Abstract
Description
The invention relates to medicine, namely to dermatology, and relates to a method for the treatment of psoriasis.
A known method of treating psoriasis by photochemotherapy using puvalen photosensitizer, which involves oral administration of puvalen at the rate of 0.6 mg per 1 kg of body weight 2 hours before irradiation and subsequent UVA irradiation in a single dose of 2.5 to 12 J / cm 2 4 times week, total radiation dose 300 600 J [1, 2]
However, treatment with this method is lengthy (an average of 25 sessions of photochemotherapy), accompanied by frequent side effects (nausea, weakness, dizziness, etc.) caused by the actions of furocoumarins.
[3] (Ellis C. N. Treatment of psoriasis by etretinate// Arch. dermatol, 1982, vol. 118, N8, p. 555 558). The closest in technical essence and the achieved result is a method of treating psoriasis with tigazone in combination with photochemotherapy [3] (Ellis C. N. Treatment of psoriasis by etretinate // Arch. Dermatol, 1982, vol. 118, N8, p. 555 558).
The essence of the method is to take tigazone 75 mg per day for 14 days, followed by photochemotherapy according to a 4-day technique with puvalen photosensitizer and at the same time reduce the dose of tigazone to 50 mg per day to a state of clinical recovery.
Я. И. Ф. 1984, N 3, с.26-31). The disadvantages of this treatment method are its duration (6 10 weeks) and the presence of side effects (peeling of the skin, dry lips, short-term nosebleeds, liver dysfunction, etc.) caused by long-term use of tigazone and a significant number of photochemotherapy procedures (Aromatic retinoids in the treatment of dermatoses , Shakhtmeister I.Ya. Pokryshkin V.I. Pisarenko M.F. et al. // Vestnik Dermatol. 1984, No. 3, p. 26-31).
The aim of the invention is to reduce treatment time, prevent side effects and increase therapeutic efficacy, which is achieved by the appointment of photochemotherapy with the use of puvalen photosensitizer and in parallel with the use of 5% Aekol ointment prepared on a tizol ointment base on the lesion sites.
E. The active components of the ointment are vitamins A and E.
Own clinical studies have shown that the use of vitamins A and E externally in combination with long-wave ultraviolet radiation (wavelength 320 400 nm) causes a photodynamic effect, has a positive effect in psoriasis. Vitamins A and E act as photosensitizers, which allows to reduce treatment time, eliminate side effects, and increase the effectiveness of treatment.
The method is as follows. According to the generally accepted method, photochemotherapy is performed; after eating, the patient is prescribed oral administration of puvalen photosensitizer at a rate of 0.6 mg per 1 kg of body weight and after 2 hours, ultraviolet irradiation (wavelength 320 400 nm) is carried out in a single initial time dose of 3 minutes, with an increase in exposure time by 3 minutes after every 3 procedures up to 15 min; irradiation intensity from 1.5 to 7.5 J / cm 2 . Irradiation is carried out 1 time per day 4 times a week. In parallel, 1 hour before irradiation, topically, 5% Aekol ointment, prepared on the basis of tizol, is applied to the lesions on the skin.
A positive clinical effect was noted from the 3-4th procedure in the form of blanching of the elements of the rash, their gradual flattening, reduction, peeling. Irradiation is carried out to the state of complete clinical recovery of the patient with psoriasis, which means the complete disappearance of psoriatic plaques, the appearance of clean, even pigmented skin of varying degrees. No side effects and complications after treatment were noted.
Example 1. Patient S. 39 years. Suffers from psoriasis since 23 years. The aggravation of the process was noted 1 2 times a year, with a duration of 2-3 months, did not regress against the background of traditional therapy. In a hospital patient was treated with the proposed method. On the 4th day of therapy, blanching and flattening of elements were noted. The patient received 16 sessions of photochemotherapy and a course using 5% Aecol ointment, was discharged on the 31st day of hospitalization in a state of complete clinical remission. Side effects, complications were not noted. After treatment, remission lasts 8 months.
Example 2. Patient W. 42 years. Sick psoriasis for 16 years. Relapses of the disease were noted 2 3 times a year, lasting 3 4 months, alternating with very brief remissions 1 2 months. Traditional therapy did not have the desired effect. During the last torpid exacerbation of psoriasis, the patient was treated with the proposed method. Regression was noted after the 3rd photochemotherapy procedure and the use of 5% Aecol ointment. The treatment was not accompanied by side effects and complications. Remission lasts 9 months.
