The invention relates to rehabilitation medicine and can be used to correct the psycho-emotional state of patients by reducing the tonic activity of the facial muscles.
The level of technology .
Emotional stress is one of the main threats to the health and economic well-being of people regardless of age and cultural affiliation (Souza-Talarico JN., 2016). Constant emotional stress is fraught with the development of additional depressions, emotional burnout, a high level of anxiety, disability, etc. According to the World Health Organization (WHO), depression today ranks fifth in terms of labor losses, and by 2026 it may take second place after cardiovascular disease. Large-scale cross-sectional studies confirm the association of work stress with psycho-emotional disorders and psychosomatic diseases (Li J et al., 2016). Obviously, emotional stress entails the discord of the entire psychophysical system of the body.
(Thomas W. Myers, 2012). Modern studies show that one of the markers of mental tension and emotional stress is increased tonic muscle activity (Dubner R., 2013). It is proved that muscular tension in mental patients is higher than in healthy people, especially it is high in persons with a predominance of pathological anxiety (Danilova N.N., Krylova A.L., 2005). Various mechanisms of psychological defense and the protective behavior associated with them contribute to the formation of “muscle armor” or muscle pattern, manifested in the tension of different muscle groups (V. Reich, 2000). Experienced emotions of fear are fixed in skeletal muscles, which is reflected in constant stoop, tension in the muscle structures of the neck, back and abdomen, constrained breathing, restraint of movements (Thomas W. Myers, 2012).
(Le Louarn C., Buthiau D., Buis J., 2007). The facial muscles are also in a state of increased tone. In the process of aging, muscles do not relax, but contract due to the gradual replacement of muscle fibers by connective tissue (Bannier B., Elbaz D., 1994, Fournier P., 1996; Gala R., 1999). MRI analysis of the muscles of the face of people of different ages showed that they are in a state of shortening and spasm (Le Louarn C., Buthiau D., Buis J., 2007).
В., 2010),According to recent studies, the tension of the facial muscles can be triggered by the skeletal muscles through chain muscle coordination (Conette de Saint-Cyr D., 1994). For example, spasms of the trapezoid and occipital muscle groups trigger excessive excitation of the frontal muscle and provoke the frontal-temporal headaches (Petrov KB, Mitichkina TV, 2010)
1996). Methods of body-oriented therapy and yoga are also widely used in restoring psycho-emotional status, including massage techniques for reducing chronic tension and body clamps (W. Reich, 2000, M. Feldenkrais, 2000, A. Lowen 1996).
For the correction of muscle tone in the practice of restorative medicine a number of measures are used: massage, physical therapy, posture correction (adhesive plaster tension), physiotherapy. However, these techniques work mainly in a narrow area - at the level of skeletal, not facial muscles. There are no clear science-based algorithms and methods for using the procedures of neuro-muscular relaxation of the facial muscles to effectively address the issues of correction of the psycho-emotional state of patients. There are no scientific data on the clinical dynamics of psycho-emotional status in the process of neuro-muscular relaxation.
№ 21). The closest analogue of the claimed invention is a method of correction of the patient's psycho-emotional state, including the detection of deep muscular tension according to the doctor's sensations and the effect on them by the following methods: through the tissue with the base of the palm, when the patient moves upward through the body, massage is performed using only the kneading technique, deepening the relaxation effect found deep muscular tension by fixing the patient’s attention to these stresses and further massage by kneading, control cosiness changes in the patient's psychoemotional state and / or changes in pain in the corresponding organs associated with these muscles and / or close to them located, kneading effects are performed until the effect of relaxation of deep muscular tension is obtained (patent RU 2395308, published: 07.27.2010 Bull. No. 21).
Р). However, this massage technique involves the effect only on skeletal muscles. It lacks methods for specific work to reduce the tension of the facial muscles, whereas it is the impact on the muscular system of the face that effectively corrects the processes of arousal and inhibition in the limbic-reticular complex and stabilizes the psycho-emotional state of the patients. At the same time, the connection of the facial muscles with the emotional centers of the brain is carried out reflexively through the trigeminal and facial nerves (Wayne Mingazova L.R.).
To eliminate these drawbacks, we propose a method of conducting a massage to improve the psychoemotional status of patients, which consists in performing a neuro-muscular relaxation of the face, including the following steps:
на первом этапе производят разминающие и прокатывающие движения для разогрева мягких тканей волосистой части головы и лица и подготовки к процедуре массажа, in the first stage, kneading and rolling movements are performed to warm up the soft tissues of the scalp and face and prepare for the massage procedure,

The technical result of the claimed invention is as follows.
1) The application of the methods of the claimed method not only reduces pathological arousal in the facial muscles, but also, on the basis of feedback, causes a decrease in bioelectric activity in the corresponding parts of the brain (limbic-reticular complex). As a result, the vicious circle is opened between the hyperactive facial muscles and the patient’s negative emotional states.
2) The massage technique allows to firmly reduce the parafunction of the masticatory muscles, stabilizes the position of the lower jaw, improves the physiological amplitude of the mouth opening, articulation, chewing functions. Reducing the tone and relieving muscle excitement in the area of the facial muscles leads to a decrease in pain, swelling, the formation of mobility and plasticity of mimicry, smoothing of the epithelial tissues and skin, changes in facial expression in the direction of a softer and more open.
3) The method also contributes to the prevention of age-related changes and provides a bright aesthetic effect, manifested in the restoration of facial proportions, improving symmetry, complexion, smoothing wrinkles, folds and skin creases. The method triggers opportunities for facial rejuvenation, which has a positive effect on the psycho-emotional state of the patient, primarily on self-esteem.

Disclosure of the invention .
The method includes the following steps.
Stage 1. Preparation and warming up the workspace
Within 5 minutes, superficial kneading of soft tissues of the scalp and face is performed. In the frontal area, light rolling movements of the active hand from the level of the eyebrows upwards to the scalp are made by the tener. The movement ends 2-3 cm above the hairline. Then, the so-called surface “search movements” are performed over the entire area of the middle and lower part of the face — folds of soft tissues are captured and their resistance to lightweight tension up and down, right and left at different depths at different tilt angles is evaluated. The access level is superficial (Eremushkin MA 2009, Tripolskaya, 2000). Tension or pressure on the fabric - not more than 0.3 kg.
Stage 2. Reduction of fibrous adhesions in the face .
The goal of the stage is to eliminate the growth of connective tissue and the pathological “gluing” of the skin with the covering tissues of the face. These processes are a frequent consequence of primary degenerative changes in muscle tissue as a result of prolonged tension and lack of blood circulation (V. Bartashov, 2005). Such work allows freeing outflow paths to the regional lymph nodes (parotid, submandibular, mastoid, jugular-double-ventricular, etc.), as well as increasing the plasticity and mobility of the facial muscular system, which is an essential condition for reducing muscle inflammation and tension. time from 15 minutes. The second stage can become a separate 45-60 minute procedure in the presence of large areas of connective tissue growth in the head area.
Work on the elimination of fibrous adhesions is carried out in the following areas: parotid-masticatory, submental areas, in the region of the upper forehead and supracranial aponeurosis.
In the parotid-masticatory area on the “cheek-eyelid” border, a fold 2-3 cm long is caught. The fold rises 1 cm perpendicular to the face and slowly rolls from top to bottom. The movements are performed in the projection of the surface part of the chewing muscle - from the angle of the eye to the angle of the lower jaw. During the massage, diagnostics is performed - fibrosing areas or skin adhesions with integumentary tissues, local seals, “pulling” of the muscle or its individual bundles (Petrov, Mitichkina, 2010), painful or tense areas, which are detected by performing a corrective technique - successively intercepting movement in a narrow area of 1-2 cm (5 to 15 times). In the process of performing intercepting movements, a volume of tissues, including the veins, lumps, compacted areas, and nodes, is collected in the tuck zone. The working area gently stretches and then stretches from the center to the side.
In the submental area, a vertical nip is captured 2-3 cm long and successive slow intercepting movements are made on a narrow section of 1-2 cm (5-7 times). In the process of performing intercepting movements, a volume of tissues, including the veins, lumps, compacted areas, and nodes, is collected in the tuck zone. Vibration, shaking and kneading of muscle seals, lumps, nodules. The working area gently stretches and then stretches from the center to the side.
Since the muscles of the face are very strongly associated with supracranial aponeurosis, the scalp, when the rigidity of the tissues of the cranial vault is high and rigid, the process of tension of the forehead, eyes, and middle part of the face starts up along the chain. Therefore, the procedure is recommended to include massage of the supracranial aponeurosis and the upper part of the forehead.
Treatment of the supracranial aponeurosis is performed as follows. On an arbitrary part of the scalp is captured a bunch of hair. The other hand is placed on the scalp 2-4 cm from the working hand. Hand movements are performed in different directions in order to increase mobility. Permissible circular, zigzag, spiral movement. After 5-10 seconds, proceed to the processing of other zones. When performing massage it is necessary to achieve an increase in the amplitude of movement of tissues by 2 or more centimeters. To do this, it is necessary to overcome the resistance of tense and rigid sections.
The area of the upper part of the forehead (along the “headband line”) is worked out by circular kneading movement with an emphasis on increasing the amplitude of skin movement.
Stage 3. Reduction of swelling in the head and neck
Within 5-10 minutes lymphatic drainage massage is performed in the head and neck. First, light kneading of tissues in the parotid-masticatory, submental, supraclavicular, and occipital areas, kneading tissues above the sternocleidomastoid muscle and the clavicle area, thus involving the main groups of lymph nodes of the head and neck. Then produce vibration movements from successively studied sections of the head and neck, each of which will be called the working section, towards the specified groups of lymph nodes.
Then, tissue squeezing of tissues from each working area to the locations of the main groups of head and neck lymph nodes is carried out. When processing the forehead, squeezing is performed from the bottom upwards along the frontal abdomen of the occipital-frontal muscle, stopping 2-3 cm above the hairline, then massaging the entire scalp and the occipital region.
Stage 4. Identification of primary foci of muscle excitation
Within 3-5 minutes, a differentiated palpation is made by search movements: distal phalanxes of the I and II fingers of both hands in the middle and lower part of the face gather an arbitrary section of integumentary tissues. The access level is fascial (Yeremushkin MA, 2009, Tripolskaya, 2000). At an angle of 90 degrees to the working surface, with slow motion, the assembled area is raised from the bone bed upward by 0.5-1 cm up to the stop.
Manipulations are performed to search for local muscle seals and painful points inside the muscle tissue, assess pain sensations in different areas of the face. Estimated resistance to grip and stretching up, the amplitude of movement, the possible loss of tissue from the grip.
When performing palpation, there are areas that are primary foci of muscle excitation, which we will call the most “compromised areas”, characterized by the following features:
• strong resistance to gripping and stretching upwards. Fabrics like a tight spring tend to break free from the grip of the fingers.
• наличие очага возбуждения в виде локального «втяжения» мышцы или ее отдельных пучков (Dicke 1929, Петров, Митичкина 2010, Трипольская, 2000) - уплотненного участка тканей в щечной, надбровной, или околоушно-жевательной области;• the presence of an excitation focus in the form of a local “retraction” of the muscle or its individual bundles (Dicke 1929, Petrov, Mitichkina 2010, Tripolskaya, 2000) - a compacted tissue segment in the buccal, brow, or parotid-masticatory region;
• низкая амплитуда движения - менее 0,5 см при силе 0,4 кг (возможный маркер сращения кожи с покровными тканями); • low amplitude of movement - less than 0.5 cm with a force of 0.4 kg (a possible marker of skin adhesion with epithelial tissues);
• сильные болевые ощущения;• severe pain;
Stage 5. Detection of individual sites of muscle tension .
Sequentially working at the previous stage with primary foci of muscular tension, following the direction of their tension, it is possible to detect certain sections of the tendon-muscular cords (Travell, Simons, 1989) - short and long, diagonal, vertical and longitudinal location, and superficial and deep occurrence.
As a rule, palpation of the tendon-muscular strand along the entire length (especially in the middle part of the face) is impossible along the entire length, since it causes acute pain - the so-called “jump syndrome”. Therefore, the work consists in thorough preparation for palpation of a string from “head to tail” (Petrov, Mitichkina, 2010).
In order to identify these strands for the purpose of separate study, they use the “root capture” technique: the distal and middle phalanxes of the I and II fingers of one hand take the compromised tissue section into the fold (subcutaneous access) and rise up to the first stop at an angle of 45 degrees. The pressure of the finger pads to each other is minimal - up to 0.2 kg. Further, successive slow intercepting movements are performed with one and the second hand on a narrow section of 1-2 cm, with an emphasis on stretching the tissues upwards from the base of the fold. Execution speed - one interception in 1-3 seconds. The optimal number of interceptions - 5-7, the maximum - 15.
In the process of manipulation, the fingers begin to sink deeper and deeper into the working area at the fascial or even periosteal level (Eremushkin MA, 2009, Tripolskaya, 2000). Due to the contractile properties of fibrous membranes, a “chain effect” occurs — an increasing number of tissues are pulled into the capture zone, including the veins, lumps, dense areas, nodes (Travell, Simons, 1989, Dicke 1929, Badalyan OL, 2013). tissues as a result may increase by 2-3 times. In the course of performing the intercepting movements, it becomes harder to perform the procedure - the force of the masseur's hands to hold the stressed tissues increases from 0.3 to 2.5 kg, since it is necessary to overcome the resistance of the tissues.
10. In the work it is necessary to take into account that the phase of exit to the deep fascial layers coincides with the patient's complaint of pain or scratching with a fingernail. This is one of the criteria for the right job. The patient is asked to evaluate on a verbal scale YOUR degree of cutting from 0 to 10 points. The optimal pain threshold should not exceed 6-7 points out of 10.
With the capture of the maximum amount of fabrics available for work, the intercepting movements become impossible to perform. Accurately the distal phalanxes of the I and II fingers of both hands must intercept the base of the assembled fold and pull as far as possible up from the working surface (base position). When carrying out proper work, the assembled fold contains a section of a tight cylindrical strand (Badalyan OL, 2013) with a strong tendency to draw deep into the thickness of the tissue.
Stage 6. Sequential inactivation of excitation of muscle stretch sites and palpation of the muscle stretch throughout .
From the base position of the previous stage 5, the fixed fold again goes up and down, left and right at different angles of inclination and determines the direction of the most tight movement, accompanied by increased pain. Keeping the depth of capture and the height of the lift, the fingers are shifted by 2 cm to the side in the direction of the most tight movement.
Practice shows that contact with the next section of muscle tension opens after a decrease in arousal in the previous section by means of inactivation of arousal.
Methods of inactivating muscle excitation are performed using the following massage techniques:
Reducing the pain threshold - a method of reflex impact on the working area, in which the massage is performed on the verge of pain to shift the pain barrier and expand the amount of muscle locomotion. Movement in the massaged area is performed until the pain threshold on a scale of 6–7 points.
Deep sliding palpation - palpation, carried out by pressing the tip of one finger perpendicular to the axial line of the muscle fibers before pressing them to the underlying structure, for example, bone. Used to determine seals and trigger points.
Tick palpation - examination of muscles by pincer grip between their thumb and other fingers. A group of muscle fibers is “rolled” between the fingertips to identify compacted cords, determine sensitive muscle points and to provoke a local convulsive response.
Ischemic compression is a strong painful pressure on the trigger point, eliminating pain sensitivity in it. During the procedure, the squeezed tissues turn pale, after cessation of the squeezing they become hyperemic.
Pinch palpation - palpation, carried out with the tip of a finger, located against the intense strand at a right angle to its direction. The tip of the finger is sharply lowered deep into the fabric, then the finger is quickly raised and at the same time hooked on the cord. Finger movements are the same as when twitching a guitar string. Such palpation is most effective for provoking a local convulsive response.
Acupressure - mechanical impact with a finger (fingers) on biologically active points that have a reflex connection with various internal organs and functional systems. On the correctness of finding the point indicate feelings of aches, pain, numbness, pain. Thus, due to the painfulness of any point, it is sometimes possible to assume the disease of an organ. The technique of acupressure includes techniques: rubbing, stroking, pressure, vibration, seizure.
Kneading - continuous or intermittent grasping, lifting, pressing, pushing, squeezing, shifting tissues (mainly muscles). It is performed by one or two hands.
Vibration is the transfer of oscillatory movements to the massaged part of the body, which are produced uniformly, but with different speed and amplitude. Vibration is performed by the thumb (fingers), forefinger and thumb (or index and middle fingers form a "fork"), palm, base of the palm, fist. Types of vibration: continuous (stable, labile), intermittent.
Stroking is done with the thumb (or middle) pad with rotational movements. It is used mainly in the head, face, neck, hands and, moreover, at the end of the entire procedure.
The rubbing is done with the thumb or middle finger clockwise. Rubbing is used independently and, as a rule, after all other methods of acupressure.
The master chooses the tactics of using the technique depending on the location of the work area, the degree of muscular tension, the presence or absence of edema.
10.After the inactivation of the excitation in the studied part of the muscle strand, they proceed to the next one, displacing the fingers by another 2 cm in the direction of the hardest movement in the fixed fold. Thus, a consistent “pulling out” of the strand from the strata of strained tissues is done through step-by-step sections. At the same time, the presence of a strand in the fold is constantly evaluated. When it is pulled deep into the tissues, it is necessary to make the “root grip” reception described in step 4, to tighten the shortened section from the thickness of the tissues. In the process of performing the techniques in step 5, an analysis is made of the direction, location and depth of occurrence, the size and topography of the voltage, and the degree of retraction of the stressed area. The length of the cord can vary from 2 to 5 cm. The optimal pain threshold should not exceed 6-7 points out of 10.
Stage 7 . Consistent work with 2-3 muscle cords in the middle and lower parts of the face .
3-5. In one session, it is enough to work with 2-3 strands of surface or deep occurrence, strictly following the algorithm described in steps 3-5.
The location of muscle cords, as a rule, is unpredictable, individually and depends on the mimic patterns, constitutional features of the face (thickness of soft tissues, width of the zygomatic bones, relief of the lower jaw, degree of abrasion of teeth), professional activity, etc. (Petrov, Mitichkina 2010, R. Slavicek, 2010). In addition, the directions of the muscle strains and the shape of the foci of muscle tension can vary, and the muscle strands can be palpated at a greater depth from procedure to procedure.
Stage 8. Completion of the procedure
Within 5 minutes, planar stroking of tissues in the working area is performed with pads of II-V fingers - a longitudinal, transverse, spiral. The emphasis is on relaxation and calming tissues, preventing stagnant processes and eliminating contractures.
