Abstract
Description
Technical field
This invention relates to the treatment of cancer, in particular the treatment of leukemia characterized by the presence of a Philadelphia chromosome in tumor cells (Ph + leukemia), leading to the transcription of the BCR / ABL1 fusion gene and the synthesis of the corresponding Bcr-Abl protein, through a new effective pharmaceutical composition, as well as a treatment method .
State of the art
Bcr-Abl is a fusion protein, the product of the BCR / ABL1 fusion gene, resulting from reciprocal translocation between chromosomes 9 and 22 (Philadelphia chromosome). Bcr-Abl is a constitutively active tyrosine kinase responsible for the oncogenic transformation of cells (oncoprotein). The constant activity of this tyrosine kinase makes the cell able to divide without the influence of growth factors and causes its excessive proliferation. BCR / ABL1 is a pathogenetic factor in the development of the so-called Ph + leukemia (Ph-positive leukemia), which includes the vast majority of cases of chronic myelogenous leukemia and 20-50% of cases of acute B-lymphoblastic leukemia in adults.
Inhibition of the kinase activity of the Bcr-Abl fusion protein is a promising strategy for combating various oncological diseases, in particular, Ph + leukemia. Among the approved agents for the treatment of Ph + leukemia, Abl kinase inhibitors such as imatinib, dasatinib, nilotinib, bozutinib, ponatinib can be distinguished.
Pharmacokinetics of the pharmaceutical composition of the invention
1.In order to determine the adequacy of the regimen of the drug according to the invention, its pharmacokinetics was studied. Patients with Ph + leukemia were given a composition of the invention containing 100 mg of the active ingredient, one, two, four or six capsules per dose. The results of the study are presented in table 1.
Table 1. Pharmacokinetic parameters of compound I in blood plasma with daily oral administration once a day by patients with Ph + leukemia of the composition of the invention
(Mian et al., PF-114, a potent and selective inhibitor of native and mutated BCR/ABL is active against Philadelphia chromosome-positive (Ph+) leukemias harboring the T315I mutation, Leukemia. As a result of the studies, it was unexpectedly found that a single oral administration of the composition according to the invention in doses of the active component of 200-600 mg allows to achieve a high therapeutic effect throughout the day (the half-life of the drug in humans was 16-25 hours). This result was unexpected, since according to the preclinical study of the pharmacokinetics of compound I, the half-life in rats (2-4 hours) and dogs (4-7 hours) testified in favor of at least twice taking the drug per day. The study of various doses of the composition according to the invention showed that in doses of the drug starting from 200 mg the average daily concentration of the drug exceeded 0.1 μM, which, according to the publication (Mian et al., PF-114, a potent and selective inhibitor of native and mutated BCR / ABL is active against Philadelphia chromosome-positive (Ph +) leukemias harboring the T315I mutation, Leukemia. 2015, 29 (5), 1104-1114), is sufficient to inhibit the proliferation and cytotoxicity of Ph + tumor cells containing the native form Abl, as well as mutant forms, including the variant Abl T315I, resistant to the vast majority are known s Abl inhibitors. In other words, these results indicate that the treatment method, comprising administering the composition of the invention once a day and in doses of the active component of 200-600 mg, allows to achieve a high therapeutic effect and at the same time is characterized by high safety and a favorable dosage regimen.
Safety and efficacy study in patients with Ph + leukemia
In a clinical trial of a pharmaceutical composition of the invention in the form of hard gelatine capsules in patients with Ph + leukemia, the safety and efficacy of various doses of the drug were studied daily, orally once a day. To determine the maximum tolerated dose of the drug, the dose was studied at which the probability of developing dose-limiting toxicity during the first 28 days of treatment would be at least one third. Dose-limiting toxicity was defined as toxicity, characterized by one of the following features:
- нарушения функций печени 3-й (третьей) степени тяжести, продолжающиеся более 14 дней после отмены приема фармацевтической композиции по изобретению или нарушение функций печени 4-й (четвертой) степени тяжести;- impaired liver function of the 3rd (third) degree of severity, lasting more than 14 days after canceling the pharmaceutical composition according to the invention or impaired liver function of the 4th (fourth) degree of severity;
- тошнота, рвота, диарея, нарушения концентрации электролитов в крови 3-й (третьей) и более высокой степени тяжести, продолжающиеся более 3 дней, несмотря на адекватную поддерживающую терапию;- nausea, vomiting, diarrhea, disturbances in the concentration of electrolytes in the blood of the 3rd (third) and higher severity, lasting more than 3 days, despite adequate supportive therapy;
- другие негематологические нежелательные явления 3-й (третьей) степени тяжести и выше;- other non-hematological adverse events of the 3rd (third) degree of severity and higher;
- пропуск более 25% от назначенной дозы препарата в связи с развитием нежелательных явлений;- Skipping more than 25% of the prescribed dose of the drug due to the development of adverse events;
- развитие фебрильной нейтропении степени 3 или выше;- the development of febrile neutropenia of degree 3 or higher;
- для пациентов с хроническим миелолейкозом в хронической фазе нейтропения или тромбоцитопения 3-й (третьей) степени тяжести и выше, продолжающиеся не менее 2-х недель после отмены препарата (в случае, если первый день возникновения продолжающейся нейтропении или тромбоцитопении 3-й (третьей) степени тяжести и выше имел место во время первого цикла терапии);- for patients with chronic myelogenous leukemia in the chronic phase of neutropenia or thrombocytopenia of the 3rd (third) severity and above, lasting at least 2 weeks after drug withdrawal (in case the first day of continuing neutropenia or thrombocytopenia is 3rd (third ) severity and higher occurred during the first cycle of therapy);
- для пациентов с хроническим миелолейкозом в фазе акселерации нейтропения или тромбоцитопения 3-й (третьей) степени тяжести и выше, продолжающиеся не менее 2-х недель после отмены препарата при отсутствии лейкемии, определяемом, как наличие, по меньшей мере, полного цитогенетического ответа (в случае, если первый день возникновения продолжающейся нейтропении или тромбоцитопении 3-й (третьей) степени тяжести и выше имел место во время первого цикла терапии).- for patients with chronic myelogenous leukemia in the acceleration phase of neutropenia or thrombocytopenia of the 3rd (third) degree and above, lasting at least 2 weeks after drug withdrawal in the absence of leukemia, defined as the presence of at least a complete cytogenetic response ( in the event that the first day of continuing neutropenia or thrombocytopenia of the 3rd (third) degree of severity and above occurred during the first cycle of therapy).
When examining the drug at a dose of 300 mg or less, no dose-limiting toxicity was observed. In the study of doses of 400 and 500 mg, one case of dose-limiting toxicity was recorded - a skin rash of the 3rd degree (at a dose of 400 mg), which refers to non-hematological adverse events of the 3rd (third) degree of severity and higher. The likelihood of this toxicity was less than one third. A dose of 600 mg was the maximum tolerated dose; however, skin rash was a dose-limiting toxicity.
The effectiveness of the drug was evaluated by indicators of the achievement of a complete hematological response, cytogenetic response, molecular response. The drug has been shown to be effective in doses ranging from 200 mg to 600 mg. It was noted that the probability of manifestation of effectiveness depended on the dose of the drug, as well as on the phase of the disease (chronic phase of the disease, phase of acceleration or blast crisis), patient dedication (resistance or intolerance to previous lines of therapy), additional mutations in the BCR / ABL1 gene, and additional chromosomal aberrations (in addition to translocation t (9; 22)). Due to the large number of factors affecting the outcome of therapy, the following general conclusions were made regarding the dosage of the drug to treat a particular patient:
- пациентам с хроническим миелолейкозом в фазе акселерации или бластного криза, или с Ph+ острым лимфобластным лейкозом может быть рекомендовано лечение в максимально переносимой дозе 600 мг, или близкой дозе, например 500 мг, поскольку цель лечения – достичь в короткие сроки ремиссии для проведения трансплантации костного мозга;- for patients with chronic myeloid leukemia in the acceleration or blast crisis phase, or with Ph + acute lymphoblastic leukemia, treatment with a maximum tolerated dose of 600 mg or a close dose, for example 500 mg, may be recommended, since the goal of treatment is to achieve remission in a short time for bone transplantation brain
- пациентам с хроническим миелолейкозом в хронической фазе заболевания с резистентностью или непереносимостью по отношению к одной или нескольким предыдущим линиям терапии их заболевания может быть рекомендовано лечение в дозе 400 мг или 300 мг;- Patients with chronic myeloid leukemia in the chronic phase of the disease with resistance or intolerance to one or more previous lines of therapy for their disease may be recommended treatment at a dose of 400 mg or 300 mg;
- пациентам с хроническим миелолейкозом в хронической фазе заболевания, не получавшим предыдущей терапии или пациентам с резистентностью или непереносимостью к иматинибу, может быть рекомендовано лечение в дозе 200 мг.- Patients with chronic myelogenous leukemia in the chronic phase of the disease who have not received previous therapy or patients with resistance or intolerance to imatinib may be recommended treatment at a dose of 200 mg.
Taking a daily dose of the drug in the form of hard gelatin capsules 100 mg
The daily dose of the drug of the invention based on compound I in the range of 200-600 mg can be optimally carried out using the dosage described above in the form of hard gelatin capsules of 100 mg of the active ingredient. A dosage of 100 mg allows you to choose any of the above therapeutic doses of 200 mg, 300 mg, 400 mg, 500 mg or 600 mg. In this case, the patient takes from 2 to 6 capsules, which is convenient from a practical point of view, and also allows to minimize dosing errors. In the case of a higher dosage, it would be impossible to dial all of the listed therapeutic doses. In the case of a lower dosage, patients would have to take more capsules, which would be less convenient and would increase the likelihood of a dosage error.
