Abstract
Description
The present invention relates to medicine, namely to implantable endoprostheses for increasing the volume and correction of the contour of the legs with muscle hypotrophy or hypoplasia.
Unilateral or bilateral hypotrophy, or hypoplasia of the muscles of the legs causes an aesthetically unacceptable violation of the inner contour of the legs, which, mainly in women, leads to persistent socio-psychological discomfort and requires surgical correction. The most stable and predictable result of an increase in the volume of the lower leg is achieved when silicone implants are placed in the soft tissues.
[3].The first known silicone implants containing a gel filler were proposed by J. Glitzenstein [1] for correction of lower legs with amyotrophy. Their disadvantage was a cigar-shaped form, and 100% gel filling, which in the postoperative period gave the lower leg an unnatural contour. Von Szalay changed the shape of the implants, made them flat and gel filled by 90% [2]. A little later, L. Montellano further improved the shape of flat lower leg implants, made them asymmetric, made the upper part of the implants more full and reduced the height of the lower part [3].
A large number of patients want to not only increase the volume of the calf muscles, but also to correct the visual manifestation of the curvature of the legs due to deformation of the tibia. This problem is solved by installing an implant of a greater length exceeding the length of the calf muscle head. In FIG. 1 shows the layout of the implant, where the
However, we noted a significant drawback of known flat asymmetric implants, which manifests itself in a noticeable deformation of the soft tissues of the lower third of the leg in the distant postoperative period (Fig. 2), due to the movement of the cohesive silicone gel into the flat lower part of the implant and its thickening. In FIG. 3 shows a removed silicone implant with a thickened
This complication is due to two reasons. Firstly, the upper part of the implant is under constantly changing pressure of the gastrocnemius muscle, which leads to the gradual extrusion of the cohesive gel. Secondly, the lower part of all known asymmetric implants is made flat, so that their width is 1.9 times or more greater than their height. Therefore, the lower part of the implant, which is outside the calf muscle, over time is filled with an extruded cohesive gel and thickens, causing visible deformation of the soft tissues of the lower third of the leg. The more flattened the implant, the greater the likelihood of its deformation when the lower part of the implant is located outside the calf muscle.
[5].A known solution to the problem proposed by Robert A. Gutstein is the use of an implant that does not contain a fluid gel filler made of a solid silicone block, in which its lower part is elongated by adding a rod of the same material [4]. The disadvantage of this method is the unnaturally high density of the lower leg tissues characteristic of all implants made of silicone blocks [5].
The aim of the invention is to eliminate the disadvantages of known implants while maintaining their advantages. An asymmetric implant consists of a silicone shell and is filled with a cohesive gel. This goal is achieved in that the upper part of the implant is made flattened, and its width is two or more times its height, which preserves the advantages of known implants. Down the upper part of the implant is gradually narrowed and smoothly passes into the lower narrow part of the implant. The lower narrow part for at least 30 mm from the lower (distal) end of the implant is made with a gradually decreasing cross section, and the width of the lower narrow part for the entire part is no more than 1.6 times its height, optimally less than 1.4 times. This embodiment of the lower narrow part of the implant makes it difficult to fill it with a gel filler, which prevents the gel from moving inside the implant and reduces the possibility of its deformation. The lower narrow part allows you to extend the implant as a whole.
In FIG. 4 shows a diagram of a silicone lower leg implant with a lower narrow portion. The
The proposed silicone implant is made for clinical use. In FIG. 5, for comparison, the known implants of asymmetric 5 and symmetrical 6 shapes and the proposed 1 implant with a lower narrow part, which is significantly different from the known ones, are presented. The use of the proposed silicone implant has been tested on 25 patients.
In 11 cases, previously installed deformed implants were replaced.
6.Example. Patient I., 29 years old, was admitted to the clinic with complaints of dissatisfaction with the shape of her lower legs. Earlier, 2 years ago, contour plasty of the lower legs with silicone implants was performed for congenital hypoplasia of the medial heads of the calf muscles. One year after the operation, she noted a swelling in the distal part of the implant. The deformation of the lower part of the implant was diagnosed. It is recommended to replace the implants with those offered with the lower narrow part. Under epidural anesthesia in the patient’s position on the abdomen, a 3 cm skin incision was made along the natural fold in the popliteal fossa on the right. Exposed own popliteal fascia. Retreating 2 cm medially from the median line, the fascia is dissected longitudinally with extension to the tibia of its own fascia. A deformed silicone implant is isolated and removed under the fascia. Under the medial head of the gastrocnemius muscle, a cavity is formed in which the proposed implant is installed with a lower narrow part. Similarly on the left side. The deformation is eliminated, a stable result is obtained. Photos before and 7 months after surgery are shown in FIG. 6.
In 14 cases, primary contour plasty of the lower legs was performed.
7.Example. Patient M., 26 years old, was admitted to the clinic with complaints of dissatisfaction with the shape of her lower legs. The congenital hypoplasia of the medial calf muscle heads, the short medial calf muscle head, and the high risk of implant deformation in the postoperative period were diagnosed. Recommended contour plastics of the lower leg with the proposed implants with the lower narrow part. Under the fascia of the tibia, a cavity is formed in which the proposed implant is installed with a modified lower part. Similarly, on the left side, a stable result is obtained. Photos before and 5 months after surgery are shown in FIG. 7.
The use of the proposed implant allows you to save the advantages of existing implants and eliminate their disadvantages. Reduced the possibility of overflow of gel contents into the lower narrow part of the implant, which prevents the contour of the implant, while maintaining the natural appearance of the lower leg and a feeling of natural softness of the tissues. The proposed implant is intended for repeated operations when replacing deformed implants, as well as for primary contour plastics of the legs.
