Abstract
Description
The alleged invention relates to medicine, in particular to physiology and cardiology, can be used in both clinical and experimental studies.
A known method for determining blood pressure (BP) by the Korotkov method [Medical devices. Development and application / Ed. Revenko S.V. - M .: Medical book, 2004. - S. 326-330], which measure diastolic and systolic blood pressure.
The disadvantages of this solution are the need to create high levels of pressure in the pinch cuff, exceeding the systolic pressure in the artery, as well as the fact that at least 15-20 seconds elapse between the measurement of diastolic and systolic pressure. Thus, the measured pressure values relate to cardiac cycles that are far apart.
Also known tacho-oscillographic method (TO) measurement of blood pressure, proposed N.N. Savitsky [Savitsky N.N. Some research methods and functional assessment of the circulatory system. Medgiz, 1956]. The method is based on the principle of measuring changes in limb volume, which occurs under the action of a pulsating blood flow in the great vessels. This method allows you to measure diastolic (Rmin), mean dynamic (Rsr), lateral systolic (Rbs) and end (Rmax) systolic pressures in the main arterial vessel of the limb, on which a pinch measuring cuff is applied. Using the above BP values, pulse (dP, Rbs, Rmin) and shock (Rud, Rmax, Rbs) BPs are calculated. The measurement error of the first four indicators of blood pressure according to the author is 5 mm Hg. when the pressure rise rate in the pinch cuff is 4-5 mm Hg / s.
The disadvantage of this method is a number of instrumental and methodological flaws that dramatically increase the measurement error.
The prototype adopted an oscillographic method for measuring blood pressure [see RF patent No. 2441581, class АВВ 5/022, BI dated 02.10.2012], including the registration and analysis of oscillograms of arteries at frequencies from 0-0.1 Hz to 40-60 Hz, followed by electrical conversion. The compression of the pinch measuring cuff is continued until the OSG waves appear. The diastolic and systolic pressure values are determined from the pressure value in the pinch measuring cuff. In this case, the limit value of the amplitude and time constant is determined. To determine the systolic pressure on the systolic part of the waveform, the current amplitude is recorded at the first time moment and the second amplitude is measured at a multiple point in time from the initial time value. From the two values of the amplitude and time instants, the limiting value of the amplitude and the time constant are found, from which systolic pressure is determined, then diastolic pressure is similarly found.
The disadvantage of the prototype is the low accuracy of measurements due to measurements by a calibration characteristic with known parameters, which in practice are usually unknown and vary non-linearly, compensating for the uncertainty of another parameter chosen arbitrarily.
The technical task of the method is to increase metrological efficiency by eliminating methodological and dynamic errors in the calibration characteristic of the maximum amplitude of the oscillogram, compensating for the uncertainty of the time constant chosen arbitrarily.
The stated technical problem is achieved by the fact that in the method for determining blood pressure, which includes registration and analysis of oscillograms of arterial vessels in the process of increasing pressure in a pinch measuring cuff with subsequent electrical conversion, registration and analysis of volumetric oscillograms (OCG) of arterial vessels is performed in a frequency band from 0-0 , 1 Hz to 40-60 Hz, the compression of the pinch measuring cuff is continued until the appearance of OSG waves, the value of diastolic and systolic yes is determined the pressure value in the pinch measuring cuff, determine the limit value of the amplitude and time constant, in contrast to the prototype, to determine the systolic pressure on the systolic part of the waveform, the time constant is recorded from the calibration characteristic, calibration is carried out a priori for two measured and known values of the upper and lower boundaries adaptive range, the calibration characteristic is the function of the maximum amplitude of the waveform, compensating for the uncertainty of the constant time selected arbitrarily, and linking the reference and measured characteristics by normalizing the measured values by known ones, find the actual values of the time constant and the amplitude amplitude limit values from the calibration characteristic, which are used to construct the calibration characteristic, reference characteristic and determine systolic pressure, diastolic pressure is similarly found .
U0i.1. The time constant T 0 is determined from the calibration function U 0i .
2. Calibration is carried out a priori for the known reference U E (Fig. 1 curve 1) and measured U (Fig. 1 curve 2) blood pressure values.
3. The calibration characteristic is the characteristic U 0i of the limiting amplitude of the waveform, compensating for the uncertainty of the time constant T * , chosen arbitrarily, and linking the reference U E and the measured U dependences due to normalization of the measured values by known
From the calibration characteristic U 0i , the characteristic U (t) is restored, which is identical to the reference
которая максимально приближена к эталонной кривой UЭ(t):which is as close as possible to the reference curve U e (t):
Эталонная характеристика UЭ(t) и характеристика, ей тождественная, U (t) получены из экспоненциальной динамической характеристики с искомыми информативными параметрами U0, Т0:The reference characteristic U Э (t) and the characteristic identical to it, U (t) are obtained from the exponential dynamic characteristic with the desired informative parameters U 0 , Т 0 :
Физический смысл информативных параметров следует из предельных соотношений:where T 0 is the time constant and U 0 is the limit value of the amplitude. The physical meaning of informative parameters follows from the limit relations:
т.е. U 0 is the limiting blood pressure for t = 0,
т.е. Т0 - постоянная времени.those. T 0 - time constant.
In practice, one of the informative parameters of the studied characteristics is usually unknown. In this case, one parameter is set arbitrarily T * , and the second takes the form of a function U 0i , which compensates for the ignorance of the first informative parameter. Using this function, the measured characteristic is calibrated.
(1).We arbitrarily set the parameter T * = const instead of the unknown real value of the time constant T 0 . To compensate for the arbitrariness of the constant T *, the limit value of the amplitude U 0 will turn into a characteristic U 0i , compensating for the ignorance of the time constant T 0 . The calibration function for the known parameters T 0 , U 0 is the exponential dynamic characteristic (1).
U0i:We express the calibration characteristic U 0i from the system of equations with known parameters T 0 , U 0 of the characteristic U Э (t), which is the reference (obtained by approximating the experimental data), and the characteristics and (t) = U i , which is measured, with an arbitrary constant T * and characteristic U 0i :
We divide one equation of the system into another in order to express the calibration characteristic:
In accordance with the laws of calibration and t E = t follows the calibration characteristic U 0i , which relates the standard and measured curves:
1, кривая 3).Therefore, the calibration characteristic is the function of the amplitude limit value, which compensates for the uncertainty of the time constant chosen arbitrarily (Fig. 1, curve 3).
Using the calibration characteristic U 0i , the real values of the time constant T 0 and the limit value of the amplitude U 0 are found , which are informative parameters that deliver the optimum to the calibration characteristic. From characteristic (4) we compose a system of equations for i = 1,2:
0:Dividing one equation of system (5) into another and prologarithmic, determine the algorithm of the time constant T 0 :
Therefore, the time constant optimization algorithm (6) is regulated by the ratio of the time range to the logarithm of the measured amplitudes of the boundaries of the waveform at multiple times.
Expressing T 0 from the first and second equations of system (5) and equating them to each other
находят алгоритм определения предельного значения амплитуды осциллограммы:find an algorithm for determining the limit value of the amplitude of the waveform:
Therefore, the algorithm (7) for optimizing the limit value of the amplitude of the waveform is regulated by the ratio of the measured amplitudes of the boundaries of the waveform at multiple times.
е. U≡UЭ.5. From the actual values of the time constant T 0 and the limit value of the amplitude of the waveform U 0 , the calibration characteristic U 0i of the limiting blood voltage and the reference characteristic U E are successively constructed. The calibration result is the identity of the measured characteristic U reference U E , i.e. U≡U E.
2, линия) искомой характеристике, когда Udi=Uэi.For informative parameters (6) and (7), a calibration characteristic U 0i (4) is constructed (approximated) (Fig. 1 curve 3), from which, according to (3), the actual U di characteristic (Fig. 2, points) is found that is identical to the reference U ei (Fig. 2, line) of the desired characteristic when U di = U ei .
3)6. Measure systolic pressure, (Fig. 3)
TS.Approximating the oscillogram according to dependence (2), a reference measure is introduced, which is equal to the time constant T S.
For the systolic part of the model, t = T S ; therefore, for the measured pressure P = νt according to the linear law:
где ν - скорость линейного набора давления в пережимной измерительной манжете.where ν is the linear pressure gain rate in the pinch measuring cuff.
7. Measurement of diastolic pressure.
Similarly, for the diastolic part, a reference measure is introduced, which is equal to the time constant T D , and diastolic pressure is measured:
PD=νt.P D = νt.
The adequacy of the proposed method to the physics of the experiment is proved by mathematical modeling of the actual characteristics, relative to the equivalent of the experimental characteristics, according to the obtained values.
The adequacy of the obtained dependencies is assessed by the formula for determining the relative error:
4а.its assessment is presented in FIG. 4a.
When setting an arbitrary value T * = const other than the reference T 0 , the limit value of the amplitude of the waveform U 0 turns into a function that compensates for the ignorance of the value of T 0 . The reference and actual characteristics are identical (an error of the order of 2 * 10 -8 (Fig. 4 a), which proves the effectiveness of the use of calibration.
Ui:The effectiveness of the accuracy of the proposed solution is a constant unit level, and for the prototype it is determined by the nonlinearity η (see Fig. 4b) of the calibration characteristic U i :
2).The nonlinearity (9) of the prototype regulates the methodological error (see Fig. 4b) for the known parameters U 0 , T 0 equivalent, but in practice, as a rule, one of the parameters is unknown. It is arbitrarily chosen by T *. In this case, the second parameter from the constant U 0 takes the form of a function U oi (Fig. 1, curve 3), which compensates for the ignorance of the parameter T 0 , which eliminates the methodological error (9) of the characteristic. This follows from the identity of the equivalent of the actual characteristics (Fig. 2).
Thus, the determination of blood pressure from the calibration characteristic of the limit amplitude of the waveform, compensating for the uncertainty of the time constant, chosen arbitrarily, in contrast to the known solutions (Fig. 4b), increases the accuracy of determining blood pressure by several orders of magnitude due to the adequacy of the proposed method to experiment in the absence of a methodological and dynamic errors.
