Abstract
Description
The present invention relates to medicine, namely to orthopedic dentistry.
According to the WHO, the prevalence of habitual dislocations and subluxations of the lower jaw is from 27 to 39% of dental patients. In this regard, the problem of treatment of this disease remains undoubted.
С. 567). Known apparatuses of Burgon V.I. (Author. Thesis. For the candidate of medical sciences. - Kiev, 1968, 19 pp.), Pomerantseva-Urban (Petrosov Yu.A. et al. - Diseases of the temporomandibular joint. - Krasnodar, 1996, p. 184-188), Schroeder (Gavrilov I.I., Scherbakov A.S. - Orthopedic dentistry. - Moscow: Medicine, 1984, p. 567).
The devices consist of a removable tire, functionally restricting the opening of the mouth with the help of pelots, abutting against the branch of the lower jaw.
Disadvantages: pelots are traumatic for the mucous membrane, kappa is poorly fixed on the upper jaw, not holding the lower jaw in the central occlusion position, in addition, they contribute to the excursion of the articular head to the left and left, which leads to even more relaxation of the muscular-articular apparatus of the temporomandibular the joint.
For the prototype, we took a device for the treatment of habitual dislocations and subluxations Kadrova S. (Khvatova V.A. Diseases of the temporomandibular joint. - M.: Medicine, 1982, p. 85-87). The main element of the device is kappa. Kappa is placed on the upper jaw. On the side surface of the mouthguard there is a pelot, abutting against the branch of the lower jaw, restricting the opening of the mouth.
The disadvantages of the prototype coincide with the disadvantages inherent in the above analogues. In addition, pelots create functional and aesthetic discomfort, cause constant pressure on the chewing muscle itself, and lead to facial asymmetries.
- повышение эффективности ортопедического лечения привычных вывихов и подвывихов нижней челюсти;
- обеспечение ограничения открывания рта и надежного удержания нижней челюсти в положении центральной окклюзии;
- устранение травматичности ветви нижней челюсти на период лечения;
- устранение экскурсии суставной головки при трансверзальных движениях;
- сокращение сроков лечения;
Tasks:
- increasing the effectiveness of orthopedic treatment of habitual dislocations and subluxations of the lower jaw;
- providing restrictions on the opening of the mouth and reliable retention of the lower jaw in the position of central occlusion;
- eliminating the trauma of the lower jaw branch during the treatment period;
- elimination of excursions of the articular head during transverse movements;
- reduction of treatment time;
- reduction in the number of relapses.
1 - каппа на верхней челюсти;
2 - каппа на нижней челюсти;
3 - выступы на каппе верхней челюсти;
4 - углубления на каппе нижней челюсти;
The essence of the invention is the supply of mouthguards on the upper jaw in the region of 6 teeth and in the frontal section along the midline of the face with protrusions and corresponding recesses in the lower jaw, and immediately beyond the tubercle of the upper jaw with thickenings to limit the opening of the mouth. The protrusions and recesses can be in various versions, including V-shaped, dimple, cylindrical. However, according to the applicants, protrusions and indentations along the contours of the shapes and sizes of natural teeth are a more effective option. The device is illustrated in the drawings (Fig. 1, 2, 3, 4), in which it is shown in vertical section along the midline of the face and which shows:
1 - kappa on the upper jaw;
2 - kappa on the lower jaw;
3 - protrusions on the upper mouth jaw;
4 - recesses on the mouth guard;
5 - thickening on the mouthguard.
The device is used as follows.
Individual pre-gingival mouthguards for the upper jaw 1, the
3, 4). The proposed device was tested in outpatient settings for 56 patients. At the same time, the use of mouthguards in the treatment of dislocations and subluxations of the lower jaw with any number of missing teeth (partial or complete), any topography: included (Fig. 1, 2) or distally limited defects (Fig. 3, 4) was a positive point.
30-40%. Using the device allows you to eliminate trauma, not to violate aesthetics, reliably fix the lower jaw from lateral (transverse) movements, reduce treatment time by 30-40%.
