Abstract
The utility model relates to the field of medicine, specifically to traumatology and orthopedics, and can be used to treat degenerative-dystrophic and post-traumatic deforming arthrosis and other deforming injuries of the metatarsophalangeal joint. The objective of the proposed hemiendoprosthesis of the metatarsophalangeal joint is to obtain a stable clinical effect, which consists in reducing the time of postoperative healing and restoring the function of the metatarsophalangeal joint by restoring its mobility in the postoperative period and ensuring stable fixation of the stem of the hemiendoprosthesis in the first metatarsal bone due to high primary stability and early functional load on joint in the early postoperative period. The problem is solved by the proposed hemiendoprosthesis of the metatarsophalangeal joint, consisting of an articular head with a stem. The novelty of the proposed hemiendoprosthesis of the metatarsophalangeal joint lies in the fact that the distal articular head is made in the form of a convex hemisphere in a shape close to the anatomical shape of the metatarsal head, has a truncated shape on the lower surface, longitudinal and transverse grooves are made on the back of the distal articular head, and the stem made in the form of a clover leaf, has at least three longitudinal ribs, passes through the center of the back side of the distal articular head and is tilted upwards in relation to it. The clinical results showed a stable clinical effect, which consists in reducing the time of postoperative healing and restoring the function of the metatarsophalangeal joint by restoring its mobility in the postoperative period and ensuring stable fixation of the stem of the hemiendoprosthesis in the first metatarsal bone due to high primary stability and early functional load on the joint in the early period after the operation.
Description
The utility model relates to the field of medicine, specifically to traumatology and orthopedics, and can be used to treat degenerative-dystrophic and post-traumatic deforming arthrosis and other deforming injuries of the metatarsophalangeal joint.
Известен эндопротез первого плюснефалангового сустава стопы, который выполнен из инертной керамики и имеет проксимальный плюсневый компонент, состоящий из выпуклой проксимальной суставной части и ножки, и дистальный фаланговый компонент, состоящий из дистальной вогнутой суставной части и ножки (Патент № RU 202487 U1).Known endoprosthesis of the first metatarsophalangeal joint of the foot, which is made of inert ceramics and has a proximal metatarsal component, consisting of a convex proximal articular part and a stem, and a distal phalangeal component, consisting of a distal concave articular part and a stem (Patent No. RU 202487 U1).
However, this endoprosthesis of the first metatarsophalangeal joint of the foot cannot provide a stable clinical effect, since it does not allow stable fixation of the hemiendoprosthesis stem in the first metatarsal bone due to the fact that the stem of the endoprosthesis has a conical shape with roundings at the tops, while only longitudinal grooves are made on the stem. . The formation of a landing bed for the stem will lead to resection of a large amount of bone tissue, and the rounded conical stem with grooves will weaken the strength of its fixation in the first metatarsal bone and will not be able to provide rotational stability of the endoprosthesis stem. At the same time, the installation of the endoprosthesis provides for resection of both the articular surface of the head of the first metatarsal bone and the articular surface of the base of the proximal phalanx of the first finger, which is a traumatic intervention and increases the risk of loosening of the endoprosthesis components.
Known hemiendoprosthesis of the joint of the finger and all metatarsophalangeal joints of the foot, containing the articular head with the stem (Patent No. RU 149954 U1) - prototype.
However, this hemiendoprosthesis of the finger joint and all metatarsophalangeal joints of the foot cannot provide a stable clinical effect, since it does not allow stable fixation of the stem of the hemiendoprosthesis in the first metatarsal bone due to the fact that the stem has a conical and rounded shape, and the first human metatarsal bone has a spongy structure and small thickness of the cortical layer and the load on it is quite large. The formation of a landing bed for the stem will lead to resection of a large amount of bone tissue, and the rounded conical stem will weaken the strength of its fixation in the first metatarsal bone.
The objective of the proposed hemiendoprosthesis of the first metatarsophalangeal joint is to obtain a stable clinical effect, which consists in reducing the time of postoperative healing and restoring the function of the metatarsophalangeal joint by restoring its mobility in the postoperative period and ensuring stable fixation of the stem of the hemiendoprosthesis in the metatarsal bone due to high primary stability and early functional load on joint in the early postoperative period.
The problem is solved by the proposed hemiendoprosthesis of the first metatarsophalangeal joint, consisting of an articular head with a stem.
The novelty of the proposed hemiendoprosthesis of the first metatarsophalangeal joint lies in the fact that the distal articular head is made in the form of a convex hemisphere in a shape close to the anatomical shape of the metatarsal head, has a truncated shape on the lower surface, longitudinal and transverse grooves are made on the back of the distal articular head, and the stem is made in the form of a clover leaf with four petals, has 4 longitudinal ribs, passes through the center of the back side of the distal articular head and is tilted upwards in relation to it.
In FIG. 1 shows the claimed hemiendoprosthesis of the metatarsophalangeal joint (side view).
In FIG. 2 shows the claimed hemiendoprosthesis of the metatarsophalangeal joint (front view).
In FIG. 3 shows the claimed hemiendoprosthesis of the metatarsophalangeal joint (rear view).
The proposed hemiendoprosthesis of the first metatarsophalangeal joint contains a distal
Longitudinal and transverse grooves made on the back side of the distal articular head are designed to enhance the ingrowth of the hemiendoprosthesis into the patient's bone. The effect is achieved by increasing the surface contact and the formation of bone growth centers. The stem has the shape of a clover with four petals, 4 longitudinal ribs, passes through the center of the back side of the distal articular head and is tilted upwards in relation to it to stiffen the stem of the hemiendoprosthesis with minimal trauma to the bone tissue and prevent rotation around its axis as a result of the load force.
The proposed hemiendoprosthesis of the first metatarsophalangeal joint is used as follows: the articular surface of the metatarsal head is resected along the guide, a channel for the pedicle is formed in the metatarsal bone, and the distal articular head with the pedicle is installed. Passive movements are performed in the operated joint, its stability is checked, and then the capsule and the wound are sutured in layers.
The proposed utility model was used in 4 patients. Clinical results have shown a stable clinical effect, which consists in reducing the time of postoperative healing and restoring the function of the metatarsophalangeal joint by restoring its mobility in the postoperative period and ensuring stable fixation of the stem of the hemiendoprosthesis in the metatarsal bone due to high primary stability and early functional load on the joint in the early period after operation.
