Abstract
Description
The invention relates to medicine, in particular to surgical suture material, and can be used in cardiovascular surgery for the prevention of arterial thrombosis in the zone of anastomosis after reconstructive surgery on the vessels.
Despite the use of suture material in medicine for several millennia, until the middle of the 20th century, the problem of threads did not cause much interest to surgeons. However, with the expansion of the possibilities of surgery, more and more attention was paid to the problem of suture material. The quality of the suture material, its structure, composition, physico-chemical characteristics make it possible to achieve quick healing of the wound, to provide a good cosmetic effect, and also to prevent the development of complications, such as seam divergence and infection.
In angiosurgery, restoration of the integrity of the vascular endothelial region in the suture area and the presence of a thread protruding into the vessel lumen turn the anastomosis zone into a thrombus formation site, which leads to vascular stenosis and dysfunction, and also increases the risk of thromboembolism.
One of the existing methods for imparting necessary properties to suture surgical materials is coating them with shells containing biologically active substances. At the same time, the components used should be fairly well fixed on the basis of the thread, not peeling off during operation, not change their properties during sterilization, and also not affect the elasticity and handling characteristics of the suture material.
п. (Tiwari A., Cheng K.S., Salacinskiet Н. et al. // Eur. J. Vase. Endovasc. Surg. - 2003. Vol.25. - №4. - P.287-295). - 2008. - №3. - С.31-38; Wagner Е., Guidoin R., Marois М. et al. // ASAIO J. - 1994. - Vol.40. - №3. - P.279-283).To date, biologically active suture materials are mainly represented by antimicrobial threads. Antimicrobial components are antibiotics - gentamicin, clindamycin, doxycycline and metronidazole, antiseptics - dioxidine, miramistin, as well as some silver compounds, etc. (Tiwari A., Cheng K.S., Salacinskiet, H. et al. // Eur. J. Vase. Endovasc. Surg. - 2003. Vol.25. - No. 4. - P.287-295). At the same time, polypropylene yarns, polyvinylidene fluoride monofilaments, lavsan woven yarns with a fluoropolymer coating and absorbable woven yarns based on polyglycolic acid (V.A. Zhukovsky. "Problems and prospects for the development of the production of surgical suture materials" can be objects of modification). Chemical fibers. - 2008. - No. 3. - P.31-38; Wagner E., Guidoin R., Marois M. et al. // ASAIO J. - 1994. - Vol.40. - No. 3. - P.279-283).
Known biocompatible suture material consisting of polyamide yarn PA-6, which is coated with a shell of chitosan with a thickness of 5-30 microns, and polyvinyl alcohol is used as an intermediate crosslinking material, which is applied with a thickness of 5-50 microns (RF Patent 2073528, application 08.12. 1993, published on 02.20.1997, IPC A61L 17/00). The disadvantage of suture material is that chitosan exhibits cytotoxic properties, which leads to aseptic inflammation, enhanced proliferation of fibroblasts and proliferation of connective tissue in the anastomotic zone, and as a result to stenosis and vessel occlusion.
A61L 17/12).Known biodegradable suture surgical material with a coating, where the core of the thread is a polyamide twisted or braided thread, and the film coating is a shell of a biodegradable polymer, which is polylactic acid containing the drug - furagin in ratios, wt.%: Polyamide thread 62,23-99 , 7; polylactic acid 0.20-30.52; furagin 0.20-7.25 (RF patent 115663, declared 01.11.11, publ. 10.05.12, IPC A61L 17/12).
The disadvantage of this suture material is that multifilament filaments are used as the basis, and the polylactic acid of the biodegradable membrane acidifies surrounding tissues during degradation and leads to the development of an inflammatory reaction in the anastomosis zone. In addition, the use of biodegradable filaments is not acceptable for vascular surgery, since the timing of the degradation of the filament does not allow the formation of a wealthy scar.
Known biologically active polymer composition intended for coating medical devices, consisting of a copolymer of 3-hydroxybutyrate and 3-hydroxyvalerate and at least one biologically active substance from the group of antibiotics or non-steroidal anti-inflammatory drugs (RF Patent No. 2447902, IPC A61L 31/08 , A61L 31/10, A61L 31/16, claimed 21.07.10, published on 04.20.2012). Additionally, surfactants, polyethylene glycol or gelatin may be included in the polymer composition. The disadvantage of this solution is that the copolymer used and the biologically active substance have a different nature of origin and cannot be combined into a single composition with one solvent, as a result of which the biologically active substance can be inactivated or washed out by the natural environment of the body without providing the necessary therapeutic effect.
Closest to the claimed suture material is a filament coated with a poly (β-hydroxybutyrate-co-β-hydroxyvalerate) copolyester (PHBV) and a biologically active substance from the group of naphthazarin and / or naphthazarin derivatives dissolved in one solvent (RF patent No. 2308295, MKP A61L 33/04, A61L 33/06, A61F 2/06, filed July 19, 2003, published on October 20, 2007). The antithrombotic effect of the known composition is based on the suppression of platelet and fibroblast aggregation by the naphthazarin derivative included in the composition. The disadvantage of this invention is that naphthazarin and its derivatives are a substance for drugs mainly anti-inflammatory, antimicrobial, anti-burn (wound healing) effects, in addition to these drugs have a pronounced effect of enhancing the proliferation of granulation tissue. Thus, along with the antithrombotic effect, naphthazarin can provoke the growth of neointima in the zone of vascular anastomosis, leading to stenosis and subsequent thrombosis.
The technical result of the invention is to increase the hemo- and biocompatibility of surgical suture material, as well as to increase its antithrombotic characteristics due to the multilayer modification of suture material with a biodegradable polymer and heparin.
Non-absorbable sutures based on polypropylene (polyolefin), which is the most inert to human tissues and is a monofilament atraumatic suture, are used as the basis for modification. This suture material is modified layer by layer with a solution of a copolymer of 3-hydroxybutyrate / 3-hydroxyvalera and heparin deposited by a chemical reaction.
The presence of heparin in the coating composition will add an antithrombotic effect to the suture material, since heparin is a physiological anticoagulant and slows down the formation of fibrin, while heparin is released into the blood as the biopolymer biodegrades and provides a long therapeutic effect.
The invention is illustrated by drawings, where figure 1 shows an unmodified suture material represented by a monofilament thread, which determines the linear linear roughness on the surface of the thread. Figure 2 shows a modified heparin suture material with a smooth, even surface of the thread. Figure 3 - suture material, layer-by-layer modified with PHBV and heparin solution, while the surface of the suture material changed its structure (became more porous), but the integrity and uniformity of the coating remained the same.
30-35°C.Antithrombotic suture material for cardiovascular surgery is prepared as follows: a sample of the PHBV copolymer powder is dissolved in chloroform to a concentration of 1-6%, thoroughly mixing for 2 hours on a magnetic stirrer while heating the solution to 30-35 ° C.
Промышленно выпускаемую полипропиленовую мононить, для удаления замасливателей, выдерживают в течение 5 минут в смеси этилового спирта и безводного серного эфира (смесь Никифорова).The industrially produced polypropylene monofilament, to remove lubricants, is kept for 5 minutes in a mixture of ethyl alcohol and anhydrous sulfuric ether (Nikiforov mixture).
After that, the thread is placed in a modifying solution of PHBV for 10 minutes, followed by drying in a dust-free box for 3 hours at room temperature. Then the thread is kept in a stream of a mixture of air and ozone for 1 hour at room temperature. Then the suture material is left in methacryloyl chloride vapors for 5 hours at a temperature of 85-90 ° C with successive immersion in a heparin solution (1000 PIECES / ml) at a temperature of 2-5 ° C for 10 hours and for 14 hours in a heparin solution (1000 PIECES ) room temperature. Final drying is carried out in a dust-free box at room temperature until the material has completely dried.
The proposed modification allows you to firmly fix two substances of different nature to each other without destroying and leaching the active substance while maintaining a prolonged therapeutic effect. The surgical suture material modified in this way acquires satisfactory bio- and hemocompatible properties, does not adversely affect blood components, and prevents the formation of blood clots in the zone of vascular anastomosis.
Так, после 40 минут контакта с кровью суммарное количество протеинов на образцах, прошитых модифицированным шовным материалом, достоверно меньше, чем на образцах, прошитых немодифицированной нитью, что свидетельствует о большей гемосовместимости модифицированного шовного материала (Табл.1).The study of the modified suture material for hemocompatibility showed that in the zone of vascular anastamosis there were no echinocytes, a significantly smaller amount of adsorbed formed blood elements was observed. In addition, after contact with blood in the area of anastomoses made with modified and unmodified suture material, differences in protein sorption were revealed. So, after 40 minutes of contact with blood, the total amount of proteins on samples stitched with modified suture material is significantly less than on samples stitched with unmodified thread, which indicates a greater hemocompatibility of the modified suture material (Table 1).
The results of the identification of adsorbed proteins indicate that albumin is adsorbed by 32.42% less, fibrinogen by 39.66%, immunoglobulin by 21.13% and D-Dimer by 69.23% in the zone of anastomoses made with a modified thread. An anastomosis made with a modified thread is covered with a more uniform protein layer, which is an important point in the adaptation of the anastomotic zone in the vascular blood flow.
The uniformity and integrity of the applied coating before and after the surgical procedures were evaluated using scanning electron microscopy.
After modification, a coating with a thickness of 3.9-8.5 microns was visualized on the surface of the thread. The integrity of the coating was maintained after the manipulations.
Thus, the layered application of a solution of PHBV and heparin on the surface of the suture material allows you to get a uniform and durable coating with pronounced antithrombotic properties.
