Abstract
Description
The invention relates to medicine, namely to ophthalmology, and can be used to treat degenerative diseases of the retina.
A known method of ultrasound therapy, which consists in the fact that the eye bath, hermetically screwed onto the head of the emitter, is filled with a solution of nicotinic acid. Under the influence of ultrasound, the drug is injected into the eye. The ultrasound intensity is 0.2-0.3 W / cm 2 the duration of the procedure is 5 minutes Treatment lasts for 10-12 days, 10 procedures for each eye. However, this method gives a short-term effect. The field of view expands after a course of treatment by only 3-7 about and remains so only for 2-3 months.
The purpose of the invention is to increase the effectiveness of treatment while reducing treatment time by normalizing lipid metabolism, increasing the mediator function of the brain, retina and improving microcirculation processes.
This goal is achieved by applying electrodes to the forehead and mastoid processes and applying direct electric current modulated by rectangular pulses with a frequency of 300 Hz at a pulse duration of 2-2.5 ms, current strength of 1-10-20 mA, the ratio of constant and variable components 1: 3 for 30-40 minutes, repeating the procedure daily for 5-10 days. In this case, electrodes of negative polarity are located in the forehead.
Experimental studies were performed on cats, which showed that transcranial electrical stimulation creates the greatest density of electric current in subcortical structures at a frequency of 300 Hz and a pulse duration of 2-2.5 ms.
According to reographic studies, these parameters are also the most optimal, since they contribute most to the increase in cerebral blood flow.
III). Based on these data, clinical studies have been conducted. In order to eliminate contraindications, the selection of patients was carried out on the basis of a thorough general neurological and instrumental (EEG, REG, ECHOEG) examination, excluding contraindications to the use of the method (epilepsy, brain tumors, hypertension III).
I". To implement the proposed method, a modified apparatus "Electronarkon-I" was used.
1:3. The method is as follows. Electrodes are applied to the forehead and mastoid processes through flannel pads moistened with water (or physiological saline if necessary to register REG). The electrodes located in the forehead are connected to the negative, the electrodes on the mastoid processes with a positive output pole of the generator. The patient, turning the knob of the current regulator of the apparatus, focusing on his sensations (light tingling, burning), sets the optimal values of the current strength, causing a state of comfort. As they adapt, these sensations disappear and the patient gradually increases the current strength until they appear, bringing it within 1-3 sessions to a tolerable limit not exceeding 20 mA. A current of greater strength causes pain in the majority of patients and an unwanted vasospasm. In most patients, complete adaptation, allowing you to set the required current strength, occurs within 2-3 sessions conducted daily. Within one day, one session is carried out, which lasts 30-40 minutes. The impact is produced by direct current modulated by rectangular electric pulses with a frequency of 300 Hz, a duration of 2-2.5 ms with a ratio of constant and variable components of 1: 3.
Optimal results were obtained after 5 procedures. In severe and neglected cases, the course of treatment can be repeated 1-2 times with an interval of 1 month, and the effectiveness of the treatment when the course is repeated increases. In between treatment courses, a gradual expansion of the visual field is observed. The implementation of the method is controlled by the study of the field of view, visual acuity, fundus picture, general neurological status, EEG, REG.
наличие грубой судорожной активности на ЭЭГ, даже без клинических проявлений эпилепсии;
наличие опухолевых процессов, выявленных клинически и инструментально;
Contraindications to the use of the method is:
the presence of severe convulsive activity on the EEG, even without the clinical manifestations of epilepsy;
the presence of tumor processes detected clinically and instrumentally;
hypertension with blood pressure above 200 mm Hg
Using the proposed method, 5 patients with retinal pigment dystrophy were treated.
The results of the clinical examination showed that the use of the method of transcranial electrostimulation allows without the use of drugs to positively affect visual function with retinal pigment dystrophy, and to expand the field of view. In very severe cases and in elderly patients, it is possible to use transcranial electrotherapy with the simultaneous use of vasodilators.
The advantage of the proposed method is also its high efficiency. The cost of treatment is determined only by the minimum energy consumption and the use of devices that have low cost.
