Нами в качестве прототипа выбран способ шва М.Б.Замощина (1939) (Журнал Хирургия, 1939 г., №5, стр.123-124). Предлагаемый способ проиллюстрирован на фиг.1, 2, где фиг.1 - схема наложения шва, фиг.2 - связывание концов нити.The proposed method is illustrated in figure 1, 2, where figure 1 is a diagram of the stitching, figure 2 is the binding of the ends of the thread.
Способ заключается в следующем: отступя от края раны на 0,8 см, с помощью круглой атравматической иглы прокалывают паренхиму органа с выколом на противоположную сторону на расстоянии 1,2-1,5 см от края (фиг.1. а-а'), захватывая 1/3 глубины раны, потом возвращают нить, делают перехлест с первой нитью и прокалывают паренхиму еще раз кнаружи от первого вкола (фиг.1. а''-а'') на 0,5-0,8 см, проводя нить уже под дно раны с выходом на противоположную сторону, выводя нить снутри от верхнего выкола. Сначала подтягивают нить, проведенную глубже, надавливая пальцем в месте выхода верхней нити, потом и верхнюю с последующим связыванием концов нити (фиг.2). Данный способ проверен сначала на изолированных макропрепаратах печени, селезенки, почек. Надежность шва проверяли методом гидропрессии и пневмопрессии, потом апробировали в эксперименте на 17 собаках при ушивании ран печени, селезенки и почек.This method was tested first on isolated macro preparations of the liver, spleen, and kidneys. Ход операции: под тиопенталовым наркозом производили срединную лапоротомию. Выводили в операционную рану печень, селезенку или почку. Наносились раны на органах поочередно разной длины и глубиной 2/3 и более толщины органа. При этом отмечали кровотечение. Далее, отступя от края раны на 0,8 см, с помощью круглой атравматической иглы прокалывают паренхиму органа с выколом на противоположную сторону на расстоянии 1,2-1,5 см от края, захватывая 1/3 глубины раны. Потом нить возвращают и делают перехлест с первой нитью и кнаружи на 0,5-0,8 см от первого вкола еще раз прокалывают паренхиму органа, проводя нить уже под дно раны с выходом на противоположную сторону и выводя нить снутри от верхнего выкола. Сначала подтягивают нить, проведенную глубже, надавливая пальцем место выхода верхней нити, потом и верхнюю нить с последующим связыванием концов нити между собой.Operation progress: median laporotomy was performed under thiopental anesthesia. В качестве шовного материала используют кетгут (№0-2). На каждый орган наносили 2-3 раны, после обеспечения гемостаза наносили раны на другой орган.
