Abstract
Description
The invention relates to medicine.
The problem of treating an ingrown toenail, including in children, is still relevant. Known methods of treatment give a significant percentage of relapses (20-70%) (Kassirov D.A., 2008) and complications (17%) (Larin V.F., 1976), and the lack of a common point of view on the optimal method of surgical intervention makes it necessary further developing this problem.
Surgical interventions associated with excision of the edge of the nail along with the nail bed (Schmiden's operation and the like), in all cases, change the structure of the groove of the nail bed, where a scar is formed instead of normal tissue. It always differs in form and structure from the physiological nail roll and only the experience of the surgeon determines how successful the final result will be and whether a relapse will occur.
An alternative to such interventions is operations such as, for example, the method of treating an ingrown nail according to SU 1042735, 1981. When treating with this method, a partial wedge-shaped excision of the ingrown edge of the nail plate is performed, and then a wedge-shaped excision is performed on healthy skin with part of the subcutaneous tissue. The skin defect is closed with sutures and the inflamed nail roller or part of it is removed towards a healthy area.
The method SU 1042735, 1981, aims at eliminating the formation of keloid scars at the site of excision of soft tissues and using skin plastics to “straighten” the roller so that it does not cover the nail plate. The likelihood of a subsequent relapse is the lower, the more you can move the skin flap of the okonogelovy roller, which depends on the size and shape of the skin fragment resected on the lateral surface of the finger. However, if such an intervention is performed on inflamed tissues, an increase in the volume of resection, in turn, increases the risk of eruption of sutures and suppuration of the postoperative wound, which worsens the result of the operation.
The lack of special tools that take into account the specifics of the treatment of ingrown toenails is another problem that significantly complicates the surgical intervention and its results.
As a rule, a standard scalpel or scissors are used to dissect the nail plate, which injure the underlying tissues.
Closer to the proposed instrument can be considered a surgical knife, known from EP 0157172, 09.10.1985, containing a handle and a removable blade mounted with the ability to move along the guide groove of the regulator of the depth of excision of tissues, which has an end surface for resting on the fabric.
The well-known surgical knife allows you to dissect the tissue to a predetermined depth, but the lack of a means of control over the installation of the depth regulator complicates the dissection of the nail plate.
The inventions are aimed at solving the problem of creating such a method of treating an ingrown nail, in which the following principles are observed: separation of the first "purulent" and the second "clean" stages to ensure optimal healing conditions during the second stage - radical intervention; atraumatic execution of the first stage, for which a special surgical knife is used; performing during the second stage of resection of the skin and subcutaneous tissue of the lateral surfaces of the nail phalanx with two semi-oval incisions, and the resection of the skin and subcutaneous tissue is carried out in a significant volume so that tissue matching is achieved with some tension, for this one incision is performed close (1-4 mm) from the nail groove, and the other as far as possible from it; performing an intervention on two okolonogtevye rolls of the fingers of both feet in all cases, even if there was inflammation of only one of them.
The technical result of the group of inventions is the prevention of relapse of the disease and complications in the postoperative period, the prevention of the ingrowth of the nail plate on the opposite foot and the possibility of treating the ingrown nail on an outpatient basis.
The essence of the group of inventions is expressed in the aggregate of essential features, in which the method of treating an ingrown toenail, including resection of the edge of the nail plate at the first stage and excision of the skin and subcutaneous tissue with subsequent suturing and tightening the edges of the wound at the second stage, differs from the closest analogue in that that at the first stage, the nail plate is minimally invasively dissected longitudinally without injuring the nail bed with a surgical knife designed to implement the method, and the second stage is wire t in 1-2 months after the first stage and the complete abatement of inflammatory phenomena, while the soft tissues are excised along the lateral surfaces of the nail phalanx with internal and external incisions, the internal incision being performed at a distance of 1 to 4 mm from the edge of the nail plate, and the external one is can be further from her.
In a particular case, the second stage of the intervention is carried out simultaneously on two legs.
A surgical knife for implementing the above method, comprising a handle and a removable blade mounted to move along the guide groove of the tissue depth excision depth regulator having an end surface for supporting the tissue, differs from the closest analogue in that the tissue excision depth regulator is made in the form placed on the handle spring-loaded clamping plate connected by a threaded rod to a holder located on the guide groove and made to install the blade, the guide groove being Inonii end to a handle and is provided with a scale on the side surface and the threaded rod is oriented transversely to the longitudinal axis of the guide trough and has a wing nut mounted to cooperate with a spring-loaded clamping strap.
The essence of the group of inventions is illustrated by drawings, where in Fig.1 shows a side view of the handle of a surgical knife, in Fig.2 is a top view of the handle and in Fig.3 is a removable blade.
5.The surgical knife contains a
3.At the first stage of treatment of an ingrown toenail, the nail plate is minimally invasively dissected longitudinally without injuring the nail bed. Longitudinal dissection of the nail plate is performed with a special surgical knife, a
1.To install the
8.Then, after assessing the position of the
