Abstract
Description
The invention can be used to treat purulent diseases of soft tissues, for the treatment of which a comprehensive approach is used, including surgical treatment, supplemented by evacuation, treatment with a pulsating jet, laser, ultrasound, adequate drainage of the wound, as well as local treatment of the wound aimed at its early closure and general intensive care. As drugs for the treatment of purulent wounds, many drugs are used, most often of which are solutions of furatsilin, chlorhexidine, 10% sodium chloride solution, Vishnevsky ointment, furacilin, levorsin, iruxol and others. The disadvantage of these drugs is the long duration of treatment, the low rate of wound healing and a slight bactericidal effect.
One of the promising directions in finding new tools for local treatment of purulent wounds is the opportunities that arise in connection with the conversion of chemical industries, in particular, the study of chemicals of a group of degassers during the wound process and wound infection. As a degasser, a polydegassing formulation based on nitric acid lanthanum IPP-10 ("Langlik") of the following composition was used,
Lanthanum nitrate
6-water 5.6 ± 0.5
Triethylene glycol 65.5 ± 1.5
Ethyl Carbitol 25 ± 1
Sodium hydroxide 0.8 ± 0.1
Water 2.5 ± 0.5
Recipe "Langlik" IPP-10 (TU 6-02-3-287-84) is used for the degassing of skin infected with organophosphorus toxic substances (FOV) and mustard gas. It can be used after revealing the fact of infection by wiping the skin. Studies on acute toxicity and other characteristics of the drug were carried out before taking it for supply as a polydegassing formulation, for which there is a permission of the USSR Pharmaceutical Committee.
The drug "Langlik" is used by wetting gauze napkins and introducing them into the wound after first opening the abscess and carrying out its processing according to generally accepted methods.
The objective of the invention is the use of the polydegassing formulation "Langlik" as a means for the treatment of purulent wounds with bactericidal action and stimulating proliferative and reparative processes.
For the study, two groups of twenty people in each with various forms of purulent diseases of the soft tissues are presented.
The age of patients is from 16 to 49 years. Nosological forms of diseases are given in table. 1. All patients underwent autopsy, surgical treatment, and wound drainage. Subsequently, wounds were treated openly. In the 1st group for local treatment, a 10% solution of sodium chloride, a solution of furatsilin 1: 500, a solution of chlorhexidine were used. In the repair stage, dressings with Vishnevsky ointment, furatsilinova, levorsin, iruksol and others were used. In the 2nd group of patients, the drug "Langlik" was used from the moment the abscess was opened until the wound was completely healed. Dressings were performed daily using gauze napkins moistened with Langlik. When the inflammatory process subsided in the wound and the end of the exudation, bandaging was performed every other day. Due to the fact that the treatment of wound infection is a complex and multicomponent therapy, defined by the relevant rules and instructions, the identification of new effective drugs is possible only against the background of conventional methods of treatment. In the application materials this is confirmed by the presence of a control group in which only generally accepted treatment methods were used. In this regard, patients in both groups underwent drug therapy: antibiotics, infusion treatment, the introduction of protein preparations, sessions of UFO autologous blood, immunostimulants, etc. Physiotherapy was prescribed according to indications.
When conducting research, the following indicators were taken into account:
measurement of the area of the wound per unit time, the number of staphylococcus colonies per 1 cm 2 / wound,
total duration of treatment
clinical blood test
clinical urine test,
biochemical blood test (sugar, total protein: protein fractions, bilirubin, cholesterol, urea, ALT, AST).
Indicators were measured on
Y где Cо начальная площадь раны;
The area of the wound was measured by imprinting on sterile cellophane and then transferring it to graph paper. The wound healing rate was determined by the formula
Y where C o is the initial area of the wound;
C 1 area of the wound on the day of measurement.
The dynamics of changes in the number of staphylococcus colonies in was determined from the calculation that on the first day it was 100%
The course of the wound process was characterized by the severity of the inflammatory process in the 1st group, profuse serous-purulent exudation, progressive alteration processes during the first few days. For 3 patients in this group for 2-3 days there was a need for an additional opening of sagging and drainage of the wound. On average, the stage of exudation lasted in the first group from 6 to 14 days. The stage of proliferation until the complete healing of the wound lasted from 7 to 30 days or more. The processes of granulation, epithelization and scar formation proceeded without any features. The course of the wound process in the second group had a significant difference. The exudation phase lasted from 2 to 5 days, and the processes of an alternative nature were much less pronounced than in the first. Granulation and epithelization actively developed, and complete wound healing occurred on the 6-14th day.
The average wound healing rate in the 2nd group was higher than in the 1st group 2.2 times on the 3rd day, 1.9 times on the 7th day, 1.3 times on the 14th day (р≅0.05) .
A similar picture was observed in the study of microbial contamination of the wound.
The analysis shows that on the 3rd day the number of staphylococcus colonies in the 2nd group was 2.7 times less than in the first, on the 7th day by 8.9 times, on the 14th day by 24.4 times (p≅0.05). By the 14th day, staphylococcus in the 2nd group was sown in 3 patients, while in the 1st group it was sown in 9 patients.
е. The average duration of treatment in the 1st group was 21.1 ± 1.6 days, and in the second group, 9 ± 0.6 days, i.e. 2.18 times less (p≅0.01).
When studying the clinical and biochemical picture of blood and urine in the studied patients in both groups, changes inherent in the picture of the inflammatory process were noted, and did not have any specific features.
Thus, the conducted studies allow us to conclude.
With topical application of the drug IPP-10 "Langlik", the rate of wound healing was 1.8 times higher compared to the control group, and microbial contamination was reduced 12 times. Moreover, the total duration of treatment was 2.18 times less.
The use of the Langlik polydegassing formulation in the treatment of patients with purulent diseases of soft tissues has an advantage and can be used as an agent that suppresses alternative processes and exudation in the wound, stimulates proliferative and reparative processes in tissues and has a pronounced bactericidal effect.
