Abstract
Description
[1].The invention relates to biotechnology, the medical and food industries and can be used in the production of bacterial preparations and functional food products intended for the correction of vaginal microflora in women and girls over 10 years old. According to statistics, one billion people in the world annually go to clinics about the treatment of various urogenital infections. Given such a high prevalence of these infections in women and an increase in the number of pathogens resistant to various drugs, the problem of probiotic therapy in clinical gynecology and urology can be considered super-relevant [1].
Currently, the main method for the correction of impaired vaginal microbiocenosis of women is the use of probiotic preparations. In Russia, the production of such drugs is based on the use of lactobacilli and bifidobacteria - representatives of the normoflora of the human gastrointestinal tract and the use of individual strains of vaginal lactobacilli without taking into account the structural features and functions of the female reproductive tract microbiocenosis.
The vaginal microbiocenosis changes dynamically with age and depending on the cyclic processes occurring in the female body. A feature of this microbiocenosis is its multicomponent nature. The structure of the vaginal microbiocenosis in women of reproductive age is characterized by a combination of resident and transient microflora. Resident microflora is divided into obligate and optional. Obligatory resident microorganisms of the vaginal microbiocenosis include lactobacilli. They predominate in the vagina of a healthy woman, while the dominance of uropathogens occurs in patients with a uroinfectious history. Using only cultural methods to study the species spectrum of lactobacilli in healthy women of reproductive age allows one, two, and less often three strains to be found in the vaginal biotope of one woman. The combination of cultural and other alternative methods of identification allows us to detect more than three types of lactobacilli in the vaginal epitope of one woman. Gardnerella vaginalis and fungi of the genus Candida are optional resident flora. These microorganisms in low concentrations are often excreted from a healthy female genital tract. The most severe effect on vaginal microbiocenosis is exerted by hormonal changes that determine the reproductive phases of a woman's life [2]. The action of hormones on vaginal microbiocenosis occurs indirectly, primarily through the enzyme lactate dehydrogenase (LDH), which catalyzes the conversion of the product of bacterial metabolism of lactic acid into pyruvic acid, which is a substrate for gluconeogenesis and glycogen synthesis. An increase in LDH activity leads to increased proliferation of the vaginal epithelium and the accumulation of glycogen, which ensures optimal conditions for the existence of normal microbiocenosis. An integral condition of these processes is the acidification of the vaginal environment due to the formation of lactic acid as a result of bacterial glycogen utilization. This determines the formation of a certain spectrum of bacteria that can exist in a healthy vagina and make up a normal microbiocenosis. The slightest change in the pH of the mucous membrane leads to increased vulnerability of the vagina to pathogenic microorganisms and increased pathogenicity of the conditionally pathogenic flora.
At the birth of the girl and during the first 1-2 months of life, maternal hormones continue to act on her body, therefore, the vaginal epithelium of newborns corresponds to that of women of childbearing age, and microbiocenosis normally is represented mainly by lactobacilli. In this period, the child develops immunological tolerance. The lactobacilli strains received from the mother during childbirth and populated the girl’s vaginal biotope are perceived by her immune system as “her own”. Such strains of bacteria are called auto-strains. After the final breakdown of the hormones of the mother (towards the end of the second month of life) and before puberty, the girl has a pronounced atrophy of the vaginal epithelium, which is represented by a monolayer of basal cells and is characterized by an alkaline environment of the mucous membrane. The microflora of such a vagina consists of a meager amount of gram-positive cocci, mainly anaerobes. After puberty, the epithelium becomes multilayered, the vaginal environment is acidified, the microbiocenosis corresponds to the microbial composition of the vaginal mucosa of women of childbearing age. In menopause, reverse processes gradually occur: atrophic changes in the epithelium develop, there is an increase in the pH of the vaginal environment, a decrease in the number of lactobacilli, an increase in the population of gram-positive cocci.
H2O2.The absence of lactobacilli in the microbiota of the vagina of women of reproductive age is a serious harbinger of premature birth. Changes in the vaginal microflora of women of reproductive age occur constantly, but in a healthy body, normal microbiocenosis is quickly restored. Dyshormonal and immunological disorders naturally affect the quantitative and qualitative composition of the vaginal microbiocenosis, and medical correction is often required. Oral use of antibiotics leads to the development in women of not only dysbiosis of the gastrointestinal tract (GIT), but also a violation of the microecology of the vaginal tract, which indicates serious damage to the microbiocenosis of the whole organism. Due to the fact that lactobacilli are an important factor in the prevention of vaginal colonization of women of reproductive age by uropathogenic bacteria, a decrease in their number or absence may predispose to urinary tract infections. If previously an infectious pathology in obstetrics and gynecology was due to a monomicrobial pathogen, now in clinical practice a polymicrobial association of microorganisms is increasingly found. Among these diseases is bacterial vaginosis (BV), an infectious non-inflammatory syndrome characterized by a sharp decrease or absence of lactobacilli and their replacement with polymicrobial associations of opportunistic bacteria G. vaginalis with strict anaerobes Atopobiumvaginae, Prevotella, Mobilincus, etc., the concentration of which increases several times and reaches 10 9 -10 11 CFU / ml [3]. The second most common infectious disease of the female genital area is candidal vulvovaginitis (CVV). It refers to opportunistic infections developing against the background of immunological deficiency. In this regard, CVE is very relevant during pregnancy, is characterized by a recurrent and therapy-resistant course. Risk factors for candida infection also include ovarian hypofunction, frequent antibiotic treatment, especially the penicillin series. In recent years, more and more often, in humans, CVE is caused not by Candida albicans, but by other varieties of fungi of the genus Candida (C. tropicalis, C. glabrata, C. krusei). If earlier such cases were rare, now these fungi account for 30-50% of all candida infections. Currently, BV and CVB are quite common at the same time as a mixed infection, accounting for 10 to 39% of all cases of infectious diseases of the vagina [4]. In the treatment of patients with mixed infection, a two-stage scheme is used - elimination of the pathogen and maintenance anti-relapse therapy, which is carried out for several months. But there are patients who require constant (lifelong) maintenance therapy, since when the antimycotics are canceled, they certainly have a relapse of candidal inflammation, and this problem has not been solved today. It has been established that the absence of strains of lactobacilli producing H 2 O 2 in the vaginal biotope of a woman significantly increases the risk of developing BV and KB B with a chronic relapsing course [5]. In connection with the foregoing, the creation of new probiotic preparations, which will include a consortium consisting of various types of lactobacilli producing H 2 O 2, is relevant.
[7].The key to solving the BV problem is the normalization of vaginal microflora. At the first stage, vaginal sanitation is performed using systemic and local antibacterial drugs (metronidazole, clindamycin), the purpose of which is to eliminate G. vaginalis and strict anaerobic opportunistic bacteria. Despite the high effectiveness of this therapy in 30-40% of cases, the development of relapse of BV is noted and in 20% of cases the disease is complicated by the development of CVI. At the second stage, local probiotic preparations are used to restore normal microflora. However, the probiotic preparations existing in Russia (Acylact, Lactobacterin, etc.) contain lactobacilli and bifidobacteria of intestinal origin or mono-preparations based on individual vaginal lactobacilli that are not able to effectively take root in the vagina due to low adhesive properties with respect to vaginal epithelial cells [6] . As a result, treatment leads to an unstable effect. In contrast to the aforementioned domestic probiotics, the foreign drug Vagilak (Lactogin) is the world's first combination of two probiotic oral strains to normalize vaginal microflora and reduce vaginal colonization by pathogenic and conditionally pathogenic bacteria. The drug is a consortium of Lactobacillus rhamnosus GR-1 and Lactobacillus reuteri RC-14, isolated from the distal urethra and vagina of healthy women. Both strains have adhesive properties in relation to vaginal epithelial cells [7].
[10].Studies have shown that Vagilac can be taken orally, daily, for two months, and no side effects are noted [8, 9]. Acceptance of this drug by women suffering from BV leads to the restoration of the vaginal flora in 82% of cases [10].
[15].The feasibility of oral administration of probiotic lactobacilli for the correction of vaginal microbiocenosis is explained by the fact that the large intestine is a reservoir from which the microorganisms of the human microbiota, including lactobacilli, are constantly released. Owing to the anatomical proximity of the anus and the vestibule of the vagina, lactobacilli of the corresponding species spectrum, the most important representatives of which are Lactobacillus crispatus, Lactobacillus gasseri, Lactobacillus jensenii, Lactobacillus rhamnosus, Lactobacillus plantarum, Lactobacillus erythroderma, and others, penetrally and completely penetrate. eleven]. The first three species (Lactobacillus crispatus, Lactobacillus gasseri, Lactobacillus jensenii), producing H 2 O 2 , are the dominant representatives and are detected in approximately 90% of cases in healthy women by means of seeding the vaginal contents on nutrient media used in the cultivation of lactobacilli [12, 13] . Other species, for example, Lactobacillus rhamnosus and Lactobacillus reuteri, are found in 1.5% and 0.3% of cases [14]. The normal amount of lactobacilli is 10 5 -10 7 CFU / ml, which corresponds to 95-98% of the entire microflora of the vagina [15].
A known strain of Lactobacillus plantarum 8P-A3, which is part of the drug Lactobacterin, which has indications for use in diseases of the female genital area intravaginally. However, according to [6], the drug is ineffective in the treatment of BV. A known strain of Lactobacillus crispatus CTV-05, producing hydrogen peroxide, which either in intravaginal capsules or suppositories [16, 17] is proposed for use to correct vaginal microflora. The disadvantage of the strain of Lactobacillus crispatus CTV-05 is the restrictions on its use in oral administration, or as part of fermented milk functional food products.
The closest technical solution (prototype) is the drug Vagilac, represented by a consortium of strains of lactobacilli Lactobacillus rhamnosus GR-1 and Lactobacillus reuteri RC-14 [18]. It was shown that strains of Lactobacillus rhamnosus GR-1 and Lactobacillus reuteri RC-14 colonize the vagina after oral administration [7, 10], contribute to the restoration of the natural acidic environment in the vagina (pH 3.8-4.5), increase the resistance of the mucosa to exposure to pathogenic microorganisms, creating conditions for improving the quality (normalization) of microflora.
The objective of the present invention is to expand the assortment of consortia of lactobacilli by obtaining a consortium that can be used to manufacture a bacterial preparation, biologically active additives (BAA), as well as direct starter culture for the fermentation of milk and to obtain fermented milk functional food products aimed at preserving and strengthening urogenital women's health, prevention and treatment of urogenital infectious diseases.

The study of the resistance of the strains of the consortium to gastric and intestinal stress (the ability to grow in an environment with a low pH or in an environment containing bile).
The early stages of BV and CVB can occur without the manifestation of specific symptoms. Oral administration of probiotic drugs makes it possible to prevent dysbiosis and symptomatic development of BV and CVB. In this regard, it is necessary that the lactobacilli strains included in the consortium are resistant to gastric and intestinal stress.

Hela.Example 8. The study of adhesion of the lactobacilli that are part of the consortium to transplantable Hela cells.
[25].The adhesion of probiotic bacteria is considered as a positive sign, which allows to protect the macroorganism from pathogenic and conditionally pathogenic microorganisms. Probiotic lactobacilli, intended for the prevention and treatment of urogenital infections, must have adhesion to the epithelium of the urogenital tract [25].

Hela.Hela cell cultures were grown in DMEM medium with 10% fetal serum + 1% penicillin-streptomycin + 0.2 mM Hepes + 2 mM L-glutamine at 37 ° C in a CO 2 incubator. Cells at a concentration of 3.0 × 10 5 l / ml were plated on 6-well plates in a volume of 2.0 ml per well and cultured for 24 hours. Upon reaching a 70-80% monolayer, the cells are washed 3 times with serum-free DMEM and then DMEM with L-glutamine is added to them. Before bacteria were added to Hela cells (18 hour culture on MPC medium), they were washed twice with PBS pH 7.2 using centrifugation at 5000 rpm for 15 minutes and resuspended in DMEM with L-glutamine. The concentration of bacteria to determine the ratio of bacterium / somatic cell is regulated by optical density (ODλ = 590 nm) and is controlled by seeding on the corresponding solid medium. Multiplicity of infection (MOI) is 50 and 100 bacteria / cell. Hela cells with lactobacilli added to them are incubated for another 5 hours at 37 ° C in a CO 2 incubator. After incubation, the cells are washed 3 times with PBS pH 7.2, then fixed in cold ethanol (3 min) and stained with Romanovsky-Giemsa stain for 30 minutes. Next, the plates are washed with distilled water, dried at room temperature and examined microscopically. The number of Hela cells with attached bacteria (percent adhesion) and the number of bacteria attached to one cell (adhesion index) are counted. As can be seen from table 4, all strains of the consortium have a pronounced adhesion to Hela cells.
Example 9. The study of adhesion of lactobacilli, which are part of the consortium, to the cells of the buccal epithelium.
The adhesion of the lactobacilli that make up the consortium to buccal epithelial cells is studied according to the method described in [26]. The multiplicity of infection was 100 bacteria per cell. Buccal epithelial cells with lactobacilli introduced into them were incubated for 5 hours at 37 ° C in a CO 2 incubator in an atmosphere containing 5% CO 2 . As can be seen from table 5, all strains of the consortium have a pronounced adhesion to buccal epithelial cells.
Given the ability of the lactobacilli of the consortium to adhere to buccal epithelial cells, it is advisable to include the consortium in the composition of chewing gum. Colonization of the oral cavity with strains of the lactobacilli of the consortium can ensure their prolonged entry into the gastrointestinal and reproductive tracts.
Example 10. The study of the antagonistic activity of strains of Lactobacillus crispatus 2029, Lactobacillus gasseri 2016, Lactobacillus plantarum 2025 and a consortium of strains.
** - резистентность к метициллину.* - clinical isolates of urogenital and intestinal infections;
** - methicillin resistance.
The study of antagonistic activity is carried out by the method of two-layer agar [27] on a wide range of indicator cultures.
Антагонистическую активность исследуемых культур в отношении патогенных и условно-патогенных бактерий (табл.6) определяют по зоне задержки роста индикаторных штаммов вокруг колоний отдельных штаммов лактобацилл и консорциума (в мм).The antagonistic activity of the studied cultures against pathogenic and opportunistic bacteria (Table 6) is determined by the zone of growth inhibition of indicator strains around the colonies of individual strains of lactobacilli and consortium (in mm).
7.Antagonistic activity against fungi of the genus Candida (Table 7) is expressed as the minimum inhibitory concentration of the antagonist (MICA) - this is the minimum amount of antagonist that inhibits the reproduction of indicator culture. Indicators for individual strains of Lactobacillus crispalus 2029, Laclobacillus gasseri 2016, Laclobacillus plantarum 2025 and a consortium of strains are presented in tables 6 and 7.
Lactobacillus plantarum 2025.The research results indicate that the inventive consortium of strains exhibits a more pronounced antagonistic activity than individual strains of Lactobacillus crispatus 2029, Lactobacillus gasseri 2016 and Lactobacillus plantarum 2025.
Example 11. Preparation of a functional milk fermented milk product to maintain and improve the urogenital health of women of reproductive age using a bacterial preparation based on a consortium as a direct starter culture.
Direct starter culture (the consortium bacterial preparation obtained in Example 4) in a volume of 5% is added to sterile milk, mixed and cultured at 37 ° C for 18-24 hours until a clot forms, after which the product is cooled to 4 ° C and packaged . The result is a fermented milk functional food product with a beneficial microflora content of at least 10 8 CFU / ml.
Example 12. The use of fermented milk functional food product or dietary supplement for the treatment and prevention of candidiasis (thrush) in women of reproductive age.
Nine women volunteers aged 27-37 years with a diagnosis of candidiasis (thrush), confirmed by laboratory analysis, were divided into three groups, three in each group. The first group drank 100 ml of fermented milk product obtained in Example 11 during dinner for 14 days daily. The second group drank 100 ml of ready-made kefir daily for 14 days with the addition of a 5% bacterial preparation of the consortium obtained in Example 4. The third group (control) drank 100 ml of kefir, not enriched with the drug of the consortium, every day for 14 days during dinner. After 14 days, the symptoms of candidiasis (thrush) disappeared in women of the first and second groups. The level of C. albicans in the vaginal discharge of the first and second groups decreased from 10 5 CFU / ml to 10 2 CFU / ml, which is normal. Side effects from taking a fermented milk functional food product or dietary supplement were absent. In the third group, the symptoms of candidiasis persisted.
The research results show the promise of using a preparation of a consortium of probiotic strains Lactobacillus crispatus 2029, Lactobacillus gasseri 2016 and Lactobacillus plantarum 2025 for the production of fermented milk functional foods or dietary supplements for the prevention and treatment of candidiasis. The method is simple, economical, affordable and can be effectively applied at home.
