Abstract
Description
The invention relates to medicine, in particular to dermatology and cosmetology.
The method of plasma therapy is considered one of the relevant and sought-after areas of development of modern aesthetic medicine. Plasma is a highly active biological environment, which includes various salts (electrolytes), proteins, lipid, carbohydrates, metabolic products, hormones, enzymes, vitamins and dissolved gases in it. Indications for the use of drugs based on autologous or allogeneic blood plasma are age-related changes in the skin of the face and body, photo-damage, acne, hypotrophic scars and striae, diffuse thinning of hair, etc.
A known method of rejuvenating the skin of the face and neck in women, including the intradermal introduction of their own native plasma into the face and neck (RF Patent No. 2405572, IPC A61M 1/34, A61K 35/16, A61P 17/00, publ. 2010). To perform the method, plasmapheresis is performed in the intermittent-flow separation mode with a sampling of 800-1000 ml of blood, at the same time intradermal administration of native plasma in a total volume of 5-10 ml is carried out.
The disadvantages of this method are its complexity, the need for the patient to conduct plasmapheresis procedures, requiring expensive equipment and time, as well as the low efficiency of the method due to the insufficient content of biologically active substances in the resulting plasma preparation. In addition, the method involves taking a sufficiently large volume of whole blood from a patient - up to 1000 ml, which may adversely affect a woman’s state of health.
A known method of stimulating reparative and trophic processes in the tissues of the body (RF patent No. 02095067, IPC A61K 35/14, publ. 1997), including the introduction of an autologous blood product obtained due to aggregation of platelets isolated from blood plasma under the influence of thrombin, and separation aggregates by centrifugation.
The disadvantages of this method is the need for manipulations associated with the need for additional stimulation of platelets with thrombin; as well as a decrease in the effectiveness of the drug due to the removal of thromboconcentrate, which in turn contains numerous biologically active substances and accordingly has pronounced biological activity, in addition, the authors recommended a parenteral route of administration, including intravenous injection of the drug.
Closest to the claimed is a method of stimulating reparative processes in the skin (Akhmerov R., Zarudy R., Rychkova I., Bochkova O. Plasmolifting - treatment of age-related atrophy of the skin with platelet-rich autoplasma // Aesthetic medicine. - 2011. - Volume X. - 2011. - Volume X , No. 2. - S.181-187). The method consists in the fact that blood sampling is carried out, centrifuged to obtain a plasma preparation rich in platelets, and then the resulting preparation is administered intradermally in a volume per course determined by the degree of the disease. In this method, venous blood in a volume of 36 ml is prepared in 4 test tubes with a volume of 9 ml each with an anticoagulant (sodium heparinate), then the blood is centrifuged twice to obtain BTP: first 5 minutes at a speed of 500-800 rpm, the erythrocyte clot is separated, and then the supernatant is centrifuged repeatedly at 1000 rpm for 5-7 minutes, divided into 2 fractions: plasma enriched in platelets and plasma depleted in platelets. The drug BoTP is injected into the dermis or subcutaneously.
The disadvantages of this method are the inability to evaluate the effectiveness of the procedure and to individualize the dosage of the BOTP drug administered to the patient, as well as the need for additional manipulations with platelet suspension associated with double centrifugation, which negatively affects the morphofunctional state of platelets and reduces the biological activity of BOTP.
The objective of the invention is to eliminate the above drawbacks, reduce the time to obtain the therapeutic drug BoTP, increase the effectiveness of therapy by assessing the biological activity of BoTP by the number of active platelets, as well as eliminating the need for additional equipment, expensive solutions and reagents.
For this, in a method of stimulating reparative and trophic processes in the skin, including blood sampling, centrifugation to obtain a plasma preparation rich in platelets, intradermal administration of the obtained drug in a volume per course determined by the degree of the disease, it is proposed to determine the number of active platelets Ta after centrifugation in the plasma preparation, and with a value of 60 <Ta <80%, administer 3-5 ml per session, with 40 <Ta≤60% administer 5-8 ml per session, with Ta≤40% administer 8-10 ml per session, with Ta≥80% inject 1-3 ml per session.
При этом известно, что тромбоциты в плазме существуют в нескольких функциональных состояниях (неактивные, активные, дегенеративные).The use of platelet-rich plasma (BTP) can significantly increase the effectiveness of the treatment. Moreover, it is known that platelets in plasma exist in several functional states (inactive, active, degenerative).
Under physiological conditions, the vast majority of platelets are inactive forms - flat, rounded cells of a discoid form - discocytes. In the process of activation, disk spheralization is associated with the rearrangement of the cytoskeleton and the structure of the cell granulomer, and the subsequent appearance of 1-3 short (less than the diameter of the cell) processes (with a low level of platelet activity) or 2-5 long (more than the diameter of the cell) processes of “antennas” "(With a high level of activity). These platelets form a group of functionally active cells. Platelets of irregular shape with an uneven tuberous surface, a large number of processes of various lengths and numerous vacuoles are classified as degeneratively altered cells that have exhausted their functional reserve.
The presence of a high concentration of active platelets that, through the ejection reaction, secretes a unique complex of biologically active compounds (growth factors, antimicrobial peptides, serotonin, histamine, etc.) can significantly enhance the therapeutic effect.
The rationale for using BOTP in the treatment of a number of dermatological diseases is the recorded normalization of keratinization processes; the manifestation of pronounced anti-inflammatory activity associated with an increase in the focus of inflammation of chemotaxis of granulocytes, macrophages and fibroblasts; antibacterial effect due to the presence in the plasma of the antimicrobial peptide defensin-3 in bacteriostatic concentration; an increase in the ability of monocytes to transform into macrophages and Langerhans cells; increased levels of RANTES (an antimicrobial peptide, a regulator of T-cell expression and secretion) and lipoxin A4 (an arachidonic acid metabolite inhibiting a number of leukotriene effects, a local increase in the concentration of the main growth factors represented by platelet-derived growth factor (PDGF), transforming growth factor (TGF), vascular endothelial growth factor (VEGF) and epidermal growth factor (EGF); optimization of regeneration processes that prevent the formation of post-acne scars and correct existing ones.
4.Figure 1, 2 presents phototrichograms before and after treatment, respectively (example 1); figure 3, 4 is the same, example 2; in Fig.5, 6 is a photo of a patient with acne before and after treatment, respectively (example 3); 7, 8 - the same, example 4.
The method is as follows.
In patients, to obtain BTP, they collect blood from the ulnar vein in a volume of 20-30 ml. After a single centrifugation at 1000 rpm for 5-7 minutes, platelet-rich plasma is obtained, which is subsequently used for its intended purpose. 100 μl of BTP are taken and, using a phase interference microscope, live platelets are examined in real time by analyzing the number of active platelets. On average, the total platelet count in the BoTP preparation is 0.8-1 × 10 6 / ml.
According to the features of the morphological form, the number of active Ta platelets in the plasma preparation is determined, and with a value of 60 <Ta <80% of the total platelet count, 3-5 ml per session are administered, at 40 <Ta≤60%, 5-8 ml are administered per session, when Ta≤40% is administered 8-10 ml per session, while Ta≥80% is administered 1-3 ml per session.
The course of treatment is from 3 to 6 procedures for 1 year with an interval of at least 3 weeks between procedures. However, in some cases, with a high content of active platelets in the drug, a pronounced effect is observed after the first procedure. The duration of the effect of BTP persists for 6-7 months.
The effectiveness of the proposed method for stimulating reparative and trophic processes in the skin is proved by the example of examination and treatment in the treatment of acne (37 patients), diffuse and androgenetic alopecia (65 patients), as well as in the correction of age-related skin changes (115 patients). The results obtained allow us to implement the principle of an individualized approach to the selection of a dose of BoTP, optimize the treatment of various conditions and avoid undesirable side effects.

The percentage of hair in the telogen stage (loss) was 24.5%, of which velus (fluff) hair - 58%.
Процент волос в стадии телогена составил 32%), из них веллусных - 65%.A 20 ml blood was prepared from the ulnar vein in a patient to obtain BTP, centrifuged at 1000 rpm for 7 minutes, and a plasma preparation enriched in platelets was obtained. 100 μl of BOTP was taken and live platelets were examined in real time on a phase-interference microscope: an analysis was performed to determine the total platelet count, which was 0.91 × 10 6 / ml, of which the active platelet count in the BTP was 73%. The patient was injected subcutaneously with 5 ml of BTP in 1 session. 5 days after the procedure, the cessation of hair loss was noted. After 3 sessions, conducted after 21 days, an increase in hair density to 200 / cm 2 was noted. The percentage of hair in the telogen stage was 32%), of which vellus - 65%.

Patient T., 19 years old. For the first time, acne on the face appeared at the age of 19. Regarding the treatment of acne, the following therapy was carried out: externally - zinerite (2 times a day for 1.5 months), to soften and prevent skin irritation - clersil lotion. The result of the treatment was insignificant. On a scale for assessing the severity of acne symptoms according to the method of C.H. Cook et al. a third degree of severity of acne was determined - on 1/2 of the face there are small papules, a small amount of pustular elements, the skin is irritated, infiltrates are single.
Obtaining BTP was carried out according to the above method. The total platelet count in BoTP was 0.8 × 10 6 / ml, the active platelet count was 38%. The patient was injected subcutaneously with 9 ml of BTP in the area around the inflammatory elements. Significant improvement in the condition of their skin was noted. The number of papular elements decreased, the infiltrates disappeared, hyperemia noticeably weakened (Figs. 5, 6). Repeated sessions were not required.

Patient R., 46 years old, complained of systematic rashes of papular-pustular elements, accompanied by pain that appeared in the perioral region in the last week of the menstrual cycle. To alleviate her condition, the patient used topical antibacterial agents with an anti-inflammatory effect. Locally on acne rash applied salicylic alcohol. As a hygienic care for oily skin daily used kerato - and seboregulatory cream with polyhydroxy acids (Keracnyl). However, the effect of the therapy did not last long.
C.H.Cook et al.).Given the mild severity of acne (no more than 10 papulopustular rashes - “mild” - according to the C.H. Cook et al. Scale for assessing the severity of acne symptoms).
Obtaining BTP was carried out according to the above method. The total platelet count in BoTP was 0.9 × 10 6 / ml, the active platelet count was 85%. The patient was injected subcutaneously with 1.5 ml of BTP in the area around the inflammatory elements. 10 days after the procedure, the patient noted a pronounced result of treatment. Clinical remission was expressed in the resolution of papules, the disappearance of dense subcutaneous infiltrates, leveling of the skin relief (Fig.7, 8). Subjectively, facial symptoms of oily seborrhea decreased.
Due to the good therapeutic effect and clinical remission that occurred after the first procedure (within 2 months from the time of the plasma therapy session, repeated rashes were not observed), the need for subsequent injections was no longer necessary.
The proposed method ensures the achievement of a good clinical effect and improves the aesthetic condition of the skin, and also allows you to remove the state of psychological discomfort and improve the quality of life of patients.
