Известно, что острые нарушения мозгового кровообращения (ОНМК) в связи с длительной иммобилизацией пациентов и тяжелыми двигательными нарушениями в виде парезов и параличей конечностей сопряжены с развитием таких осложнений, как гипостатические и аспирационные пневмонии, а также тромбоз глубоких вен (ТГВ) нижних конечностей и тромбоэмболия легочной артерии (ТЭЛА).It is known that acute cerebrovascular accidents (stroke) due to prolonged immobilization of patients and severe motor impairment in the form of paresis and paralysis of the extremities are associated with the development of complications such as hypostatic and aspiration pneumonia, as well as deep vein thrombosis (DVT) of the lower extremities and thromboembolism pulmonary artery (PE).

Как видно из вышеизложенного, в число важнейших задач реабилитации больных с тяжелыми двигательными нарушениями вследствие инсульта входит их ранняя активизация (мобилизация), способствующая стабилизации гемодинамических показателей, профилактике развития спастичности и осложнений раннего периода, а также подготовке больных к стоянию и к последующему обучению ходьбе. Это достигается, в том числе, и активным использованием поворотных столов-вертикализаторов. Однако у пациентов в острейшем периоде инсульта их использование ограничено в связи с частым развитием ортостатических реакций в виде постуральной гипотензии, сопровождающейся кратковременной потерей сознания. Причем, чем дольше продолжается период иммобилизации пациента, тем чаще возникают эти реакции при попытке перевести пациента в вертикальное положение. С целью облегчения процесса мобилизации больных с тяжелыми двигательными нарушениями фирмой Hocoma (Швейцария) был создан стол-вертикализатор Erigo, который, в отличие от классических поворотных столов, снабжен интегрированным роботизированным ортопедическим устройством, позволяющим одновременно с вертикализацией больного проводить интенсивную двигательную терапию в виде пассивных динамических движений нижних конечностей с возможностью циклической нагрузки на них. Предварительные исследования показали, что использование стола-вертикализатора Erigo в острейшей стадии тяжелого острого ишемического инсульта способствует более ранней активизации больных и их переводу в вертикальное положение (3). Средний срок перевода в вертикальное положение для больных основной группы, в которой мобилизация осуществлялась с его применением, составил 17,9+2,13 дня и для больных контрольной группы, в которой мобилизация проводилась классическим способом с применением кинезотерапии, - 32,1+11,2 дня (р<0,05). Следует также отметить, что при использовании стола-вертикализатора Erigo ни у одного больного не наблюдалось ортостатических реакций. По данным различных авторов стол-вертикализатор Erigo с успехом используется при активизации тяжелых, длительно иммобилизованных больных (кома, вегетативный статус и т.д.).Preliminary studies have shown that the use of the Erigo vertical table in the acute stage of severe acute ischemic stroke contributes to the earlier activation of patients and their translation into an upright position (3). Учитывая изложенное, был разработан способ профилактики гипостатической пневмонии и ТГВ у пациентов с инсультом, который заключается в использовании роботизированного стола-вертикализатора со встроенной системой пассивных шаговых движений. Преимуществами данного способа является меньшая частота развития осложнений иммобилизации у пациентов в остром периоде ОНМК, ранняя вертикализация пациентов, что может сократить время перехода пациентов из горизонтального положения (постельный режим) в вертикальное, а также специальный режим проведения процедур. Данный режим позволяет минимизировать риск развития осложнений, связанных с применением стола в остром периоде инсульта, и дозировать воздействие, ориентируясь на состояние пациента во время процедуры.Based on the foregoing, a method was developed for the prevention of hypostatic pneumonia and DVT in patients with stroke, which consists in using a robotic vertical table with an integrated system of passive step movements. Способ осуществляли следующим образом.The method was carried out as follows.
Процедуры начинали на 3-12 день после развития острого инсульта (в среднем, 7,5 дня). Сначала закрепляли пациента на столе при помощи ремней безопасности, затем прикрепляли каждое бедро больного к приводному механизму предусмотренными манжетами и фиксировали стопы пациента в вариабельных опорных подставках, оснащенных интегрированными пружинами, которые обеспечивают нагрузку во время разгибания нижней конечности. Для оценки эффективности и безопасности метода вертикализации на системе «Erigo» во время процедуры осуществлялось неинвазивное мониторирование ЧСС и АД (прикроватный монитор Siemens). Средние значения систолического артериального давления (САД), диастолического (ДАД) и частоты сердечных сокращений (ЧСС), измеренные непосредственно перед началами процедур вертикализации, принимались за фоновые значения.The procedures were started on 3-12 days after the development of acute stroke (on average, 7.5 days). После 4 минут пребывания больного на столе с наклоном в 40 градусов при стабильных гемодинамических показателях наклон стола уменьшали до 20 градусов и затем через 4 минуты стол переводили в горизонтальное положение. 1. Был обследован 61 пациент с ОНМК, поступившие в НЦН РАМП в течение первых 7 суток от начала заболевания. 38 пациентам (30 - с ишемическим инсультом (ИИ), 8 - с геморрагическим инсультом, средний возраст 65 [52; 72] лет, 16 женщин, 22 мужчины) помимо стандартной медикаментозной терапии, лечебной физкультуры и массажа паретичных конечностей назначался курс роботизированной активизации с помощью стола-вертикализатора Erigo (Hocoma, Швейцария). 23 пациента (16 - с ИИ, 7 - с геморрагическим инсультом, средний возраст 66 [56; 72] лет, 13 женщин, 10 мужчин) составили группу сравнения, в которой больным назначалась стандартная медикаментозная терапия, лечебная физкультура и массаж паретичных конечностей. Пациенты обеих групп были сопоставимы по тяжести инсульта: 79% пациентов основной группы и 74% пациентов группы сравнения имели среднюю тяжесть неврологических нарушений (оценка по шкале NIHSS от 6 до 15 баллов); 21% пациентов основной группы и 26% пациентов группы сравнения были оценены по шкале NIHSS в 16 и более баллов. Критериями оценки эффективности исследуемой методики служила частота развития пневмонии (диагноз выставлялся клинически на основании данных аускультации, признаков инфекционного заболевания (лихорадка, лейкоцитоз в общем анализе крови), рентгенография и/или компьютерная томография органов грудной клетки) и ТГВ (диагноз выставлялся клинически и верифицировался ультразвуковым дуплексным сканированием вен нижних конечностей) в течение 21 суток наблюдения за пациентами.1.
