Description
The utility model relates to medicine, to medical technology, namely to laparoscopic and robot-assisted surgery, urology, gynecology, transplantology, and is intended for dissection of soft tissues and organs in a sharp way with the possibility of automatic irrigation of the lavage fluid into the operation area without the need for additional assistance from an assistant and replacement of instruments .
In modern medicine, the prevailing number of operations is performed by minimally invasive methods. The use of laparoscopic and robotic-assisted operations can significantly reduce patient trauma, facilitate the postoperative period, and allow the patient to return to work earlier.
However, the minimally invasive nature of interventions imposes certain restrictions on specific stages of the operation, which leads to difficulties in their implementation. In some cases, these difficulties even lead to a complete rejection of laparoscopic and robotic-assisted methods and the implementation of some interventions only in the form of an open operation, or severely limit minimally invasive techniques.
One of these difficulties is the constant maintenance of the operation area in a "dry" form, that is, the exclusion of contamination of the surgical field with blood clots, urine and other biological fluids. This is the most important requirement, since the better the surgeon sees and evaluates the area of operation, the higher the probability of making the right decision and a favorable outcome of the operation for the patient.
In the case of an open operation, this problem is solved by washing the surgical wound with a sterile liquid, directly infusing the liquid through the incision and further drying the operation area with napkins or aspirating the washing fluid and blood with an aspirator.
In the case of minimally invasive surgery, the active use of napkins for drying the surgical wound is technically inapplicable due to the method. For these purposes, a tool is used - an irrigator-aspirator. It is a metal tube containing a switch to which two hoses are attached: one from a container with a sterile liquid, the other from a vacuum apparatus. If it is necessary to wash the operation area, the fluid is irrigated; if the wound is dried, aspiration is performed.
The greatest difficulties arise in the case of organ-preserving operations on parenchymal organs. The impossibility of completely shutting off the blood flow against the background of a good anatomical blood supply leads to irreversible persistent contamination of the resection area with blood. The continuous use of an aspirator in this case is unacceptable, as it leads to excessive aspiration of gas and a decrease in intra-abdominal pressure, which in turn exacerbates bleeding.
Therefore, in the case of resection of full-blooded parenchymal organs, in some cases, it is more acceptable not to episodic aspiration of blood, but to constantly irrigate the lavage fluid into the resection area, which allows the surgeon to maintain a good visualization of the resection area. To irrigate the surgical area with liquid in the case of minimally invasive surgery, it is possible to use an irrigator-aspirator.
The problem is that in the case of resection of a parenchymal organ, it is most expedient for the surgeon to use a laparoscopic clamp in one hand and scissors in the other, in order to be able to manipulate the tissues and directly perform the dissection. At the same time, during the operation, the surgeon constantly has to change instruments (clip on the aspirator-irrigator and vice versa) to irrigate the operation area with liquid, which significantly increases the operation time and leads to unnecessary manipulations. In addition, after washing the resection area, even in the case of a quick change of instruments (5-10 seconds), the surgical field in the incision area is refilled with blood and clots, which leads to the need to repeatedly change instruments and wash the wound.
Closest to the proposed is the scissors, curved along the
The disadvantage of the known instrument is the impossibility of its use for simultaneous irrigation of fluid into the wound and tissue dissection, which limits its functionality.
The technical result achieved by using the proposed utility model is to provide irrigation of the fluid to the operation area while maintaining the possibility of dissecting soft tissues and organs in a sharp way without the help of an assistant and changing instruments during laparoscopic and robotic-assisted operations.
The specified technical result is achieved by the fact that laparoscopic scissors with automatic irrigation of the liquid contain a tube, handles, branches, while the inner diameter of the tube is equal to the outer diameter of the fitting intended for attaching the irrigation system hose and the diameter of the hole in the tube located at the attachment point of two branches. Two valves are installed in the tube channel, one of which is connected to a button located on the fixed handle, and the second is directly connected to the movable handle and is activated when it moves relative to the fixed handle.
The proposed laparoscopic scissors make it possible to dissect soft tissues and organs in a sharp way with the possibility of automatic irrigation of the lavage fluid into the operation area without the need for additional help from an assistant and replacement of instruments.
The utility model is illustrated by drawings, in which:
On FIG. 1 is a schematic representation of laparoscopic scissors with automatic fluid irrigation.
On FIG. 2 and FIG. 3 is a schematic representation of laparoscopic scissors with automatic fluid irrigation while flushing the area of fluid resection and tissue dissection.
14.Laparoscopic scissors with automatic irrigation of the liquid include a
3).The device operates as follows (Fig. 2 and Fig. 3). Laparoscopic scissors with automatic fluid irrigation are inserted into the abdominal cavity through a trocar. The hose of the
14.Thus, in the condition of the
Thus, the device allows you to combine the function of scissors and an irrigator, while the presence of a shut-off valve allows the surgeon to arbitrarily turn on / off the irrigation function, and the automatic valve independently regulates the fluid supply depending on the stage of tissue cutting. This ensures the possibility of dissecting soft tissues and organs in a sharp way with automatic irrigation of the lavage fluid into the operation area without the need for additional help from an assistant and replacement of instruments. All of the above leads to a reduction in the time of the operation, a decrease in blood loss and an improvement in the immediate and long-term results of the operation.
