Фортелизин® - это инновационный отечественный фибринселективный тромболитический лекарственный препарат, разработанный фармацевтической компанией «СупраГен» для лечения больных с острым инфарктом миокарда с подъемом сегмента ST [2]. Фортелизин® - это рекомбинантный белок, полученный по генно-инженерной технологии Е. coli и содержащий аминокислотную последовательность неиммуногенной стафилокиназы. В отличие от нативной стафилокиназы в молекуле Фортелизина® были заменены 3 аминокислоты в иммунодоминантном эпитопе, что привело к отсутствию нейтрализующих антистафилокиназных антител при однократном введении и минимальному их образованию при повторном введении на 45-е сутки наблюдения (титры антител: 100 - Фортелизин®, 12000 - стафилокиназа).Fortelizin® is an innovative domestic fibrin-selective thrombolytic drug developed by the SupraGen pharmaceutical company for the treatment of patients with acute myocardial infarction with ST segment elevation [2]. Режим приема препарата был подобран на основании проведенного клинического исследования. Это исследование было проведено в соответствии с разрешением Комитета по Этике при Федеральной службе по надзору в сфере здравоохранения и социального развития и Росздравнадзора №196 от 28.04.2010 г. и в 6-ти клинических центрах по протоколу «Открытое проспективное исследование переносимости и эффективности препарата Фортелизин vs Актилизе у больных с острым инфарктом миокарда (III фаза)» с целью сравнения эффективности и безопасности Фортелизина® и Актилизе® у пациентов с острым инфарктом миокарда с подъемом сегмента ST.The regimen of the drug was selected on the basis of a clinical study. В исследование включено 54 пациента, которые были рандомизированы в соотношении 3:1 на 2 группы: 1-я группа, пациентам которой вводился Фортелизин® (n=41), и 2-я группа, пациентам которой вводилась Актилизе® (n=13). Пациенты 1-й группы, в свою очередь, были рандомизированы в соотношении 1:1 на 2 подгруппы: пациентам первой подгруппы (n=20) Фортелизин® вводился двумя болюсами: 10 мг (1490000 ME) и через 30 минут оставшиеся 5 мг (745000 ME); пациентам второй подгруппы (n=21) Фортелизин® в дозе 10 мг (1490000 ME) вводился болюсом, затем оставшиеся 5 мг (745000 ME) вводились инфузией в течение 30 мин. Актилизе® вводилась согласно инструкции по применению: 15 мг - в/в струйно, 50 мг - в/в инфузия в течение первых 30 минут с последующей инфузией 35 мг в течение 60 минут до достижения максимальной дозы - 100 мг. According to the main clinical and anamnestic characteristics, patients in the groups did not differ.
Частота реперфузии коронарной артерии по ЭКГ-критериям между группами Фортелизина® и Актилизе® не различалась и составила по 85%. Частота реперфузии коронарной артерии в подгруппах Фортелизина® - двойной болюс и болюс + инфузия - также не различалась и составила 80% и 90% соответственно. По данным экстренной коронарной ангиографии, выполненной в течение 180 минут от начала тромболитической терапии у всех больных, частота реперфузии коронарной артерии (по критериям группы TIMI степени реперфузии 2+3) наблюдалась в группе Фортелизина® у 34 больных, т.е. в 83% случаев, в группе Актилизе® - у 10 больных, т.е. в 77% случаев различие статистически незначимо. Необходимо отметить более высокую степень восстановления кровотока (3 степень по TIMI) в группе Фортелизина® по сравнению с группой Актилизе®: 54% против 31% (p=0,03). Частота восстановления коронарного кровотока (TIMI 2+3) в подгруппах Фортелизина® - двойной болюс и болюс + инфузия - не различалась: 80% против 86%.According to ECG criteria, the frequency of coronary artery reperfusion between the Fortelizin® and Actilize® groups did not differ and amounted to 85%. The frequency of restoration of coronary blood flow (TIMI 2 + 3) in the Fortelizin® subgroups - double bolus and bolus + infusion - did not differ: 80% versus 86%.
Летальных исходов, геморрагических инсультов в группах не наблюдалось, также не отмечено аллергических реакций, тошноты, рвоты после введения препаратов. Больной П. 61 г. поступил в отделение неотложной кардиологии НИИ кардиологии СО РАМН (г.Томск) 06.11.2010 в 14:10 с диагнозом: ИБС: Острый повторный передний инфаркт миокарда с подъемом сегмента ST от 06.11.10.Patient P., 61, was admitted to the emergency cardiology department of the Research Institute of Cardiology SB RAMS (Tomsk) on 06/06/2010 at 14:10 with a diagnosis of IHD: Acute repeated anterior myocardial infarction with ST segment elevation from 11/6/10.
Сопутствующие болезни: гипертоническая болезнь 2 стадии, 3 степени, риск 4. Хронический бронхит вне обострения.Concomitant diseases: hypertension 2 stages, 3 degrees, risk 4. Chronic bronchitis without exacerbation.
Больной поступил через 3 часа 10 мин от начала ангинозного приступа с сохраняющимся болевым синдромом и подъемом сегмента ST в отведениях V2-V4 до 3 мм. После подписания информированного согласия больному проведена тромболитическая терапия: в/в болюсом введено 10 мг Фортелизина® (в 14:30), затем через 30 минут (в 15:00) - второй болюс 5 мг. При регистрации ЭКГ в 16:00 (через 90 минут от начала тромболизиса) зафиксировано возвращение сегмента ST на изолинию и формирование отрицательных зубцов «Т» в этих отведениях, т.е. получены косвенные признаки реперфузии коронарной артерии. Через 3 часа после начала тромболизиса проведена коронарная ангиография, на которой подтверждена реперфузия передней нисходящей артерии с субтотальным остаточным стенозом и пристеночным тромбозом. Сразу же проведена баллонная ангиопластика и стентирование остаточного стеноза. В дальнейшем заболевание протекало без осложнений, и на 13 сутки от начала заболевания больной был выписан на амбулаторное лечение.The patient was admitted after 3 hours 10 minutes from the onset of a sore throat with persistent pain and ST segment elevation in leads V2-V4 to 3 mm. When registering the ECG at 16:00 (90 minutes from the beginning of thrombolysis), the ST segment returned to the isoline and the formation of negative T waves in these leads, i.e. indirect signs of coronary artery reperfusion were obtained. 3 hours after the start of thrombolysis, coronary angiography was performed, which confirmed reperfusion of the anterior descending artery with subtotal residual stenosis and parietal thrombosis. Immediately performed balloon angioplasty and stenting of residual stenosis. Subsequently, the disease proceeded without complications, and on the 13th day from the onset of the disease the patient was discharged for outpatient treatment.
Предлагаемый способ применен у 20 больных. Фортелизин® при заявляемом режиме назначения обладает тромболитическим действием, которое обеспечивает восстановление антеградного коронарного кровотока в инфарктсвязанной коронарной артерии у больных инфарктом миокарда с эффективностью, не уступающей альтеплазе.The proposed method was applied in 20 patients.
