Abstract
Description
The invention relates to medical equipment, namely to otorhinolaryngology.
A device for the treatment of chronic tonsillitis (AS USSR 1457928, MKI A 61 H 1/06, 1989), comprising a transparent applicator body with a funnel at the end, an acoustic unit with a waveguide connected to an ultrasonic oscillator. To enhance the hydrodynamic effects in the area of the treated surface, the device is equipped with an electro-hydraulic probe in the form of a channel inside the applicator, moreover, this channel has a funnel-shaped guide screen at one end and the other end is connected to an electric pulse generator, the funnel is also connected to a channel located inside the applicator and connected to source of negative pressure through the on-off valve.
Treatment of chronic tonsillitis, carried out by simultaneous exposure to the tonsils with a medicinal substance, low-frequency ultrasonic vibrations and high-frequency pulsed liquid jets.
A disadvantage of the known device is the lack of a holder-handle, which creates inconvenience if it is necessary to hold it in the hand for a long time. Fixing the funnel due to the tight nozzle on the body is not reliable, which creates the possibility of its fall and the risk of asphyxiation. In addition, access to the tonsil when using this device is difficult.
As the closest analogue, a device for treating tonsils (a.s. The device is equipped with an ultrasonic waveguide located in the outer tube and made in the form of a rigid rod with a beveled end, and its other end is located in the holder and connected to the ultrasonic generator through an acoustic unit, and a damping element with holes is located at the end of the ultrasonic waveguide, located on the outer tube limiter with the ability to move.
However, the known device does not take into account the variation in the size of the tonsils and does not provide for removable nozzles. The funnel-shaped end of the outer tube is made of hard material, and the fittings are located in front of the holder, which can lead to injury to the mucous membrane of the oropharynx and the patient’s teeth. The location of all elements of the device, including the holder, on the same axis also creates inconvenience in work, lengthens the time of the procedure.
The objective of the invention is to provide a device for the treatment of chronic tonsillitis, which eliminates possible complications, provides ease of use and reduces the time of the procedure.
The essence of the invention lies in the fact that in a device for the treatment of chronic tonsillitis containing a holder, an outer tube with a nozzle at the end, an inner tube for supplying a medicinal substance and fittings, the holder is made in the form of a handle bent at an angle of at least 90 o , the outer tube has the working part is at least 15 cm long, and the other part is located inside the holder and is equipped with a fitting for connecting to the vacuum pump of the suction device, the tube for supplying the drug substance consists of two parts, one of they are placed inside the outer tube and made of metal, the other is made of medical PVC compound and is located inside the holder along its inner bend, on the side of which a locking element is mounted, containing a clip in the form of a roller mounted with the possibility of movement in the groove, while the nozzle is made removable from an elastic material - medical rubber, or silicone, or fluoroplastic, has an oval cross section and is attached to the working part of the outer tube by means of a threaded joint neniya. A polyurethane foam gasket is installed between the parts of the tubes inside the holder to ensure their stability.
Using the invention allows to obtain the following technical result.
The presence of a convenient holder, made in the form of a handle at an angle of not less than 90 o to the body, allows you to freely manipulate the device. The nozzle is made of soft elastic material - medical rubber, or fluoroplastic, or silicone, has a threaded connection to the outer tube, which eliminates damage to the soft tissues of the oral cavity and teeth, as well as its fall. The implementation of the removable nozzle provides for the possibility of its selection in size and shape corresponding to the palatine tonsils of the patient. In this case, there is no need to wash each lacunae in the tonsil.
Performing the procedure using the proposed device eliminates the aspiration and ingestion of mucus, pus, and caseosis by the patient, and prevents complications such as asphyxiation with subsequent tracheotomy due to the vacuum effect in the system that is created between the nozzle and tonsil tissue and allows the removal of pathological contents by aspiration. The time of the procedure on one amygdala is 10-15 seconds. All this creates the convenience for the doctor’s work and comfortable conditions for the patient, which is especially important when working with children with age-related psycho-vegetative features.
Improving the effectiveness of the treatment procedure is due to the design and principle of operation of the device, made with the possibility of attaching to an aspiration device.
The device operates on the basis of the differential pressure principle in the system. The pressure drop is created using a vacuum pump. Negative pressure in the tonsil system - the device allows for aspiration of pathological contents. Under the action of a vacuum, the drug is supplied to the tonsil. The therapeutic effect is achieved through repeated repetition of the aspiration procedure and the supply of the drug to the tonsil.

The operation of the device is as follows.
The patient is in the most comfortable sitting position. The procedure is carried out under local anesthesia with an aerosol of 10% lidocaine solution or without anesthesia. Two nozzles 6 are selected, corresponding in size and shape to the palatine tonsils of the patient. One of the nozzles 6 is fixed by screwing on the thread of the nipple 7 located on the working end of the outer tube 2. The device starts to work when the position of the roller 13 overlaps the lumen of the tube 3, in the locking element 11. The device is held by the handle 1, the working part is inserted into the patient’s oral cavity 4 of the outer tube 2 until the edges of the nozzle 6 come in contact with the arches of the palatine tonsil. Then, with the help of the foot pedal, the electric vacuum pump of the suction device is turned on. In this case, a vacuum of 0.2-0.3 kgf / cm 3 is created between the nozzle and the tissues of the palatine tonsil. The procedure is carried out in three stages: first, the pathological contents are aspirated from the surface of the tonsil, then the roller 13 is moved to the position that opens the lumen of the tube 3, and the drug substance is supplied from the reservoir to a predetermined dose, after which the remains of the drug substance and caseous masses dissolved in it are removed from palatine tonsil lacunae. After the procedure is completed, the suction device is turned off and the working end of the tube 2 with the nozzle 6 is removed from the patient's oral cavity. In the same way, another tonsil is treated.
