Abstract
Description
The invention relates to veterinary medicine, in particular to veterinary surgery, and can be used in the treatment of superficial flat wounds, purulent wounds, non-healing ulcers, burn surfaces, and effective engraftment of skin grafts in skin plastics in animals.
- С. 140-226].In veterinary medicine, methods of treatment using wound dressings (based on tissue, cell-collagen and polymeric materials, including various drugs) are known, a significant drawback of which is low efficiency [Lebedev AV, Lukyanovsky VA, et al., General Veterinary Surgery. - Moscow: Colossus, 2000. - P. 140-226].
Closest to the claimed method is the treatment of purulent wounds and burn surfaces using a therapeutic membrane-controlled dressing comprising a semi-permeable acetate-cellulose membrane made in the form of a tube with an outer diameter of 2.7 cm and a pore size of 2.5-2.8 nm, permeable substances with a molecular weight of up to 10,000 daltons, containing a xerogel of chitosan organic salt (10,000 daltons) and the medicinal composition includes medicines with a molecular weight of up to 5,000 Da [Invention Patent 2233674 Russian Federation, IPC 7 A61L 15 / 00, A61F 13/15, claim 03/20/2003; publ. August 10, 2004]. The method provides an increase in the rehydration of the dry gel and long-term uniform dehydration.
The method is used in surgery, but is not used in veterinary medicine, since the dressing is easily displaced when the animal is moving and injures the wound surface, does not perform the function of artificially covering the wound. Contained in the composition of the bandage xerogel, in the form of a plate, does not provide its congruent fit to the wound surface of the animal. The dressing requires replacement every 6 hours.
The objective of the invention is to create a reliable method for the treatment of superficial plane wounds, non-healing ulcers, burn surfaces, and a tool that increases the effectiveness of skin grafts in skin plastics in animals using membrane dialysis.
The task is achieved in that for membrane dialysis a membrane dialysis dressing for animals is used, the cavity of which is designed as a wide central part and four longitudinal end sections for reliable adhesion to the animal's wound.
Membrane dialysis dressing is applied to the surface of the open wound after its surgical treatment and fixed using an adhesive roll coating. The cavity of the dressing after its installation is filled to volume of high-osmolar gel-based powder of gelatin with a molecular weight of 300,000 Da, antiseptic solution of antiseptic® 0.005%, antiseptic-stimulator Dorogova fraction 2 (ASD® 2-F), anesthetic, with a certain ratio of components. The replacement of dialysate is carried out as the cavity is filled, 1 time in 24-48 hours.
High osmolar gel based on gelatin powder with a molecular weight of 300,000 Da provides a high (9.7 mOsm) osmotic pressure in the membrane capsule, causing massive dehydration of the pathological focus while simultaneously removing low molecular weight toxic compounds, while maintaining the high molecular weight proteins needed to build the cell barrier and regeneration. Antiseptic solution of convoutin®, colloidal silver based drug and benzyldimethyl [3- (myristoylamino) propyl] ammonium chloride. Its activity is based on the complex suppression of the growth of bacteria, viruses, protozoa at any stage. Possessing a broad antiseptic effect, Arsenistin® does not have a local irritant and sensitizing effect, does not possess immunotoxic, hepatotoxic, embryotoxic, teratogenic and carcinogenic properties. ASD ® -2F in the composition of the dialyzing gel, accelerates the processes of regeneration of wounds, has an antiseptic and anti-inflammatory effect, stimulates the activity of the reticuloendothelial system, normalizes the trophism of damaged tissues.
10.12.2012].Membrane dialysis is carried out using a membrane dialysis dressing for animals, developed by the author of the proposed method [US Pat. No. 122575, Russian Federation, IPC A61D 1/00 (2006/01). - №2211105515 / 14, appl. June 13, 2012, publ. 10.12.2012].
The method is as follows.
Pre-prepare the surface of a surface plane or burn wound of an animal for dialysis. For this purpose, processing of the surgical field, anesthesia and primary surgical treatment with necrotomy were performed. At the final stage of the operation, a membrane dialysis dressing for animals was applied to the wound surface, ensuring its congruent fit and full coverage of the defect surface. After applying the dressing on the wound, it is fixed in any available way, for example, using an adhesive roll fixing coating, previously taking out the solution exchange tubes, and fill the cavity with volume of medicinal gel. The approximate composition of the gel for dialysis of surface flat wounds:
аргумистин® 100,0 мл;Argumentia® 100.0 ml;
желатин пищевой - 100,0 г;edible gelatin - 100.0 g;
АСД® 2-Ф - 0,5 мл;ASD® 2-F - 0.5 ml;
Novocain - 0.1 g
Dialysate is changed once every 24-48 hours. The duration of treatment is 4-5 days.
Example. By the present method treated 8 dogs, 6 cats, 12 cows and 10 horses, with the presence of burns, extensive ulcers, superficial wounds with a loss of substance, localized in the head, distal limbs. Surgical interventions were performed on the background of local conduction anesthesia. The surface of the wound was cleared of dead tissue, dirt, exudate, foreign particles, dried and covered with a membrane dialysis dressing. The dialysing gel was injected through the solution-exchange tubes, after the membrane was fixed on the surface of the defect. During wound dialysis, dialysate was replaced once every 24–48 hours, as the dressing cavity was filled.
The basis for the termination of dialysis was to improve the general condition of the animal, reduce puffiness, the appearance of healthy granulations, the disappearance of functional disorders, the normalization of laboratory parameters. Removal of dressings was carried out technically simply, without delivering painful sensations to animals and without causing resistance on their part.
