, through metabolic mitochondrial support, enhance the function of cellular immunity systems in the foci of infection, entering into synergistic interaction with silver in terms of its antibacterial component.
