Description
The invention relates to medicine, namely to surgical dentistry and maxillofacial surgery, and can be used in the surgical treatment of high fractures of the branches of the lower jaw and replacement of defects of the mandibular bone, where there are indications for replantation of the head of the lower jaw.
The closest technical solution is a method for the surgical treatment of condyle of the condyle of the lower jaw with a complete medial dislocation of the head of the lower jaw, in which a vertical (L-shaped) osteotomy of the branch of the lower jaw from the middle of the notch to the horizontal osteotomy, which begins from the posterior edge of the branch at 2, 5-3 cm below the notch, milling holes are applied in the branch area, the osteotomized free fragment of the lower jaw branch is removed from the wound, the head is isolated and removed under visual control mandible operate metalosteosynthesis osteotomised fragment and the head of the mandibular brackets TiNi, replantiruyut, in the fixed branch condylar process of NiTi staples on the previously chosen burr hole.
A61B 17/24, 31.05.2010.Application No. 201021975/14, IPC A61B 17/24, 05/31/2010.
The disadvantages of this method are the occurrence of facial deformities and temporomandibular dysfunctions caused by partial resorption of the replant and cicatricial changes in the surrounding tissues, as well as functional disorders of the lateral pterygoid muscle.
The objective of the invention is to increase the efficiency of replantation of the head of the lower jaw in patients with traumatic injuries of the condylar process, tumor and tumor-like conditions of the lower jaw.
This object is achieved in that in the proposed method for replanting the head of the lower jaw, including performing a vertical osteotomy of the branch of the lower jaw from the middle of the notch, milling holes are performed on the branches before removing the osteotomized fragment, under visual control, extraction, removal of the head of the lower jaw from the wound, osteotomy metal osteosynthesis of the fragment and head of the lower jaw with a bracket made of titanium nickelide with shape memory followed by replantation of the condylar process with restoration An increase in its height and fixing with titanium nickelide brackets with shape memory along the designated milling holes, a new one is that a superelastic mesh braided nickelide-titanium implant with a diameter of 10-20 mm, 10-25 mm long with a mesh size is fixed on the lateral pterygoid muscle stump 1-3 mm from a thread with a thickness of 50-60 μm, the mandibular head is covered with a mesh implant fixed with a superelastic nickelide-titanium thread 50-60 μm thick around the base, the stump of the lateral pterygoid muscle is fixed to the neck of the lower jaw nickel alloy, titanium weld.
A mesh nickel-titanium implant in the implantation of the head of the lower jaw, fixed around the base of the condylar process, makes it possible to avoid functional disorders of the temporomandibular joints in the postoperative period even after partial resorption of the replanted head. Osteosynthesis with devices made of titanium nickelide ensures stable fixation of the implant to the main mandibular fragment or endoprosthesis with the implementation of early chewing load. Due to the correspondence of the hysteretic behavior of the implantation material and body tissues, connective tissues from the areas surrounding the implant grow through the cellular structure of the implant without causing aggressive reactions, and the formed single regenerate due to early functional load does not impede the movements of the lower jaw. A super-elastic thin-profile mesh implant fixed to the stump of the lateral pterygoid muscle contributes to the strength of fixation of the latter to a similar implant fixed to the replanted head, does not interfere with the contractility of the muscle and its return to its original position during the functioning of the masticatory apparatus.
The proposed method is as follows.
