Способ заключается во введении в толщу межостистой связки позвоночника на уровне L1-L5 комплексной лекарственной смеси, состоящей из 32 ЕД лидазы, 4 мг дексаметазона, 100 мг лидокаина, 2 мл трамала, разведенных до 6 мл 40%-ной глюкозой. Кратность выполнения манипуляций - 5 с интервалом 48 часов. Противопоказанием для выполнения инъекций являются: непереносимость какого-либо препарата, входящего в состав лекарственной смеси, местные гнойные процессы в области выполнения инъекций. С целью оценки эффективности межостистых лимфотропных лимфостимулирующих инъекций был проведен сравнительный анализ историй болезни 61 пациента с лимфедемой нижних конечностей 1-4 стадии. Оценка эффективности проводимой терапии проводилась путем сравнительного анализа оценки уровня боли по визуальной аналоговой шкале, антропометрического исследования, реолимфовазографии (РЛВГ). В результате было доказано, что применение межостистых лимфотропных лимфостимулирующих инъекций в лечении больных лимфедемой нижних конечностей позволяет в 1,6 раза увеличить объемную и линейную скорости регионарного лимфотока, улучшить периферическое кровообращение за счет возрастания периферического венозного кровотока в 2 раза, позволяет достичь быстрого купирования болевого синдрома с 3,8 до 0,3 балла по визуальной аналоговой шкале (ВАШ). In order to evaluate the effectiveness of interspinous lymphotropic lymphostimulating injections, a comparative analysis of case histories of 61 patients with lymphedema of the lower extremities of stages 1-4 was carried out. As a result, it was proved that the use of interspinous lymphotropic lymphostimulating injections in the treatment of patients with lymphedema of the lower extremities allows a 1.6-fold increase in the volumetric and linear velocities of the regional lymphatic flow, an improvement in peripheral blood circulation due to a 2-fold increase in peripheral venous blood flow, and allows for rapid relief of pain from 3.8 to 0.3 points on a visual analogue scale (YOUR).
Ниже приводятся сравнительные оценки вышеизложенных показателей в числовом выражении. Контрольной группой послужили пациенты с такой же патологией, этой же возрастной группы и такой же степени тяжести течения заболевания, получавшие только традиционную консервативную терапию. В таблицах группа обозначена как "К", число наблюдений - 30 больных. Основная группа, обозначаемая в таблицах как "О" (число наблюдений - 31 пациент), сформирована из больных, которым помимо традиционного лечения были назначены межостистые лимфотропные лимфостимулирующие инъекции. Больная Ш., 64 лет, поступила 30.03.98 г. с жалобами на отечность левой нижней конечности; также беспокоят чувство тяжести, тянущие боли в конечности в вечернее время суток. Из анамнеза: отек развился 3 года назад после перенесенного острого тромбофлебита глубоких вен левой голени. Отек плотный, носит постоянный характер; отмечаются гиперпигментация, индуративные изменения кожи левой голени в нижней трети. Ранее больная проходила 2 курса комплексной консервативной терапии в клинике НИИК и ЭЛ; последняя госпитализация в сентябре 1997 г. Клинический диагноз: вторичная лимфедема левой нижней конечности 3 стадии. Данные антропометрии при поступлении (слева/справа, в см. ): стопа - 36/32; нижняя треть голени - 35/33; средняя треть голени - 44/40, верхняя треть голени - 49/47. При поступлении больная оценивала интенсивность болевых ощущений по ВАШ в 4,2 балла. По данным РЛВГ, проведенной 31.03.98 г., имелись выраженные различия параметров лимфатического и венозного оттока в левой конечности по сравнению со здоровой конечностью: объем периферической лимфы (слева/справа) - 0,13/0,26 Ом; скорость лимфатического оттока - 0,24/0,54 Ом/с; объем крови в периферических венах - 0,11/0,19 Ом; скорость тока крови в периферических венах - 0,47/0,50 Ом/с. По данным тепловизионного исследования от 1.02.98 г. отмечалась выраженная термоасимметрия в дистальных отделах нижних конечностей; разница кожной температуры в симметричных участках стоп составляла 1,70 град, на уровне нижней трети голени - 2,68 град, средней трети голени - 2,15 град, верхней трети голени - 1,11 град. Больной был назначен курс комплексной консервативной терапии (эластическое бинтование, элевация конечности, детралекс по 1 таб. 2 р/сут 20 дней, диазолин по 1 драже 1 р/сут 20 дней, УФОК N 3, кислородные ванны N 10, облучение конечности гелий-неоновым лазером N 10) в сочетании с межостистыми лимфотропными инъекциями, которые были выполнены трижды. В состав инъекции включали 32 ЕД лидазы, 4 мг дексаметазона, 100 мг лидокаина, 2 мл трамала, разведенных до 6 мл 40%-ной глюкозой. Перед выполнением первой инъекции больная оценивала уровень боли по ВАШ в 3,6 балла, через 1 час после ее выполнения - в 0,9 балла; 09.04.98 г. - в 2,1 балла. Вторая инъекция выполнена 10.04.98 г., уровень болевых ощущений оценивался больной перед ее выполнением в 1,7 балла, через 1 час - в 0,6 балла. Третья, проведенная 13.04.98 г. , позволила уменьшить интенсивность боли с 1,3 до 0,7 балла. За курс лечения достигнута отчетливая положительная клиническая динамика - больная отмечает уменьшение неприятных субъективных ощущений (уровень боли при выписке оценивался ею в 0,7 балла), отечность левой нижней конечности уменьшилась, отек стал более мягким. По данным антропометрии достигнуто уменьшение окружности конечности на уровне стопы на 3,0 см, на уровне нижней трети голени на 4,0 см, средней трети голени на 2,5 см, верхней трети на 2,0 см. По данным РЛВГ от 22.04.98 г. отмечается увеличение параметров периферического венозного и лимфатического оттока в пораженной конечности: объем периферической лимфы увеличился до 0,26 Ом, скорость лимфатического оттока - до 0,54 Ом/с, объем периферического венозного кровотока возрос до 0,19 Ом, скорость тока крови в периферических венах - до 0,50 Ом/с. Больная выписана на амбулаторный этап долечивания 22.04.98 г. Edema is dense, is permanent; hyperpigmentation, inductive changes in the skin of the left lower leg in the lower third are noted. Previously, the patient underwent 2 courses of complex conservative therapy in the clinic of NIIK and EL; last hospitalization in September 1997. Clinical diagnosis: secondary stage 3 lymphedema of the left lower limb. Anthropometry data at admission (left / right, in cm.): Foot - 36/32; lower third of the tibia - 35/33; middle third of tibia - 44/40, upper third of tibia - 49/47. At admission, the patient assessed the intensity of pain according to YOUR at 4.2 points. According to the RLVH data, performed on March 31, 1998, there were pronounced differences in the parameters of the lymphatic and venous outflow in the left limb compared with a healthy limb: the volume of the peripheral lymph (left / right) was 0.13 / 0.26 Ohm; lymphatic outflow rate - 0.24 / 0.54 Ohm / s; the volume of blood in the peripheral veins is 0.11 / 0.19 Ohm; the speed of blood flow in the peripheral veins is 0.47 / 0.50 Ohm / s. According to a thermal imaging study dated 1.02.98, pronounced thermal asymmetry was noted in the distal parts of the lower extremities; the difference in skin temperature in the symmetrical sections of the feet was 1.70 degrees, at the level of the lower third of the lower leg - 2.68 degrees, the middle third of the lower leg - 2.15 degrees, the upper third of the lower leg - 1.11 degrees. The patient was prescribed a course of complex conservative therapy (elastic bandaging, elevation of the extremities, detralex 1 tab. 2 r / day 20 days, diazolin 1 tablet 1 r / day 20 days, UFOK N 3, oxygen baths N 10, irradiation of the limb helium- neon laser N 10) in combination with interspinous lymphotropic injections, which were performed three times. The injection included 32 units of lidase, 4 mg of dexamethasone, 100 mg of lidocaine, 2 ml of tramal diluted to 6 ml with 40% glucose. Before the first injection, the patient estimated the level of pain according to YOUR at 3.6 points, 1 hour after its completion - at 0.9 points; 04/09/98 - 2.1 points. The second injection was performed on 04/10/98, the level of pain was evaluated by the patient before performing it at 1.7 points, after 1 hour - at 0.6 points. The third, carried out on 13.04.98, allowed to reduce the intensity of pain from 1.3 to 0.7 points. During the course of treatment, a clear positive clinical dynamics was achieved - the patient noted a decrease in unpleasant subjective sensations (the pain level at discharge was estimated at 0.7 points), swelling of the left lower limb decreased, the edema became milder. According to anthropometry, a limb circumference was reduced at the level of the foot by 3.0 cm, at the level of the lower third of the lower leg by 4.0 cm, the middle third of the lower leg by 2.5 cm, the upper third by 2.0 cm. According to the RLVG from 04.22. 98, there was an increase in the parameters of the peripheral venous and lymphatic outflow in the affected limb: the volume of the peripheral lymph increased to 0.26 Ohms, the speed of the lymphatic outflow to 0.54 Ohms / s, the volume of the peripheral venous blood flow increased to 0.19 Ohms, the current speed blood in peripheral veins - up to 0.50 Ohm / s. The patient was discharged to the outpatient phase of aftercare on 04/22/98.
