Известны методы фармакодинамики, позволяющие исследовать и оценивать распространение, превращение и концентрацию лекарственных препаратов от места его введения до органов-мишеней. Другими словами, эти методы определяют зависимость функции органа или системы органов от концентрации препаратов (1, 2, 3). При этом анализ эффективности и ее оценка не выполняются методами фармакодинамики. В то же время эффективность или полезность могут не совпадать с концентрацией препарата, уменьшением или увеличением функции, метаболической активности тканей, органов или систем целого организма. Известен метод изучения фармакокинетики лекарственных препаратов, основанный на имитационном моделировании с использованием мультикомпартментальных фармакокинетических моделей (4). Мультиэкспоненциальные модели описывают фармакокинетику анестетиков (алфентанила, фентанила, пропофола, тиопентала и др.). Целью их использования является имитация концентрации лечебного препарата в сосудистом русле после инфузии различной продолжительности, обычно, определение времени снижения концентрации препарата на 50%. Фармакодинамические отношения связывают концентрацию с функцией. Уменьшением или увеличением функции, метаболической активности тканей, органов или систем организма как и концентрации препаратов в общем случае не совпадают с эффективностью или, более строго, полезностью действия препарата. Сам выбор не определяется этими методами и делается на основании личного опыта врача, интуиции и других знаний о полезности уменьшения, увеличения или более сложного изменения функции органа-мишени, то есть по существу не определяется алгоритмически вычисляемой количественной оценкой клинической эффективности действия фармакологического препарата. Поставленная задача достигается выделением основного патологического процесса, фиксацией его изменения под действием лечебного препарата. А именно формируется и затем идентифицируется на основе мониторного контроля интегрально-аналитическая оценка тяжести ведущего патологического процесса, выполняется процедура определения изменения тяжести патологического процесса в зависимости от действия препарата, вычисляется количественный показатель изменения тяжести в ответ на вид и дозу препарата. Больная К., 12 лет, поступила с диагнозом: единственный желудочек, стеноз легочной артерии. Наложен тотальный кавапульмональный анастомоз в условиях искусственного кровообращения длительностью 145 мин (аорта пережималась на 34 мин), гипотермии и сочетанной кардиоплегии. В раннем послеоперационном периоде - невыраженная сердечная недостаточность: сердечный индекс снижен на 20%, левопредсердное давление повышено на 24%, насосная способность сердца снижена на 50%. На фиг. 1 изображена диаграмма кровообращения больной К, 12 лет, после наложения тотального кавапульмонального анастомоза. Наблюдается умеренное возрастание сопротивления сосудов легких (ОЛС), вслед за этим снижается насосная способность сердца (КЛ) и вследствие этого - сердечный индекс (СИ). В качестве нормы (серым цветом) взято исходное, перед инфузией нитроглицерина, состояние кровообращения этой же больной. После инфузии снижается общее легочное сопротивление (ОЛС), общее периферическое сопротивление (ОПС), растет насосная функция сердца (КЛ), увеличивается сердечный индекс (СИ). Эффективность лекарств оценивалась оперативно, во время каждого комплексного измерения у каждого больного, что позволяло контролировать результат лечебных воздействий практически в реальном времени (изменение состояния сердечно-сосудистой системы в динамике подавалось в образном виде на экран каждую минуту). Кроме того, подсчитывалась статистика по нескольким измерениям до начала введения (или изменения дозы), и через некоторое время после. По этим данным оперативно, методами оптимизации, составлялись системные наборы и сравнивались в виде образов, где фоном в качестве "опорных значений" визуализировался образ кровообращения больного до введения лекарств. Эта процедура составляла около 3 мин дополнительно к анализу по комплексному измерению.
