Количество лиц со стойкой нетрудоспособностью в результате перенесенной ЧМТ достигает 25-30%. При этом основной контингент пострадавших - лица трудоспособного возраста (от 20 до 50 лет). Общая летальность при ЧМТ, включая ЧМТ легкой и средней степени тяжести, составляет 5-10%, а при тяжелых формах ЧМТ возрастает до 41-85% (http://neurosklif.ru/Diseases/Head Injury). Среди зарегистрированных и разрешенных к медицинскому применению препаратов на Российском фармацевтическом рынке широкое распространение для лечения последствий ЧМТ получили Глиатилин® (производитель Италфармако/Фармакор (Италия/Россия)) и его дженериковые версии - Церепро® (производитель Верофарм (Россия)) и Церетон® (производитель Сотекс (Россия)), содержащие в качестве действующего вещества холин альфосцерат (RU 2406506, 2010). Холин альфосцерат обладает холиномимети-ческим действием, стимулирует преимущественно центральные холиноре-цепторы. Препарат активирует церебральный кровоток, стимулирует метаболизм ЦНС, возбуждает ретикулярную формацию. У больного повышается настроение, стимулируется умственную деятельность, улучшается концентрация внимания, способность к запоминанию и воспроизведению полученной информации. Наряду с холин альфосцератом в качестве базисных средств фармакотерапии у больных с черепно-мозговой травмой используют другие производные аминоэтанола, в частности, деанолаацеглумат, а также цитиколин. (Регистр лекарственных средств России, 2007 (15). М.: «РЛС», 2007, с. 973). Эти препараты способны оптимизировать холинергическую передачу в ЦНС, снижать выраженность воспаления и окислительного стресса в поврежденных нейронах, а также стимулировать нейро- и ангиогенез. Так, как ЧМТ сопровождается накоплением в цереброспинальной жидкости (ЦСЖ) токсических для ЦНС метаболитов (средне молекулярных олигопептидов, оксигемоглобина, билирубина и серотонина), появляющихся в результате деструкции мозговой ткани и кровоизлияния в субарахноидальное пространство, то перспективным в лечении ЧМТ является детоксикация ЦСЖ. В качестве лечения применяют осмодиуретики (маннитол, рео-глюман), барбитураты (тиопентал натрия, фенобарбитал), глюкокортикоиды (дексазон). (RU 2252023, 2004).
