Изобретение относится к области медицины, а именно к способам артроскопической классификации для диагностики форм дегенеративных поражений менисков коленных суставов.The invention relates to medicine, namely to methods of arthroscopic classification for the diagnosis of forms of degenerative lesions of the meniscus of the knee joints.
Известен способ магнитно-резонансной классификации повреждений менисков коленных суставов по Stoller D. W. et al, 1997 [1], который невозможно использовать для визуальной артроскопической диагностики дегенеративных поражений менисков. К сожалению, в доступных нам специальных литературных источниках, посвященных этой проблеме, нами не обнаружен запатентованный способ артроскопической классификации форм дегенеративных поражений менисков коленных суставов, позволяющей визуально оценить выраженность и распространенность их поражения патологическим процессом.A known method of magnetic resonance classification of injuries of the menisci of the knee joints according to Stoller D. W. et al, 1997 [1], which cannot be used for visual arthroscopic diagnosis of degenerative meniscus lesions. Unfortunately, in the special literature available to us devoted to this problem, we did not find a patented method for arthroscopic classification of forms of degenerative lesions of the meniscus of the knee joints, which allows us to visually assess the severity and prevalence of their lesions by the pathological process.
Цель изобретения - улучшение результатов артроскопической диагностики форм дегенеративных поражений менисков коленных суставов путем применения визуальной систематизирующей артроскопической классификации форм дегенеративных поражений менисков.The purpose of the invention is the improvement of the results of arthroscopic diagnosis of forms of degenerative lesions of the meniscus of the knee joints by applying a visual systematizing arthroscopic classification of forms of degenerative lesions of the meniscus.
Для решения данной проблемы нами предлагается следующий способ артроскопической классификации форм дегенеративных поражений менисков коленных суставов, основанной на анатомическом делении мениска на отделы и зоны кровоснабжения.To solve this problem, we propose the following method of arthroscopic classification of forms of degenerative lesions of the meniscus of the knee joints, based on the anatomical division of the meniscus into departments and blood supply zones.
В мениске коленного сустава различают три анатомических отдела: тело, передний и задний рог. Схема разделения мениска коленного сустава на отделы приведена на рис.1, где 1 - передний рог, 2 - тело, 3 - задний рог.Three anatomical sections are distinguished in the meniscus of the knee joint: body, anterior and posterior horn. The division scheme of the meniscus of the knee joint into departments is shown in Fig. 1, where 1 is the front horn, 2 is the body, 3 is the horn.
Анатомически в зависимости от уровня кровоснабжения в мениске коленного сустава условно выделяют три зоны: первая (бессосудистая), вторая (переходная) и третья (сосудистая). Схема условного разделения мениска коленного сустава на зоны кровоснабжения приведена на рис.2, где А - вид сверху, Б - вид спереди, I - бессосудистая зона, II - переходная зона, III - сосудистая зона.Anatomically, depending on the level of blood supply in the meniscus of the knee joint, three zones are conditionally distinguished: the first (avascular), the second (transitional) and the third (vascular). The scheme for conditionally dividing the meniscus of the knee joint into the blood supply zones is shown in Fig. 2, where A is a top view, B is a front view, I is an avascular zone, II is a transition zone, III is a vascular zone.
Выраженность дегенеративных поражений мениска коленного сустава мы разделили на степени:The severity of degenerative lesions of the meniscus of the knee joint we divided into degrees:
- I степень выраженности дегенеративного поражения мениска (дегенеративные изменения локализуются только в пределах первой зоны),- I degree of severity of the degenerative meniscus lesion (degenerative changes are localized only within the first zone),
- II степень выраженности дегенеративного поражения мениска (глубина поражения дегенеративными изменениями превышает пределы первой зоны, но не выходит за пределы второй),- II degree of severity of the meniscus degenerative lesion (the depth of the lesion with degenerative changes exceeds the limits of the first zone, but does not go beyond the second),
- III степень выраженности дегенеративного поражения мениска (глубина поражения дегенеративными изменениями в одном из анатомических отделов мениска превышает пределы второй зоны, распространяясь на третью),- III degree of severity of the degenerative lesion of the meniscus (the depth of the lesion by degenerative changes in one of the anatomical sections of the meniscus exceeds the limits of the second zone, extending to the third),
- IV степень выраженности дегенеративного поражения мениска (глубина поражения дегенеративными изменениями в двух анатомических отделах мениска превышает пределы второй зоны, распространяясь на третью).- IV degree of severity of the degenerative meniscus lesion (the depth of the lesion with degenerative changes in the two anatomical meniscus exceeds the limits of the second zone, extending to the third).
Типы дегенеративного поражения мениска коленного сустава мы разделили в зависимости от количества отделов, пораженных патологическим процессом:We divided the types of degenerative lesion of the meniscus of the knee joint depending on the number of departments affected by the pathological process:
- тип А - дегенеративные изменения локализуются в одном анатомическом отделе мениска,- type A - degenerative changes are localized in one anatomical department of the meniscus,
- тип В - дегенеративные изменения локализуются в двух анатомических отделах мениска,- type B - degenerative changes are localized in two anatomical meniscus,
- тип С - дегенеративные изменения локализуются в трех анатомических отделах мениска.- type C - degenerative changes are localized in three anatomical meniscus.
Способ артроскопической классификации форм дегенеративных поражений менисков коленных суставов:The method of arthroscopic classification of forms of degenerative lesions of the meniscus of the knee joints:
I степень выраженности дегенеративного поражения мениска, тип А: дегенеративные изменения локализуются в одном анатомическом отделе мениска, глубина поражения в нем не превышает пределы первой зоны (рис.3);I degree of severity of a meniscus’s degenerative lesion, type A: degenerative changes are localized in one anatomical meniscus, the depth of the lesion in it does not exceed the limits of the first zone (Fig. 3);
I степень выраженности дегенеративного поражения мениска, тип В: дегенеративные изменения локализуются в двух анатомических отделах мениска, глубина поражения не превышает пределы первой зоны (рис.4);I degree of severity of a meniscus’s degenerative lesion, type B: degenerative changes are localized in two anatomical meniscus, the depth of the lesion does not exceed the limits of the first zone (Fig. 4);
I степень выраженности дегенеративного поражения мениска, тип С: дегенеративные изменения локализуются в трех анатомических отделах мениска, глубина поражения не превышает пределы первой зоны (рис.5);I degree of severity of a meniscus’s degenerative lesion, type C: degenerative changes are localized in the three anatomical meniscus, the depth of the lesion does not exceed the limits of the first zone (Fig. 5);

IV степень выраженности дегенеративного поражения мениска, тип В: дегенеративные изменения локализуются в двух анатомических отделах мениска, глубина поражения в них превышает пределы второй зоны, распространяясь на третью (рис.12);IV degree of severity of a meniscus’s degenerative lesion, type B: degenerative changes are localized in two anatomical meniscus, the depth of the lesion in them exceeds the limits of the second zone, extending to the third (Fig. 12);
