В настоящее время около 500 млн. человек во всем мире страдают разной степенью хронической болезни почек (ХБП), то есть каждый десятый.Currently, about 500 million people worldwide suffer from varying degrees of chronic kidney disease (CKD), that is, one in ten.
Терминальная стадия ХБП является показанием к трансплантации почки или проведению заместительной почечной терапии (ЗПТ) методом гемодиализа (ГД) или перитонеального диализа (ПД). Около 2 млн. человек получают лечение этими методами для продления их жизни с ежегодным приростом числа пациентов около 8%. В настоящее время эта тема остается наиболее обсуждаемой среди специалистов по ГД, поскольку ни один из известных видов не признан идеальным. У подавляющего большинства из них в качестве постоянного сосудистого доступа использовалась нативная артериовенозная фистула (АВФ) - 88,4% от числа всех больных, у 4,2% - сосудистый протез, у 4,1% - туннельный центральный венозный катетер. Однако от 28% до 50% вновь сформированных доступов невозможно использовать для проведения ГД ввиду так называемой первичной недостаточности, под которой подразумевают отсутствие адекватной функции фистулы для проведения гемодиализа в течение 30-ти дней после ее создания. При этом в ряде случаев требуется имплантация временного двухпросветного центрального венозного катетера (ЦБК) для проведения ЗПТ, что может обуславливать развитие стеноза центральных вен у 40% пациентов. Частое использование ЦБК в России обусловлено высоким процентом больных с поздно выявленной, далеко зашедшей уремией, которые нуждаются в экстренном начале ГД в условиях, когда постоянный сосудистый доступ отсутствует.The gold standard is the formation of a distal radio-cephalic fistula (v. Выполняя неоднократно операции по реконструкции области венозного анастомоза, мы отмечали большие технические трудности ввиду выраженного рубцового процесса, особенно в зоне анастомозов, длительное время хирургического вмешательства, повышенную кровопотерю и необходимость специализированного долгосрочного анестезиологического пособия, значительно повышающего риски интра- и послеоперационных осложнений, что особенно важно у данной группы больных с существующим у них выраженным коморбидным фоном. Учитывая локализацию стенозов, мы попробовали создать иной путь оттока крови и наиболее успешной оказалась имплантация сегмента БПВ в участок синтетического протеза, ранее анастомозированного с веной. Это в свою очередь позволило обойти существующий стеноз в области протезо-венозного анастомоза и обусловило значительное увеличение общей(кумулятивной) проходимости сосудистого протеза, обеспечивая возможность выполнения операции под местной анестезией, не прибегая к анестезиологическому пособию. Given the localization of stenosis, we tried to create a different pathway for blood outflow and the most successful was the implantation of the segment of BPV in the area of a synthetic prosthesis previously anastomosed with a vein. This, in turn, made it possible to bypass the existing stenosis in the area of the prosthetic-venous anastomosis and caused a significant increase in the total (cumulative) patency of the vascular prosthesis, providing the possibility of performing the operation under local anesthesia without resorting to anesthetic aid. This significantly reduced the time for performing the surgical intervention and, which is important, allowed to significantly reduce the risks of intra- and postoperative complications, as well as the invasiveness of the surgical intervention and minimize intraoperative blood loss.

В связи с отсутствием возможности к формированию ПСД на верхних конечностях пациенту 03.06.2013 г. был сформирован ПСД сосудистым протезом на правом бедре. В качестве антиагрегантной терапии пациент получал препараты Ацетилсалициловой кислоты 150 мг в сутки., в октябре 2014 г. - был диагностирован тромбоз доступа, выполнена классическая тромбэктомия. Пациенту на функционирующем доступе было выполнено УЗДС конечности, на которой он был ранее сформирован, по результатам его установлена проходимость общей бедренной вены, области сафено-феморального соустья, большой подкожной вены, вен подвздошного сегмента и диагностирован стеноз зоны протезо-венозного анастомоза с поверхностной бедренной веной до 65%. В связи с удовлетворительной функцией протеза последующие сеансы ГД проводились через протез на бедре. На 5-е сутки пациент в удовлетворительном состоянии выписан под наблюдение врача отделения гемодиализа по месту жительства с явкой для контрольного обследования каждые 6 месяцев.
