Перед осуществлением папиллосфинктеротомии папиллотом вводят через папиллу в большом дуоденальном соске (БДС) на определенную глубину, натягивают нить с образованием подобия лука, включают ток и разрезают БДС. Для образования электрической цепи в устройстве применяют вспомогательный (пассивный) электрод, который в виде токопроводной пластины большой площади накладывают на поясничную область или конечность. Известное биполярное устройство содержит жестко закрепленный в торце электроизолированного корпуса неподвижный электрод, поверхность электроконтакта которого выполнена в виде прямоугольной пластины. Также в торцевой части корпуса закреплена направляющая для перемещения выполненного в виде пластины подвижного электрода вдоль продольной оси устройства, связанная посредством привода продольного перемещения с механизмом управления электроинструментом. Поставленная задача решается благодаря биполярному устройству для эндоскопической хирургии, содержащему неподвижно закрепленный в торце электроизолированного корпуса электрод и подвижный электрод, выполненный с возможностью перемещения вдоль оси устройства посредством привода продольного перемещения, также закрепленного в торце корпуса и связанного с механизмом управления, при этом электроды соединены с расположенными в корпусе проводниками, подключенными к источнику тока, в котором в соответствии с предлагаемым техническим решением в проксимальном конце бранши неподвижного электрода выполнено отверстие, в которое вставляют браншу подвижного электрода, изготовленного в виде буквы Т, основание которой, служащее токопроводом, расположено под углом к оси устройства и связано с приводом продольного перемещения, а крыша, часть которой является рабочей поверхностью электрода, жестко закреплена на основании параллельно оси устройства. При этом бранша неподвижного электрода выполнена с закруглениями на дистальном конце, а форма ее обусловлена анатомо-физиологическим особенностями оперируемого органа БДС. Основание подвижного электрода выполнено в виде усеченного конуса с расширением в дистальной части и с криволинейными боковыми поверхностями. Все элементы устройства покрыты электроизоляционным материалом, за исключением мест контакта электродов между собой. Верхняя поверхность подвижного электрода выполнена с миллиметровыми делениями. In this case, the jaw of the fixed electrode is made with curves at the distal end, and its shape is due to the anatomical and physiological features of the operated BDS organ. The base of the movable electrode is made in the form of a truncated cone with expansion in the distal part and with curved side surfaces. All elements of the device are covered with insulating material, with the exception of the places where the electrodes contact each other. The upper surface of the movable electrode is made with millimeter divisions.
